ASDR is nearing its 40 th year f peratin. In Durham Regin, it is estimated that 10,000 peple live with Alzheimer s disease r a related dementia with this number expected t duble in the next 15 years. The current Visin, Missin & Values f ASDR can be fund in the attached Appendix A. With a staff rster f 28, ASDR prvides educatin and supprt t persns living with dementia and his/her caregiver & family (1:1 and grup), public educatin, and scial/recreatinal prgramming. Our perating budget f $2.4 millin (2017-18) is funded thrugh the Central East LHIN, private dnatins and ther fundraising effrts. An rganizatinal chart and Bard rster has been included as Appendix B. PURPOSE ASDR is seeking prpsals frm qualified cnsultants and/r cnsulting firms t assist in the develpment f a fur-year cmprehensive strategic plan that will fcus n the rganizatin s values, services and visin fr the future. Strategic plans have been develped and are available frm the Alzheimer Sciety f Ontari (ASO) and the ASiO (the prvincial federatin cnsisting f the 29 lcal AS lcal chapters). The ASiO 3 P a g e
updated strategic plan will be available in May 2018. Our strategic plan must be cnsistent with these as well as meeting the current bjectives f the Central East LHIN and its Integrated Health Service Plan (IHSP). The successful candidate, in cnsultatin with the Bard and ur key stakehlders, will use the abve plans as a base t prepare a cmprehensive plan which wuld map ut the strategic directin fr ASDR fr the next 3-5 years. Our stakehlders include the Bard f Directrs, staff, clients and caregivers, ther Alzheimer chapters (particularly thse in the CE LHIN) and cmmunity partners. SCOPE The cnsultant shall, at minimum, accmplish the fllwing: Planning Activity and Final Deliverable 1. Design and execute a strategic visining and cmprehensive planning prcess including cnsultatin with ASDR Bard f Directrs, staff, users f ASDR services and key cmmunity partners 2. Develp an actinable fur-year strategic plan 3. Develp recmmendatins regarding the plan s implementatin and supprt structure Research It is anticipated that these tasks will be accmplished thrugh a cmbinatin f activities, such as: Fcus grups, interviews, and/r any ther methd that will be useful in receiving stakehlder and cmmunity input Facilitated grup meetings with partners t create cnsensus regarding a strategic plan (including gals, bjectives, strategies, and tactics) Review and present available related data and stakehlder strategic plans as well as any relevant research and best practices. ANTICIPATED PROCESS FOR STRATEGIC PLAN DEVELOPMENT ASDR seeks the cnsultant s recmmendatins regarding the best prcess t develp an actinable strategic plan t be cnducted in tw stages. Stage 1. Visin and key Strategic Directin and gals This stage includes cnsensus building amng the key infrmants t: a) Cnfirm that the current strategic visin and missin: Is still relevant Is clearly stated Is cmpelling Is timely Describes a clear and present need Mtivates peple t act Is a wrthwhile challenge Is audacius b) Identify key strategic directins & gals after cnsultatin with clients, inclusive f persns living with dementia and care partners, Bard f Directrs, staff,and key cmmunity partners, This stage shuld prduce the substantive dcumentatin and slutins needed fr infrming the strategic planning prcess. Stage 2. Write the strategic plan Utilizing infrmatin frm the first stage, this stage will include the develpment f an actinable strategic plan that will serve as the verall blueprint fr ASDR s actin plans fr the next three years. This plan shuld include: An Executive Summary f the main findings, including key recmmendatins numbered and pririty ranked A cmprehensive, detailed plan that identifies: Visin & Missin Key Strategic Directins Gals Objectives Strategies Tactics Stakehlders and their rles Measures Outcmes 5 P a g e

Criteria fr Evaluating Success f the Prject The Bard f Directrs will deem this a successful prject when they are given a clear reprt which utlines the strategic directins, areas f imprvement and a recmmended actin plan. The plan must build upn the Alzheimer Scieties in Ontari s 2018-2022 Strategic Plan and reference ther studies and plans. Previus strategic and ther plan materials will be prvided t the cnsultant as backgrund infrmatin upn request. References The prpsal shuld include three references f individuals wh can speak t their experience with the cnsultant in cnducting prjects f similar scpe. Infrmatin regarding each reference shuld include the individual s name, address, telephne number, and email address. Previus Wrk Prduct The prpsal shuld include at least tw examples f written wrks similar t the scpe f wrk requested within this RFP (e.g., strategic plan). APPLICATION REVIEW CRITERIA The prpsals will be reviewed and evaluated based n the fllwing criteria: 1. Qualificatins (35 pints) 2.
