UTAH DWELLING FIRE PROGRAM UNDERWRITING GUIDELINES DEDUCTIBLE: Base rates contemplate a $500.00 deductible per occurrence. POLICY TERM: Policies will be issued for a maximum term of 12 months. Any other terms should be submitted to the company. MINIMUM PREMIUM: The minimum a policy may be issued for is $200 per policy. CANCELLATIONS/MINIMUM EARNED PREMIUM: All policies are subject to Pro Rata cancellation. NO FLAT CANCELLATIONS PERMITTED. $25 fee assessed for mid-term cancellations at insured s request. Minimum earned premium applies. Minimum earned premium is $50 per policy and is applicable to bound and issued policies. APPLICATION: A current complete Acord Dwelling Fire Application shall be obtained on submissions prior to binding or policy issuance. All applicable information should be provided including the current loss history for the past 3 years. INSPECTIONS AND/OR PHOTOS: Photos are required on all new business. As a general rule, the company will conduct property inspections and obtain photos. Photos may be required of the agency for Dwellings located in outlying areas. WOODSTOVE/SPACE HEATERS: Woodstove questionnaires and photos are required on all new business with woodstoves as primary source of heating. Proof of Certified installation contractors is also required. SEASONAL DWELLINGS: Require minimum of 6 month occupancy and must be owner occupied only. No Vandalism or Malicious Mischief or Contents coverages offered. LIMITS OF LIABILITY: Section I Maximum Minimum Coverage A Dwelling $700,000 $10,000 Coverage B Other Structures Coverage C- Personal Property Coverage D- Fair Rental Value 10% of dwelling limit $50,000 Max Per policy conditions Section II Coverage L-Personal Liability $25,000 - $500,000 Coverage M-Medical Payments $1000 per person ELIGIBILITY: Owner or tenant occupied dwellings, with up to four families or four apartment units. The Insured Premises must be located in the State of Utah. INELIGIBLE RISKS: Vacant or unoccupied dwellings. Appurtenant structures without Dwellings Contents for Tenants only. Mobile Homes, Modular Homes or any Commercial use properties. House Boats. Farms or Barns. Risk with vicious or guard dogs. Dwelling with more than two (2) Mortgages Applicants with dwellings involved in bankruptcy or foreclosure proceedings. Dwelling or appurtenant structures with existing damage from a prior loss, unless specifically excluded. Risks of sub-standard construction. Risks with poor housekeeping and maintenance conditions. Homes with open foundations. Homes not built by a licensed contractor. (Contractor must have minimum of $1M CGL policy for homes in the course of construction.) Unusual liability exposure (e.g. unfenced pools, stairs with no handrails, etc.). Dwellings on a slope of 35 degrees or more. Dwellings built on piers or posts. Dwelling which are architecturally unique (e.g. geodesic, earthen, log homes). Dwellings with less than 1000 square feet of living area. SUBMIT ITEMS: Risk with a loss in excess of $10,000 of two or more losses within the last two years. Schedule of dwellings where total limits are in excess of $500,000 Course of Construction is provided and the insured is the contractor. Liability limits in excess of $300,000. DWELLINGS One to four family only. Owner or tenant occupied. CONTENTS Contents may be written without the dwelling for owner occupied dwelling only. Coverage is not available for seasonal dwellings. PERSONAL LIABILITY May be written for owner and tenant occupied dwellings. PREMISES LIABILITY Non-owner occupied one to four family units and seasonal dwellings. Not available for monoline coverage. COURSE OF CONSTRUCTION BUILDERS RISK Coverage available for owner occupied dwellings and work must be completed by a licensed contractor. (DP1 only) SEASONAL DWELLINGS May be written for owner occupied dwellings only. Must be occupied a minimum of six months annually. No coverage available for Vandalism & Malicious Mischief or Contents. VANDALISM & MALICIOUS MISCHIEF May be written on owner or tenant occupied dwellings. Coverage is not available for seasonal dwellings. BINDING FOR RISKS WITH PRIOR LOSSES Any risk with prior claims in the last three (3) years requires approval from the underwriting department prior to binding. In order to received approval, a quotation must be submitted through INSURE P.A.S. and a copy of the completed underwriting property questionnaire must be submitted via fax along with the following information: date or dates of prior losses, approximate amount for each loss, type of damage thatcaused the loss, and a detailed description of what has been done to mitigate future claims from this loss, i.e. replaced roof/plumbing. BINDING FOR OLDER DWELLINGS Older homes must have the following renovations: Over 30 years of age- New roof within the past 20 years Homes prior to 1960 must have updated Electrical system- 100 Amp or better and circuit breakers. Homes prior to 1945 must have updated plumbing system- no lead or galvanized pipes for incoming water.
