MASSACHUSETTS PRIVATE PASSENGER AUTOMOBILE INSURANCE MANUAL Auto Loan/Lease Coverage CIC 2125 A policy providing Part 7 - Collision and Part 9 - Comprehensive may be extended to provide coverage for the difference between the outstanding indebtedness on a loan/lease agreement on a new vehicle, and the actual cash value of the vehicle, subject to the following: a. Auto Loan/Lease Coverage may be provided only to a vehicle that is a private passenger auto, pickup, or van, as defined in Rule 27, except that antiques and motor homes/camper bodies are not eligible; and b. The insured must request, and the auto loan/lease coverage must be added, within 30 days of leasing or financing a new vehicle. A new vehicle is defined as one which is 5 model years old or less; or c. Coverage may be applied to a vehicle written on a new business policy provided the vehicle had the same coverage on a previous policy and is 5 model years old or less. There is a 5% charge of both the Part 7 - Collision and Part 9 - Comprehensive premiums for this endorsement. Motorcycle Accessory Coverage CIC 2174 Motorcycle Accessory Coverage may be purchased to provide protection from physical damage to or loss of accessories, on a motorcycle or motorcycle trailer, other than those that are original manufacturer installed and which alter the appearance or performance of a motorcycle or motorcycle trailer. This includes but is not limited to: a. Permanently installed electronic equipment, antennas, and other devices used exclusively to send or receive audio, visual, or data signals, or playback recorded media. Items must be installed using bolts or brackets, including slide-out brackets; b. Sidecars; c. Custom paint, custom plating, custom seats and custom exhaust; d.. Safety riding apparel. This includes safety apparel specifically designed to minimize injury from an accident. This includes but is not limited to helmets, leather clothing, riding boots and gloves, and protective eyewear. Certain exclusions will apply. Please refer to the endorsement for additional details. This endorsement may be purchased for any motorcycle that has Part 9 - Comprehensive Coverage in increments of $1,000, up to a maximum limit of $25,000. Please refer to the Miscellaneous Rate Page for the applicable premiums per $1,000 of coverage. MASSACHUSETTS PRIVATE PASSENGER AUTOMOBILE INSURANCE MANUAL Auto Loan/Lease Coverage CIC 2125 A policy providing Part 7 - Collision and Part 9 - Comprehensive may be extended to provide coverage for the difference between the outstanding indebtedness on a loan/lease agreement on a new vehicle, and the actual cash value of the vehicle, subject to the following: a. Auto Loan/Lease Coverage may be provided only to a vehicle that is a private passenger auto, pickup, or van, as defined in Rule 27, except that antiques and motor homes/camper bodies are not eligible; and b. The insured must request, and the auto loan/lease coverage must be added, within 30 days of leasing or financing a new vehicle. A new vehicle is defined as one which is 5 model years old or less; or c. Coverage may be applied to a vehicle written on a new business policy provided the vehicle had the same coverage on a previous policy and is 5 model years old or less. There is a 5% charge of both the Part 7 - Collision and Part 9 - Comprehensive premiums for this endorsement. Motorcycle Accessory Coverage CIC 2174 Motorcycle Accessory Coverage may be purchased to provide protection from physical damage to or loss of accessories, on a motorcycle or motorcycle trailer, other than those that are original manufacturer installed and which alter the appearance or performance of a motorcycle or motorcycle trailer. This includes but is not limited to: a. Permanently installed electronic equipment, antennas, and other devices used exclusively to send or receive audio, visual, or data signals, or playback recorded media. Items must be installed using bolts or brackets, including slide-out brackets; b. Sidecars; c. Custom paint, custom plating, custom seats and custom exhaust; d.. Safety riding apparel. This includes safety apparel specifically designed to minimize injury from an accident. This includes but is not limited to helmets, leather clothing, riding boots and gloves, and protective eyewear. Certain exclusions will apply. Please refer to the endorsement for additional details. This endorsement may be purchased for any motorcycle that has Part 9 - Comprehensive Coverage in increments of $1,000, up to a maximum limit of $25,000. Please refer to the Miscellaneous Rate Page for the applicable premiums per $1,000 of coverage. DriveAdvisor Program Endorsement CIC 2229 DriveAdvisor is a voluntary program available to eligible operators to promote safe, accident-free driving. As a DriveAdvisor participant, the insured will be provided with a device to install in their auto. The device collects data, which is analyzed for the purpose of providing online feedback and information about the insureds driving patterns. Participation in DriveAdvisor is subject to the program s terms and conditions. DriveAdvisor Endorsement is eligible to Insureds who: 1. at the time of program enrollment, have an operator on their policy with: a. less than three years driving experience; or b. 5 or more Merit Rating points, 2. have a 1996 or newer automobile that is compatible with the DriveAdvisor device; and 3. provide a valid email address. This program has no impact on the premium or coverage selections. Dear Policyholder, Congratulations on enrolling your vehicle(s) in our new DriveAdvisor Program. Once you install your device(s), be sure to use the DriveAdvisor website often to get useful information about the driving patterns of your enrolled inexperienced driver(s). This insight can help him or her learn to drive as safely as possible. What s Next? Within a couple days of enrolling a vehicle in the DriveAdvisor program, you will receive a Welcome email with a personal link to the DriveAdvisor website and instructions on how to register. Registration involves setting a personal password and accepting the Terms of Use. Once registration is complete, the device(s) will be shipped and you will receive a Shipment email with a USPS Priority Mail tracking number. The Shipment email provides instructions on how and where to install the device and includes instructions on how to locate a vehicle