US CUSTOMS FORMS Following are instructions for completion of the attached US Customs Forms. There are three forms; 1. Declaration for Free Entry of Unaccompanied Articles Form 3299 2. Supplemental Declaration for Unaccompanied Personal and Household Effects 3. Power of Attorney Note: There are five pages of instructions (including this one) and four pages of forms to complete PLEASE RETURN THE COMPLETED, SIGNED FORMS TO THE MOVING COMPANY AT THE TIME OF PICK UP AND KEEP A COPY FOR YOUR FILES. DECLARATION FOR FREE ENTRY FORM 3299 Item numbers correspond to the numbers on the form. Following are the definitions; IMPORTER: HOUSEHOLD GOODS: PERSONAL EFFECTS: Owner of the goods Furniture and durable items Items for personal use, recreation or pleasure i.e. clothing, cameras, stereo etc. A copy of the used pages of your passport and the appropriate US visas must accompany this form. PART I 1. Print your full name as it appears in your passport 2. Your date of birth 3. Your date of arrival in the US 4. Your current US address and telephone number 5. The city where you arrived and went through US Customs 6. The name of the airline and the flight number upon which you arrived in the US 7. The names of family members who accompanied you on your flight or will be joining you later 8. Parts A, B, C, D, E and F will be completed in the US by The Moving Company s destination partner PART II 9. Check the appropriate box A. Indicate the country where you most recently resided outside the US B. Indicate your length of residence overseas C. Check the appropriate box. If you are immigrating to the US, please attach a copy of your green card Note on Residency Status: Generally you are considered as a returning resident if, when you left the US, you intended to return as a resident. However, if you have lived abroad for over three years and no longer maintain connections with the US, you may be considered a non-resident. For Customs purposes, non-residents can import all personal effects purchased within the preceding 12 months. Household goods purchased within the last 12 months are dutiable for both returning residents and non-residents. 10. SUPPLEMENTAL DECLARATION FORM The Supplemental Declaration Form includes information regarding the owner of the goods (importer). It must be submitted with the 3299 Form. POWER OF ATTORNEY Most used household goods shipments can be cleared through US Customs with a completed 3299 Form and Supplemental Declaration. However, if there are dutiable items in the shipment, additional forms are required by Customs. The Power of Attorney form enables our US Customs Brokers to complete and sign these forms on your behalf. Any additional information required from you will be obtained by telephone or fax. The form will be kept on file with the Customs Broker. Item numbers correspond to the numbers on the form.
