MOTOR INSURANCE (MOTORCYCLE NON CONSUMER) Product Disclosure Sheet (Please read this Product Disclosure Sheet before you decide to take out this insurance. Be sure to also read the general terms and conditions.) 1. What is this product about? This policy provides Comprehensive cover, Third Party, Fire & Theft cover and Third Party cover. What is the Period of Cover and Renewal Option? Duration of cover is for one year. You need to renew the insurance cover annually. 3. How much premium do I have to pay? The total premium that you have to pay may vary depending on the sum insured of the vehicle, cubic capacity, noclaim-discount (NCD) entitlement and the underwriting requirements of the insurance company. What is NCD? NCD is awarded if no claim was made against your policy during the preceding 12 months of policy. Your NCD entitlement will depend on the class of your vehicle. 4. What are the fees and charges I have to pay? What are some of the key terms and conditions that I should be aware of? a) Insurable Interest you are required buy cover as soon as you purchase a vehicle. b) Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for a purpose related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. In addition to answering the questions in the Proposal Form (or when you apply for this insurance), you are required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. c) You must ensure that your vehicle is insured at the appropriate amount as it will affect the amount you can claim. If sum insured is less than the market value, you are deemed as self-insuring the difference. In the event of a loss, you will only be partially compensated. d) Compulsory excess - that is the amount of loss you have to bear. e) Contribution we shall only be liable for our rateable proportion of the loss in the event an insured has more than one policy to cover their vehicle. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy. 6. What are the major exclusions under this policy? This policy does not cover certain losses, such as: Your own death or bodily injury due to a motor accident; (unless You purchase for our optional benefit, Motorcyclist Personal Accident Add-On, by paying an additional premium with the coverage of death or bodily injury caused by motor accident); Your liability against claims from pillion in your vehicle; (unless You purchase for our optional benefit, Passenger Liability Cover to Pillion, by paying an additional premium with the coverage for liability against claims from pillion rider); Loss or damage to your vehicle caused by act of nature i.e. flood, typhoon, hurricane, storm, tempest, volcanic eruption, earthquake, landslide, landslip, subsidence or sinking of the soil / earth or other convulsions of nature. (unless You purchase for our optional benefit, Inclusion of Special Perils, by paying an additional premium with the coverage for loss or damage to your vehicle caused by act of nature); Theft of non-factory fitted vehicle accessories unless otherwise declared. Consequential loss, depreciation, wear and tear, mechanical or electrical breakdown, failures or breakages. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy. 2

7. Can I cancel my policy? You may cancel your policy at any time by giving written notice to the insurance company. Upon cancellation, you are entitled to a refund of the premium based on short period rates, where any minimum premium paid under the policy is not refundable. 8. What do I need to do if there are changes to my contact details? It is important that you inform us if there are changes in your contact details to ensure that all future correspondences reach you in a timely manner. 9. What you should know when making a claim? Report to the police within 24 hours and immediately notify your insurance company with full details in writing. Repairs must be conducted by an Authorized Panel Workshop selected and approved by us if you make an own damage claim against your own comprehensive policy. If you are not at fault in the accident, you can submit the claim either directly to the insurance company of the party at fault, or if you have a comprehensive policy, you are encourage to submit to your insurance company for speedier claims processing without losing your NCD entitlement. You will lose your entire NCD entitlement once an own damage or a third party claim is made against your policy. 10. Where can I get further information? 