s OBD port which is where the device will be self installed. The shipping package also includes instructions on how and where to install the device. You must register on the website within 21 days of enrolling and must install the device in the vehicle within 28 days of shipment to remain in the program. DriveAdvisor Program Frequently Asked Questions What is DriveAdvisor and how does it benefit customers? DriveAdvisor is a voluntary program that offers feedback and insight about driving patterns to inexperienced drivers and their parents through the use of a telematics device. This program educates inexperienced drivers about improving their safe driving habits and can help to reduce their risk of having an accident. How does the DriveAdvisor Program work? Participating insureds: 1. enroll eligible vehicles through their agent; 2. register on the DriveAdvisor website; 3. install and keep the DriveAdvisor device(s) in the correct vehicle; and 4. monitor their driving performance on the DriveAdvisor website to gain insight on how to improve safe driving habits. Will a participant s driving data be used to change their policy premium? No, DriveAdvisor participation does not affect the policy premium. The program is designed as a free Loss Prevention Service to help participants become safer drivers. Who is eligible for the DriveAdvisor Program? Insureds who: 1. have an inexperienced (0-2 years licensed) driver on their policy OR a driver that has 5 or more SDIP points on their license 2. have a 1996 or newer automobile that is compatible with the DriveAdvisor device; and 3. provide a valid email address.

How is vehicle compatibility checked? All 1996 and newer automobiles are automatically checked during the quoting or endorsement process if a vehicle is selected for the DriveAdvisor Program. If a selected vehicle is NOT compatible with the DriveAdvisor device, enrollment will be removed and an error message will be displayed on the screen. What information does the DriveAdvisor device collect? The device records the vehicle speed, engine RPM, and vehicle location and heading on up to a second-by-second basis. This data is securely transmitted within 24 hours at the end of each trip automatically via cellular networks. The device also captures the vehicle s VIN and when the device is installed and disconnected. What driving information can participants see on the password-protected DriveAdvisor website? Based on the raw data collected, the driving information available to participants includes how much, when, how, and where the vehicle is driven including daily miles driven, time of day driven, hard braking events, severe braking events, hard acceleration events, high speed events, and extreme speed events. Vehicle location is captured to provide Trip Maps that pinpoint where any driving events occur for additional insight. Participants use their email address as their user id and the password they created during registration to access their driving information on the DriveAdvisor website. The website address is driveadvisor.mapfrecommerceinsurance.com. Can a participant withdraw from the program after enrolling? Participation is optional and voluntary. A participant can withdraw any or all vehicles from the program at any time without penalty. If a participant fails to comply with the Terms of Use of the program, we will remove the vehicle(s) from the program without penalty. An email providing instructions on how to return the DriveAdvisor device is sent to participants when a vehicle is withdrawn from the program. The device must be returned within 45 days of withdrawal or a $100 Non-Returned Device Fee will be charged. Can a participant move the DriveAdvisor device from one vehicle to another depending on which one is being driven? No one of the conditions of the program is that the device remains in the enrolled vehicle for the duration of participation in the program. Each device is assigned to a specific vehicle and reports VIN mismatches when a device is installed and disconnected so we can assure the consistency and authenticity of the data collected. A device may be removed temporarily for vehicle maintenance if access to the OBD port is needed but should be reinstalled immediately after the service is complete. If a replacement vehicle is purchased for a sold or totaled DriveAdvisor vehicle, the participant will be informed if the device can be transferred to the replacement vehicle. Getting Started

Getting Started There are four easy steps to installing and using this device. Step 1 Find Your Vehicle s OBD Port (On Board Diagnostic System) This port is typically located in one of four places: 1. Driver side kick panel 2. Driver side under dashboard 3. Front of center console 4. Center console You may have to get down on your hands and knees to find the OBD port. A flashlight might be handy, depending on how light your surroundings are. You may have seen someone access your OBD port before. It s the interface mechanics plug into to talk to the engine s computer system. It s the same interface used by the emissions control service personnel. Some vehicles have the OBD port access in unusual spots. If you cannot locate your vehicle s OBD port, check your owner s manual or call the MAPFRE Commerce Insurance DriveAdvisor SM Customer Support Team at 1.855.889.9500. Step 2 With the car turned OFF, plug the device into the OBD port. Important: Push the device firmly into the port to ensure proper connection. Each device MUST be installed in the vehicle that is printed on its package label. Devices cannot be transferred to another vehicle unless you are directed to do so by Customer Support. Step 3 Take a trip This first trip will initialize the device and may not appear on your trip records. Once the device is initialized, subsequent trips will start appearing on the DriveAdvisor SM website (driveadvisor.mapfrecommerceinsurance.com) your personal and secure portal. Please keep in mind that it might take up to several hours before your trips upload to this site. Step 4 Log in to your account and view your trips on the DriveAdvisor SM website. Your user name is your email address and you have already set up your password. This portal gives you access to view your trips and driving reports with details like your driving Score, driving behavior, alerts, user management and Customer Support.

