ACCIDENT SUPPORT REPAIR PLUS + Product Disclosure Sheet (Please read this Product Disclosure Sheet before you decide to take up this insurance. Be sure to also read the general terms and conditions.) 1. What is this product about? This policy is an additional purchase (as a standalone coverage) on top of Your Private Motor Comprehensive Insurance Policy and provides Out-of-Pocket Motor Accident Related Expenses which is not covered under Your Private Motor Comprehensive Insurance Policy. 2. What are the covers / benefits provided? (i) Benefit (A) SPRAY PAINTING OF WHOLE VEHICLE (Reimbursement limit as specified in the schedule) Coverage is invoked in the event of a Motor Accident that triggers an Own Damage claim under Your Corresponding Private Motor Comprehensive Insurance Policy involving repainting of the damaged parts but does not allow for the spray-painting of the whole vehicle in its repair. Note: Duration of cover is for one year. You need to renew the insurance cover annually together with Your Private Motor Comprehensive Insurance Policy. 3. How much premium do I have to pay? The total premium that you have to pay depends on the benefits you have selected. 5. What are some of the key terms and conditions that I should be aware of? Your Vehicle must be insured with us on a Private Car Policy with comprehensive insurance cover concurrently with this policy. The age of Your Vehicle does not exceed fifteen (15) years. You must either be a corporation, association or an individual person between the age of twenty four (24) years and sixty five (65) years. Consumer Insurance Contract Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance wholly for purposes unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form (or when you apply for this insurance). You must answer the questions fully and accurately. Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. In addition to answering the questions in the Proposal Form (or when you apply for this insurance), you are required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. Non-Consumer Insurance Contract Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for a purpose related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. You can only make a claim under this policy if you are also making a claim under Your Private Motor Comprehensive Insurance Policy. Upon settlement of a claim under any of the benefits within this policy, that particular benefit terminates automatically unless you reinstate this policy by paying an additional premium to us. High performance vehicles such as Porsche, Lamborghini, Lotus, Jaguar, etc. are NOT to be accepted for benefit (B) Reimbursement of Betterment Cost. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy. 6. What are the major exclusions under this policy? This policy will not entertain any claim(s) under any of the following situations: If the claim under section A of Your Corresponding Private Motor Comprehensive Insurance Policy is repudiated or declined. Accident that results directly or indirectly from an illegal act or a criminal offence. Accident whereby the vehicle is being test driven or where the driver is under the influence of alcohol or drugs to such an extent as to be incapable of having control of Your Vehicle. Accident arising directly or indirectly from flood, storm, landslip, land subsidence, strike, riot & civil commotion unless such claim is also payable under Your Corresponding Private Motor Comprehensive Insurance Policy. 2

Accident arising from defective design or defective repairs. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of the general exclusions under this policy. 7. Can I cancel my policy? You may cancel your policy at any time by giving written notice to the insurance company. Upon cancellation, you are entitled to a refund of the premium based on short-period rates provided that you have not made a claim on the policy. 8. What do I need to do if there are changes to my contact details? It is important that you inform us if there are changes in your contact details to ensure that all future correspondences reach you in a timely manner. 9. What should I do before I submit a claim? You must lodge a notice of intention to claim to us within seven (7) days from the date of loss. Failure to lodge such notice of intention to claim will forfeit your rights to file a claim under this policy. Thereafter, proof of claim must be filed. Repairs must be conducted by Authorized Panel Workshop selected and approved by us if you make an own damage claim against your own comprehensive policy. 10. Where can I get further information? Polisi ini adalah pembelian tambahan (sebagai perlindungan tersendiri) kepada Polisi Insurans Komprehensif Kenderaan Persendirian Anda dan memberikan perlindungan untuk Perbelanjaan Tanggungan Sendiri Berkaitan Kemalangan Kereta yang tidak dilindungi di dalam Polisi Insurans Komprehensif Kenderaan Persendirian Anda. 2. Apakah perlindungan / faedah yang disediakan? (i) Manfaat (A) PENYEMBURAN CAT UNTUK KESELURUHAN KENDERAAN (Had pembayaran balik seperti yang tercatat di dalam jadual) Perlindungan boleh dituntut sekiranya Kemalangan Motor telah mencetuskan tuntutan Kerosakan Sendiri di bawah Polisi Insurans Komprehensif Kenderaan Persendirian yang Berkenaan dengan Anda melibatkan penyemburan cat terhadap bahagian yang rosak tetapi tidak membenarkan penyemburan cat untuk keseluruhan kenderaan yang dibaiki. Perlindungan ini tertakluk kepada had yang ditetapkan seperti di dalam jadual. Perlindungan ini tertakluk kepada penyemburan cat untuk keseluruhan kenderaan di panel bengkel yang sama semasa menjalankan pembaikan kerosakan. (ii) Manfaat (B) PEMBAYARAN BALIK KOS BETTERMENT (Had seperti yang tercatat di dalam jadual) Perlindungan boleh dituntut ke atas pembayaran Betterment yang menimbulkan tuntutan Kerosakan Sendiri terhadap Polisi Insurans Komprehensif Kenderaan Persendirian yang Berkenaan dengan Anda yang timbul daripada usia Kenderaan Anda. Pembayaran balik ini tertakluk kepada perbelanjaan sebenar yang dikenakan menurut skala Betterment yang disenaraikan di dalam Polisi Insurans Komprehensif Kenderaan Persendirian yang Berkenaan dengan Anda. Nota: Tempoh perlindungan adalah selama satu tahun. Anda dikehendaki membaharui polisi insurans anda setiap tahun bersama dengan Polisi Insurans Komprehensif Kenderaan Persendirian Anda. 3. Berapakah premium yang saya perlu bayar? Jumlah premium yang anda perlu bayar bergantung kepada manfaat yang anda pilih. Manfaat Pelan Premium Tahunan Penyemburan Cat untuk Keseluruhan 1,000 Had Maksimum 120 Kenderaan 1,500 Had Maksimum 180 2,500 Had Maksimum 300 Pembayaran Balik Kos Betterment Kenderaan berumur 5-7 120 Kenderaan berumur 8-15 220 (Tidak Termasuk 6% CBP dan RM 10.00 duti setem) Nota: Premium yang disebut adalah premium yang dibayar ke atas policy yang dikeluarkan untuk satu tahun. Premium yang dibayar untuk tempoh insurans selain daripada satu tahun akan dikira secara pro rata. 4. Apakah yuran dan caj yang saya perlu bayar? Apa yang anda perlu bayar sebagai tambahan kepada premium Jumlah Cukai Barangan dan Perkhidmatan (CBP) 6% daripada premium Duti setem RM 10.00 Apa yang termasuk dalam premium Komisen yang dibayar kepada pengantara insurans (jika ada) Jumlah 10% daripada premium (ASRP/PDS 122016) 4

