NJWELL OVERVIEW Administered by the NJDPB Program Year November 1 through October 31 Pensions & Benefits

WHAT IS NJWELL? NJWELL is an incentive based wellness program for active State Health Benefits Program (SHBP) and School Employees Health Benefits Program (SEHBP) eligible members and their spouses/partners NJWELL aims to cultivate healthy lifestyle choices and improve the wellbeing of actively employed members of the SHBP and SEHBP Introduced in 2014, NJWELL is a multi year approach to raising awareness and improving the health of SHBP and SEHBP members Eligible active members and their covered spouse or partner who meet the program s wellness guidelines and goals each year receive financial rewards. Depending on your health carrier, you can receive MasterCard/Visa Prepaid Card(s) and/or retail gift card(s).

WHAT ARE THE ELIGIBILITY REQUIREMENTS? ELIGIBLE All active employees enrolled in a SHBP/SEHBP health plan through their employers Spouses/partners covered under an active employee s SHBP/SEHBP health plan INELIGIBLE Employees who have waived SHBP/SEHBP health coverage Anyone who is not eligible for SHBP/SEHBP coverage Anyone enrolled in COBRA or retiree SHBP/SEHBP coverage Dependent Children, including those over the age of 18 and Chapter 375 subscribers

HOW DO I KNOW IF I M ELIGIBLE? Take out your health benefits card and look for the WE VE GOT YOU COVERED or the NJDPB logo. If it s on your card, you are eligible!

WHAT S IN IT FOR ME? EMPLOYEES and SPOUSE/PARTNER Earn $250 EACH in MasterCard/Visa Prepaid Card and/or retail gift card* Manage and improve your health Learn about your risk factors and address concerns with your health carrier or physician EMPLOYERS Healthier workforce Reduced absenteeism due to health conditions Added NO COST benefit to help employees with overall wellbeing Earn a 1% premium reduction** on your SHBP/SEHBP health benefits premium in the next calendar year! **premium reduction is for Local Government and Local Education Employers * Depending on health carrier

HOW DOES NJWELL WORK? Eligible members and their covered spouse/partner complete health screenings and activities and earn points 800 points translate into a financial reward AETNA MEMBERS Reach points goal (800) and you may select a MasterCard Prepaid Card and/or a retail gift card (electronic or physical). HORIZON MEMBERS Reach points goal (800) and you may select a Visa Prepaid Card that can be redeemed electronically or by mail.

BENEFIT FOR BOTH EMPLOYEE AND/OR SPOUSE/PARTNER DO I HAVE TO PARTICIPATE? No! But why wouldn t you? You can receive $250! DOES MY SPOUSE/PARTNER HAVE TO PARTICIPATE? No! But if they do, they can receive $250! CAN ONE OF US PARTICIPATE AND THE OTHER NOT? Absolutely! But why wouldn t you BOTH participate and receive $500 for doing so! WHAT IS THE COST TO PARTICIPATE? NOTHING! It s a FREE benefit offered through your existing health plan

HOW MANY POINTS DO I NEED TO EARN $250? An eligible employee and/or their spouse/partner must accrue 800 points to receive a $250 financial reward *Initial MasterCard/Visa Prepaid Card redeemable upon attaining 800 points **NOTE: Retail gift cards available for Aetna members only

SOUND GOOD? GREAT! LET S START EARNING POINTS!! To get started you must complete two main components of NJWELL Obtain a BIOMETRIC SCREENING worth 100 points Complete a HEALTH ASSESSMENT worth 100 points Depending on your health carrier, BOTH of these requirements MAY need to be completed before you can see your points accrue on your carrier s portal

WHAT S A BIOMETRIC SCREENING? It s a screening that helps you learn about your biometric numbers. The screening analyzes your Cholesterol, Triglycerides, Glucose (sugar) and Body Mass Index or BMI (based on height & weight). You will also have your pulse and blood pressure measured during the screening. Then you will KNOW YOUR NUMBERS!