DriveAdvisor Program Terms of Use and Privacy Policy Welcome to the DriveAdvisor insurance program (the "Program") offered by MAPFRE Commerce Insurance ("MAPFRE Commerce Insurance" "we," or "us."). We are excited about offering you a chance to participate in the Program. This document contains important information about the Program offered by MAPFRE Commerce Insurance. By participating in the Program you accept these Terms of Use and Privacy Policy. You should read this document carefully to understand your rights, benefits, costs and limitations under the Program. We may modify these Terms of Use and Privacy Policy at any time in our discretion. When used in these Terms of Use, you means a policyholder or eligible operator, as appropriate. Your participation in the Program is optional and voluntary. You can withdraw from the Program at any time any or all vehicles you have enrolled. Also, if you fail to comply with these Terms of Use, we will remove your vehicle(s) from the Program. The Purpose of the Program. The purpose of the Program is to provide you with information that will allow you to monitor your driving performance through web-based and other tools and for us to collect that information (the "Data") for actuarial purposes (i.e., to study the value of the Data generally for consumer driving performance). The Program does NOT at this time involve using the Data in order to determine the rates of insurance you pay, to quote insurance prices, or to otherwise underwrite insurance products. The Program will, however, give you invaluable insight into your own driving patterns and give us the opportunity to study how driving performance generally might help us offer improved insurance products to our customers and the driving public. If MAPFRE Commerce Insurance determines, in its sole discretion, to change the purpose of the Program or the types of Data we collect, we will provide you with timely notice, and you will have the opportunity to opt-out of the Program. How the Program Works. We will provide you with a small telematics device (a "Device") for you to install in the vehicle you enroll in the Program. The Device captures certain Data about your vehicle and your driving patterns. The Data is described more fully below. The Data is then transmitted via wireless means to computer servers that will allow you to access the Data through a secure, passwordprotected, web-based tool called the DriveAdvisor website at www.driveadvisor.mapfrecommerceinsurance.com (the DriveAdvisor website ). By using the DriveAdvisor website, you will be able to see, for example, the number of miles you drive, your braking and acceleration patterns, the times of day you drive, the roads on which you drive your vehicle and the places you park your vehicle, your vehicle speed and other information that will help you assess how you drive your vehicle. We will also have secure access to the Data, and we plan to use the Data to determine whether it is relevant to the products we might offer you and other drivers.