Pembayaran premium hendaklah dibuat sebelum perlindungan berkuatkuasa. 5. Apakah terma dan syarat utama yang saya harus ketahui? Kenderaan Anda mesti diinsuranskan dengan kami di bawah perlindungan insurans Komprehensif Kenderaan Persendirian bersamaan polisi ini. Usia Kenderaan Anda tidak boleh melebihi lima belas (15) tahun. Anda semestinya satu perbadanan, persatuan ataupun individu yang berumur di antara dua puluh empat (24) tahun dan enam puluh lima (65) tahun. Kontrak Insurans Pengguna Menurut Perenggan 5 daripada Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini sepenuhnya untuk tujuan yang tidak berkaitan perdagangan, perniagaan atau profesion anda, anda mempunyai kewajipan untuk mengambil langkah yang munasabah untuk tidak salah nyata dalam menjawab soalan-soalan di dalam Borang Cadangan (atau semasa memohon insurans ini). Anda dikehendaki menjawab soalan-soalan dalam Borang Cadangan ini dengan lengkap dan tepat. Kegagalan untuk mengambil langkah yang munasabah dalam menjawab soalan-soalan, mungkin mengakibatkan pembatalan kontrak insurans anda, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan di atas hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Sebagai tambahan kepada soalan-soalan di dalam Borang Cadangan (atau semasa memohon insurans ini), anda dikehendaki untuk mendedahkan apa-apa perkara lain yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami (atau semasa permohonan insurans ini), apa-apa maklumat yang dinyatakan dalam Borang Cadangan tidak tepat atau sudah berubah. Kontrak Insurans Komersial Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apaapa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan di atas hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bila-bila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami (atau semasa permohonan insurans ini), apa-apa maklumat yang dinyatakan dalam Borang Cadangan tidak tepat atau sudah berubah. Anda hanya boleh membuat tuntutan di bawah polisi ini jika anda juga membuat tuntutan di bawah Polisi Insurans Komprehensif Kenderaan Persendirian Anda. Apabila sebarang tuntutan terhadap manfaat di bawah polisi ini diselesaikan, manfaat tersebut akan dibatalkan secara otomatik kecuali ianya dikembalikan semula dengan pembayaran premium tambahan selanjutnya. Kenderaan berprestasi tinggi seperti Porsche, Lamborghini, Lotus, Jaguar dan lain- lain TIDAK akan diterima untuk manfaat (B) Penyemburan Cat untuk Keseluruhan Kenderaan Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kontrak polisi untuk senarai penuh bagi pengecualian di bawah polisi ini. 6. Apakah pengecualian utama di bawah polisi ini? Polisi ini tidak akan melayan apa-apa tuntutan yang melibatkan perkara seperti di bawah: Jika tuntutan di bawah Seksyen A Polisi Insurans Komprehensif Kenderaan Persendirian yang Berkenaan dengan Anda telah dibantah atau ditolak. 5

Kemalangan yang berlaku secara langsung atau tidak langsung dari perbuatan yang salah di sisi undang-undang atau jenayah. Kemalangan di mana kenderaan sedang dipandu uji atau di mana pemandu memandu di bawah pengaruh minuman keras atau dadah sehingga tidak mampu mengawal pemanduan ke atas Kenderaan Anda. Kemalangan akibat secara langsung atau tidak langsung daripada banjir, ribut, tanah runtuh, penenggelaman tanah, mogok, rusuhan dan kekacauan awam kecuali tuntutan tersebut dibayar di bawah Polisi Insurans Komprehensif Kenderaan Persendirian yang Berkenaan dengan Anda. Kemalangan akibat kecacatan pada reka bentuk atau pada pembaikan. Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk kontrak polisi untuk senarai penuh bagi pengecualian am di bawah polisi ini. 7. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi anda pada bila-bila masa dengan memberi notis bertulis kepada syarikat insurans. Sewaktu pembatalan, anda layak untuk mendapat pengembalian premium berdasarkan kadar jangka pendek dengan syarat tiada tuntutan dibuat. 8. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran perhubungan saya? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 9.