HOW DO I COMPLETE A BIOMETRIC SCREENING? ALL PARTICIPANTS must register for the www.my.questforhealth.com portal, using the registration key of NJWELL, to access their options for a Biometric Screening. AETNA MEMBERS: There are two ways to obtain a Biometric Screening: Schedule an appointment with your physician to complete your Biometric Screening and earn 100 points. Download the Physician Results Form from the site listed below and take it with you to your appointment for completion. Submit the form through the www.my.questforhealth.com portal or fax the form to the number listed on the form; OR schedule an appointment for a no cost Biometric Screening at Quest Diagnostics using the www.my.questforhealth.com portal. There is no fasting required and you may take any regularly scheduled medications as usual. HORIZON MEMBERS: Schedule an appointment with your physician to complete your Biometric Screening and earn 100 points. Download the Physician Results Form from the site listed below and take it with you to your appointment for completion. Submit the form through the www.my.questforhealth.com portal or fax the form to the number listed on the form.

WHAT S A HEALTH ASSESSMENT? It s a private and secure online questionnaire about medical history and lifestyle habits completed through your health carrier s online portal If you haven t done so already, you will need to register through your health carrier s portal (Aetna or Horizon) Responses to the questionnaire, combined with the Biometric Screening results, help measure a member s health status, determine health risks and offer a plan of action on how the member s health can be improved WHAT ELSE DOES THE HEALTH ASSESSMENT MEASURE? Current health conditions Vital statistics including cholesterol and blood pressure Lifestyle factors including alcohol consumption, tobacco use, stress, exercise and diet Utilization of health care including doctor visits, prescriptions and hospitalizations

WHAT HAPPENS AFTER I COMPLETE A BIOMETRIC SCREENING AND HEALTH ASSESSMENT? First and foremost you will earn 200 points towards your reward! Second your health carrier will analyze your results and create a plan for a healthier you! Third your health carrier may reach out to you to notify you of potential risk factors and/or engage you in conversations, or coaching, to help you achieve your goals

WHAT IF MY RESULTS ARE WITHIN HOORAY FOR YOU BECAUSE.. NORMAL LIMITS? Goal Achievement = MORE POINTS Cholesterol (less than 240) 100 points BMI (body mass index) (less than 30) 100 points Non smoker (based on Health Assessment response) 100 points Glucose (fasting less than 126/non fasting less than 200) 100 points Blood Pressure (systolic less than 140/diastolic less than 90) 100 points That s a possible 500 points for Goal Achievement! Add that to the 200 points for your Biometric Screening and Health Assessment and you ve just earned 700 points!!

WHAT IF I DON T REACH THOSE GOALS? No worries! There are plenty of other opportunities to earn points! Age/gender appropriate screenings 25 points each (including annual gynecological exam, mammogram*, pap test screening*, colon cancer screening*, prostate screening*) Dental Exam (one annually) 50 points Skin Cancer Screening 50 points Vision Exam 50 points Annual Physical Exam 100 points Flu Shot 100 points A maximum of 450 points total for the following three options: Online Coaching 150 points** Online Activities 50 points each (for a maximum of 150 points) Telephonic Wellness/Disease Management Coaching** 150 points * If recommended by your physician ** Only available for those individuals identified as eligible based on their Biometric Screening and/or Health Assessment.

THINGS YOU SHOULD KNOW Rewards are considered taxable income Remember the taxability of this benefit is minimal. The amount earned is $250, and taxes will be calculated based on that amount The DPB s NJWELL Program will provide employers with an electronic report at the end of the calendar year listing employees and /or their spouse/partner who have earned rewards The employer will include this amount on the employee s W 2 as taxable income whether the employee earned the reward or their spouse/partner, it is taxable to the employee

THINGS YOU SHOULD KNOW All information obtained through the NJWELL Program is CONFIDENTIAL Any Personal Health Information (PHI) gathered during the Biometric Screening or Health Assessment, or any other engagement through your health carrier, is protected by Federal and State privacy laws including the Health Insurance Portability and Accountability Act (HIPAA) The employer will only know that a member and/or their spouse/partner earned a financial reward for tax reporting purposes Neither the NJWELL Program nor the employer will receive any information concerning your health conditions or screening results

THINGS YOU SHOULD KNOW Some points will be automatically uploaded to your account through your health carrier s portal Some points are self reportable meaning you must fill in the dates of the screenings or tests through your health carrier s portal Some points won t be awarded until you finish an activity or coaching according to the guidelines established Points received through Physician Results Forms may take up to 10 business days to upload to your account
