TEAM MEMBER BENEFITS Benefits at Vidant Health Your benefits are a valuable part of the rewards of working here. To carry out the mission of the organization of improving the health and wellbeing of Eastern North Carolina, each year, Vidant Health invests more than $500 million in its employees through a comprehensive package that represents much more than just competitive pay and benefits. Through our competitive benefits program, we strive to demonstrate our commitment to you. Benefits Eligibility Team member s eligibility is based on Full-Time Equivalency (FTE). Team members are considered benefit-eligible if their FTE is.5 or greater. Benefit elections must be made within 30 days of hire/benefit-eligible status. Benefits become effective the first of the month following your 30 th day of hire/benefit-eligible status change. Team members must re-elect benefits during annual enrollment each year. Medical Eligible team members can choose from two self-insured health plans offering medical and prescription coverage for a wide range of services. The cost of the benefit is shared between employer and team member. Team member costs are listed on the next page. Wellness Additional Premium Team members enrolling in the medical plan who opt not to complete their WellScreen and/or Health Risk Assessment during stated times will pay an additional premium of $25 per pay period. Tobacco Additional Premiums Additional medical premiums of $40 per pay period will be charged if anyone covered under your MedCost plan is a tobacco user. Tobacco users include smoking, chewing, dipping, e- cigarettes, etc. Tobacco users that have enrolled in a tobacco cessation program may apply to have the additional tobacco premium waived. Team members must complete a certification stating that they are actively enrolled in a cessation program. To find out more about the tobacco cessation program, please contact Employee Wellness at 252-847-1389 or Chris Lee at clee@vidanthealth.com Spousal/Domestic Partner Additional Premiums If you cover your spouse/domestic partner under one of the medical plan options offered by the organization, you will pay an additional premium if your spouse or domestic partner is eligible for medical coverage through his or her employer. This $50 addition will be added to premiums you pay for your medical coverage. It does not apply when: You and your spouse/domestic partner are both team members of Vidant Health Your spouse/domestic partner has no group medical coverage available Your spouse/domestic partner is enrolled for Medicare coverage Your spouse/domestic partner is enrolled in TriCare

With this voluntary plan, you may use in or out-ofnetwork providers, but the level of benefit is higher when you receive care from an in-network provider. A listing of in-network providers can be found at www.superiorvision.com or by calling Superior Vision directly at 800-507-3800. Life Each full-time team member automatically receives employer-basic life insurance coverage equal to one times his or her annual base salary. Each team member eligible for part-time benefits will automatically receive employer-basic life insurance equal to $10,000. This benefit is 100% employer paid. Supplemental Life Team members may purchase additional life insurance coverage for themselves, their spouse, as well as their dependents. Whole Life Whole Life Insurance is designed to provide death benefits to you or your beneficiaries, but it can also build cash value that you can utilize while you are still alive. Flexible Spending Accounts (FSA) Flexible Spending Accounts are an easy way for you to keep more of your take-home pay by using pre-tax dollars for eligible expenses. Simply present your FSA debit card for the purchase of eligible services and goods. Using the debit card allows you to directly tap into your Health Care and Dependent Day Care FSA, meaning better cash flow for you and no waiting for reimbursement.

Disability Short-Term Disability Vidant provides 60% income replacement for full-time team members at no cost after a seven-day waiting period for up to 90 days when absence is due to a qualified illness or injury. New team members are eligible to elect an additional 20% income replacement at a cost. Long-Term Disability This benefit replaces 50% of base pay after a 90-day waiting period when absence is due to a qualified illness or injury. Retirement Savings Plan 401(k) Partnership Savings Plan - The 401(k) Partnership Savings Plan allows team members to set aside retirement savings on a pre-tax basis through payroll deduction. Employer-paid matching contributions are available after meeting one year of service with Vidant Health. Critical Illness Group Critical Illness insurance is designed to help you offset the financial impact of a covered critical illness. Some examples of a critical illness would be heart attack, stroke, major organ transplant, cancer, etc. Depending on the benefit amount you choose, it pays a tax-free lump sum benefit directly to you (unless otherwise assigned) to use any way you wish, in addition to any other coverage. Paid Time Off (PTO) Paid Time Off (PTO) is a bank of hours that provides you flexibility in receiving pay while away from work. Each pay period, benefit-eligible team members accrue PTO hours based on their hours worked, length of service and status. PTO can be used for vacation, holiday, religious observances, personal/family business, personal/family illness, or as best fits a team member s current life needs. Employee Assistance Program (EAP) Employee Assistance Program (EAP) services are available to help team members and their immediate family members meet the challenges they face in everyday life. EAP services offer FREE & CONFIDENTIAL guidance. Services are available Monday through Friday, 8:30 am to 4:00 pm. Tuition Reimbursement The organization recognizes the value of education and is committed to helping team members gain jobrelated education that enhances the team member s current skills or develops skills for future job opportunities within the system. Tuition reimbursement is available for benefit-eligible team members that have completed at least six months of service with Vidant. Tuition Reimbursement Amounts Full-time Part-time Associate Degree $4,000 $2,000 Bachelor Degree $4,000 $2,000 Graduate/Professional $5,000 $2,500 Adoption Assistance Program Team members can receive up to $2,500 per child, with a maximum of $5,000 in a budget year towards financial assistance to reimburse eligible expenses associated with the adoption of a child. Eligible expenses include legal and court fees, temporary foster care charges, travel expenses associated with the adoption, counseling, placement, and agency fees.