4. Apakah yuran dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada premium Jumlah Cukai Barangan dan Perkhidmatan (CBP) 6% daripada premium Duti setem RM 10.00 Apa yang termasuk dalam premium Komisen yang dibayar kepada pengantara insurans (jika ada) Premium hendaklah dibuat sebelum perlindungan boleh diberikan. Apakah terma dan syarat utama yang saya harus ketahui? a) Kepentingan Boleh Insurans pastkan anda membeli insurans sebaik sahaja anda membeli kenderaan. b) Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apa-apa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan diatas hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami (atau semasa permohonan insurans ini), apa-apa maklumat yang dinyatakan dalam Borang Cadangan tidak tepat atau sudah berubah. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami (atau semasa permohonan insurans ini), apa-apa maklumat yang dinyatakan dalam Borang Cadangan tidak tepat atau sudah berubah. c) Anda harus memastikan kenderaan anda diinsuranskan pada amaun yang berpatutan, kerana ia akan memberi kesan ke atas jumlah yang anda boleh tuntut. Jika nilai diinsuranskan rendah daripada nilai pasaran, anda dianggap menginsuranskan sendiri perbezaan tersebut. Jika berlaku kerugian, anda akan dibayar hanya sebahagian daripada ganti rugi. d) Lebihan wajib sebagai amaun kerugian yang anda harus tanggung sendiri. e) Sumbangan - syarikat insurans akan hanya bertanggungjawab mengikut perkadaran setimpal bagi kerugian dalam peristiwa yang diinsuranskan jika anda mempunyai lebih daripada satu polisi yang melindungi kenderaan anda. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi terma dan syarat di bawah polisi ini. 6. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak melindungi sesetengah kerugian, seperti: Kematian atau kecederaan diri anda sendiri yang disebabkan oleh kemalangan motor; (kecuali Anda membeli manfaat pilihan kita, Kemalangan Diri Penunggang Motosikal, dengan membayar premium tambahan untuk melindungi kematian atau kecederaan diri yang disebabkan oleh kemalangan motor); Liabiliti terhadap tuntutan daripada penumpang di dalam kenderaan anda; (kecuali Anda membeli manfaat pilihan kita, Liabiliti dari Segi Undang-undang terhadap Pembonceng, dengan membayar premium tambahan untuk melindungi liabiliti terhadap tuntutan daripada penumpang); Kerugian atau kerosakan akibat kejadian semula jadi seperti banjir, taufan, hurikan, rebut, rebut kencang, letusan gunung berapi, gempa bumi, tanah runtuh, gelinciran tanah, penurunan atau penenggelaman tanah ataupun malapetaka alam yang lain; (kecuali Anda membeli manfaat pilihan kita, Perlindungan Tambahan Bencana Khas, dengan membayar premium tambahan untuk melindungi kerugian atau kerosakan terhadap kereta Anda akibat kejadian semula jadi); Kecurian aksesori kenderaan yang bukan dipasang di kilang, melainkan sekiranya diisytiharkan. Segala kerugian turutan, susut nilai, haus dan lusuh, kegagalan/kerosakan mekanikal atau elektrik Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kepada kontrak polisi untuk senarai penuh bagi pengecualian di bawah polisi ini. 2

7. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi anda pada bila-bila masa dengan memberi notis bertulis kepada syarikat insurans. Sewaktu pembatalan, anda layak untuk mendapat pengembalian premium berdasarkan kadar jangka pendek, dimana sebarang premium minima dibayar di bawah polisi ini tidak boleh dipulangkan. 8. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran peribadi saya? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 9. Apa yang anda harus tahu apabila membuat tuntutan? Buat laporan polis dalam masa 24 jam dan maklumkan syarikat insurans anda secara bertulis dengan memberi butirbutir lengkap dengan segera. Pembaikan hendaklah dikendalikan oleh bengkel panel yang dipilih dan diluluskan oleh kami sekiranya anda membuat tuntutan kerosakan sendiri terhadap polisi komprehensif anda sendiri. Sekiranya anda bukan pihak yang bersalah dalam kemalangan tersebut, anda boleh mengemukakan tuntutan sama ada secara terus kepada syarikat insurans pihak yang bersalah atau, sekiranya anda mempunyai polisi komprehensif, anda digalakkan mengemukakan tuntutan anda kepada syarikat insurans anda sendiri untuk mempercepatkan pemprosesan tuntutan tanpa kehilangan kelayakan DTT anda. Anda akan kehilangan keseluruhan kelayakan DTT anda apabila tuntutan kerosakan sendiri atau pihak ketiga dibuat terhadap polisi anda. 10. Di manakah saya boleh dapatkan maklumat lanjut?