Obligations Relating to the Device. By participating in the Program, you agree that we own the Device, to install and to maintain the Device according to the directions that we provide, and not to damage or destroy the Device. You also agree to return the Device to us if you withdraw from the Program or upon our written request at any time. If you fail to return the Device within 45 days of withdrawal or when we request its return in writing, you agree to pay us one hundred dollars ($100) (the Non-Returned Device Fee ). We and our partners and vendors retain all right, title and interest in and to the Device and the software and firmware included in the Device. You may not reverse engineer, disassemble, decompile, alter, duplicate, translate, make copies, or create derivative works from the Device or firmware, or otherwise attempt to derive the source code of the software or firmware associated with the Device. You may only install the Device on the vehicle you have enrolled in the Program. If you are not the owner of the vehicle enrolled in the Program, you agree that you have the consent of the owner to install the Device and provide the Data to us as described in these Terms of Use. Switching the device between vehicles without prior approval is not permitted, and the Device will send a VIN mismatch signal if installed in a different vehicle. You may remove the Device from your vehicle prior to having your vehicle serviced, but you should reinstall the Device promptly when your vehicle is returned to you. Continuous or frequent removal and/or disconnection of the Device may jeopardize your participation in the Program. Tampering with the Device. You agree not to tamper with the Device. If you do tamper with the Device, we may turn off the Device, require that you return the Device to us, or take other action, including removing you from the Program. The Device Draws on Your Vehicle's Battery. The Device draws a minimal amount of current from your vehicle's battery, even when your vehicle is parked. You should be careful about using the Device if your vehicle has an old or weak battery. The Data and the Obligations Related to the Data. As we explained, the Device not only captures certain Data, but it transmits the Data to computer servers so that you and we can access the Data. We will be collecting the following Data from the Device: vehicle speed engine RPM vehicle heading time of day vehicle is being driven vehicle location (while being driven and when parked)

vehicle identification number, or VIN Device connect(s) and disconnect(s) You agree that we may, after giving you reasonable notice, change the type of Data that we collect as part of the Program. If, after you receive such a notice, you wish to opt-out of the Program, you need only let us know either by contacting us at driveadvisor@mapfreusa.com, by calling the MAPFRE Commerce Insurance DriveAdvisor Customer Support Team at 1-855- 889-9500or by notifying your insurance agent. By participating in the Program, you agree to provide the Data to us and that we own the Data gathered by the Device. We may retain and use the Data according to these Terms of Use and Privacy Policy. Once the Data is transmitted to us from the Device, it will be erased from the Device, and we will retain the Data for as long as we wish or as allowed by applicable law. In addition, upon your returning the Device to us, any Data remaining on the Device will be erased from the Device. Once we receive the Data, it will be stored in a secure environment. However, the Data will be transmitted from your vehicle by wireless means and there exists the possibility that someone or some organization might intercept the Data. You agree and understand that neither we nor any of our third party vendors, including The Evogi Group, Inc. ( Evogi ) are responsible if any Data are delayed, lost, altered or intercepted (for any purpose) during the transmission of such communications across networks that are not owned and/or operated by MAPFRE Commerce Insurance or its third party vendors, including Evogi. Permission to Use the Data and Certain Restrictions. MAPFRE Commerce Insurance grants you the right, known as a limited, non-exclusive license, to use the DriveAdvisor website and to access the Data through the DriveAdvisor website, each according to MAPFRE Commerce Insurance's rules, which we may change in our discretion. We may revoke this license at any time for any reason. If at any time, you opt-out of the Program or remove the Device from your vehicle in violation of these Terms of Use and Privacy Policy, your license to use the DriveAdvisor website and to access the Data shall automatically terminate. Just as we retain all right, title, and interest in the Device and its associated software and firmware, we and our partners and vendors, including Evogi, retain all right, title and interest in and to the Data. You may not decompile, alter, duplicate, translate, make copies, or create derivative works arising from or relating to the Data. Notice Concerning Possible Disclosure of the Data. We will treat the Data as confidential information and will protect it as required by applicable law, including without limitation 201 Code of Massachusetts Regulations 17.00 (Standards for the Protection of Personal Information of Residents of the Commonwealth). We will only use the Data for purposes of implementing and improving the Program. We will not disclose any Data that identifies you in any way to any person or entity unless we are ordered to do so by a

The DriveAdvisor website is owned and maintained by Evogi, with whom MAPFRE Commerce Insurance is collaborating to make the Program available to you. By accessing and using the DriveAdvisor website, you are indicating your acceptance of these Terms of Use and the Privacy Policy. If you allow any other person to access your personal account, he or she must comply with these Terms of Use and the Privacy Policy, as they may be applicable to him or her. You will remain responsible for any action by such person that violates these Terms of Use. The software used on the DriveAdvisor website is protected by copyright and other intellectual property laws and is licensed, not sold, to you. Your use of the DriveAdvisor website and the software will not give you any ownership or intellectual property rights in the DriveAdvisor website or the software, firmware, object or source code that comprises the DriveAdvisor website. If you choose to stop using the Device and cancel your participation in the Program (or if we determine, in our sole discretion, that you may not participate in the Program, use the Device, or access the DriveAdvisor website), your license to use or access the website shall automatically terminate. Wireless Phone Alert/Text Service. Depending upon the product and service you order, you may have the option to receive alerts and texts on a cell phone number you provide. You acknowledge that some wireless service providers may charge additional fees for sending and receiving text messages, and that you are responsible for any such charges or fees. If you change your cell phone number while participating in the Program, you may update this information by logging into your personal account at the DriveAdvisor website and changing your wireless contact information for this alert/text service.
