I. NTRODUCTION Congratulations, you have taken the first steps to becoming a Service Provider Firm with ServiceLive, Inc. This manual provides the information and instructions you will need to complete the insurance portion of the registration process. II. PROVIDER FIRM INSURANCE RECOMMENDATIONS Proper and adequate insurance is necessary in today s working environment. All Provider Firms are required to carry insurance. ServiceLive, Inc. has established what many buyers may require as minimum insurance levels for the type of work you perform, however you are not required to carry these recommended limits to become a ServiceLive-Approved provider. 1. General Liability - Commercial Business General Liability insurance is part of standard business practice. It is designed to protect you and your company s assets against unforeseen circumstances. The following minimum insurance limits are required by most buyers: GL Coverage/Each Occurrence Amount Contractor Type 1,000,000 Plumbers, Electricians, Gas 500,000 Appliance Installers and Mechanical All other contractors 2. Worker s Compensation (Including Employer s Liability) Worker s Compensation insurance is designed to cover the injury of your employees while on the job. In most states, Worker s Compensation is not required for a sole proprietorship, or one-man type businesses. Each state has their own legal requirements, and it is best to check with your insurance agent to determine if your state requires such coverage. If required, you must obtain it before you may perform work. Minimum insurance coverage recommended: Workers Compensation Coverage to provide benefits as required by State Law. Employer s Liability insurance with limits of liability of at least 100,000 per accident or disease and 500,000 aggregate by disease. Worker s Compensation Insurance Certification If your company is not required by the state or states within which you provide service and you have elected not to acquire this insurance, then you must click to accept the conditions under the Insurance Requirements section for Worker s Compensation Insurance. 3. Vehicle Liability Insurance It is important that all vehicles used by your company or your employees are properly covered with liability insurance rated for business use. In some cases, vehicles covered in conjunction with homeowner s or personal vehicle policies are not covered for use in a business. Coverage should include all owned, non-owned, hired or leased vehicles. Your insurance agent is the best source to determine your business insurance needs and can address your insurance questions. Minimum Insurance coverage recommended 100,000 each occurrence (for bodily injury and property damage) 4. Umbrella Policies Insurance companies offer Excess Liability coverage, also referred to as umbrella policies, which provide additional insurance over and above the written coverage amounts on your other liability policies. III. INSTRUCTIONS 1. Determine Worker s Compensation Insurance Certification If your company is not required by the state or states within which you provide service and you have elected not to acquire this insurance, then you must click to accept the conditions under the Insurance Requirements section for Worker s Compensation Insurance. It reads as follows: Worker s Compensation Insurance Certification I hereby represent and certify that I am not required to have Worker s Compensation Insurance either because I do not currently have any employees or I am otherwise exempt from workers compensation insurance requirements under the laws of the states in which I live, work, and/or do business. In the event that employees are hired at any future time, I will acquire the necessary Worker s Compensation Insurance before they are allowed to work on any job. 2. Choose to become ServiceLive-Verified or Self-Declared You have two insurance status options with ServiceLive. You can either become ServiceLive- Verified or Self-Declared. Most buyers require insurance certificate verification, but you are not required to be verified to be a ServiceLive-Approved provider. Option 1 ServiceLive Verified Insurance Buyers will see your insurance status as VERIFIED on your profile. It is very important that, when filling out all Certificates of Insurance and any other forms that may be required that you please use the same name on each form. Please see 3.2.3 of this guide for detailed instructions. Verification requires that your insurance carrier have a rating of A- or better in the current Best s Insurance Reports published by A.M. Best Company. Provider lists Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries as the additional insured and assigns as Certificate Holder. Provider uploads insurance certificate(s) and ServiceLive verifies. Option 2 - Self-Declared Insurance Provider declares they have the state required insurance(s) and limits. Provider does not upload insurance certificate(s). Provider does not list Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries as the additional insured or certificate holder. To become ServiceLive-Verified Name Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries as Additional Insured Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries must be named as an Additional Insured on all your insurance policies, including products/completed operations. Most insurance companies will add additional insured requests at no cost. Please check with your agent. Additional insured coverage afforded to Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries must be in the form of endorsements CG 20 10 07 04 and CG 20 37 07 04 or endorsements providing similar coverage. Name Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporations and affiliates and subsidiaries as Certificate Holder ServiceLive, Inc. requires proof that you have purchased insurance. This proof of insurance (usually a Certificate of Insurance) must include a statement that Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries is assigned as Certificate Holder on your Commercial General Liability and Commercial Vehicle Liability Insurance Policy(s). List Consistent Provider Firm Name It is extremely important that, when filling out all Certificates of Insurance and any other required forms, the same name is used on each form. All transmissions must also contain this exact name. For example, if Inc. is part of your corporate name, then Inc. must be carried throughout the paperwork. If you operate as Doe, John d/b/a John Doe Appliance Repair, then that exact name should appear consistently throughout the paperwork. Confirm Insurance Company Rating A.M. Best Rating Requirement Verification requires your insurance carrier is rated A- or better in the current Best's Insurance Reports. Please visit and register at A.M. Best Company to check your carrier's rating. Get the Certificate of Insurance Filled Out (see next page for sample form) Form to be filled out to become a ServiceLive-verified provider only. This form, sometimes titled ACORD Certificate of Liability Insurance, is completed by your insurance agent or broker. It states the various types and levels of insurance coverage. Here are the highlights of the form. The numbers refer to the bullet points on the form to assist you. 1. Name, address, phone, fax and e-mail contact information of your insurance agent or broker. 2. The name of the insured. If you are a sole proprietor, it will show your name. If you are incorporated, it will show the company s name. 3. Worker s Compensation Insurance may or may not be state mandated for your company. It is your responsibility to obtain the needed coverage if required by law. Your insurance agent can assist with this information. 4. General liability insurance is required. Insurance limits, as stated elsewhere in this manual, are minimum levels required by most buyers. 5. Vehicle insurance is required for all vehicles used by your company when providing services. 6. This line indicates the policy number for each type of insurance carried. 7. Effective and expiration dates are very important. This tells you for what period you are covered. It is very important to monitor the expiration date and renew your coverage before the date of expiration. You are responsible for ensuring uninterrupted insurance coverage. 8. This section shows your total liability limits and liability limits for each occurrence. 9. This space will name Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries as the additional insured. 10. Sears, Roebuck and Co. and its affiliates and subsidiaries or Sears Holdings Corporation and affiliates and subsidiaries should be shown here as the certificate holder. Please note the certificate holder s address has changed. The date the policy is issued. Contractor s insurance shall be primary and non contributory to all valid and collectable insurance. 12. The signature of your insurance agent or other authorized representative. 13. Completed product operation needs to be completed with the dollar amount. Upload Certificate of Insurance to ServiceLive During the online registration process, please upload your Certificate of Insurance documents for ServiceLive to verify and keep on file. IV. THINGS TO REMEMBER ABOUT INSURANCE It is your responsibility to maintain active insurance coverage. Your insurance agent is the best source to determine your business insurance needs and can address your insurance questions. Failure to provide and maintain insurance coverage can lead to buyers not posting service orders to your firm. If you decide not to become ServiceLive-Verified, you will not be eligible for service orders from certain buyers who require Verified insurance.
