INTRODUCTION This is a quick reference guide to the benefits offered to eligible Eide Bailly staff members. Eligibility in any given plan is subject to the terms and conditions of that particular benefit plan. This guide is meant as an overview. For complete plan information, refer to the plan documents located on Firmworks, Eide Bailly s Intranet. Every effort has been made to ensure accuracy within this reference guide; however, if a discrepancy is discovered between this guide and the related plan documents, the information contained in the plan documents will apply. ELIGIBILITY The benefits outlined are available to staff members scheduled to work 1,000 hours or more on an annual basis. Your coverage start date will be the first day of the month following your hire date, unless otherwise stated. You must complete the appropriate enrollment within 30 days of your hire date in order to be covered. If you do not elect benefits when you are first eligible, you must wait until the next annual open enrollment period, unless you experience a qualifying event. BENEFIT DATE OF ELIGIBILITY WHO PAYS? Any plan changes must correspond with the qualifying event (i.e., if you have a baby, going from Single Plus Spouse to Family coverage) QUICK 2 GUIDE
HEALTH INSURANCE Eide Bailly offers its staff members a self-funded, Preferred Provider Organization (PPO) plan administered by BlueCross BlueShield of North Dakota (BCBSND). As a new hire you are not immediately subject to the difference in premiums, however, you will want to read the wellness brochure included with your orientation materials so that you understand how it will affect you for the next plan year. If you have questions regarding the wellness program, please contact your Office Administrator. Prescription Drug Program This plan includes a preferred pharmacy network. Below are the co-payments associated with prescriptions. These co-payments do count toward the plan out-of-pocket maximum. A list of formulary drugs can be found at www.bcbsnd.com in the Pharmacy section. Copay Generic $10 Brand Formulary $35 Brand Non-Formulary $70 Specialty $150 Specialty Drug Program Specialty drugs are prescribed for those with more complex conditions, such as multiple sclerosis, hemophilia, rheumatoid arthritis and hepatitis C. If you are receiving a specialty drug, you must work through the preferred specialty drug provider in order to receive the highest level of benefits. To enroll in the program, call 1.888.216.6710. Additional information regarding the specialty pharmacy program, as well as a list of specialty drugs, can be found on Eide Bailly s Intranet, Firmworks. Step Therapy A step therapy program is a step approach to providing the medications your physician prescribes for you. This means that you may first need to try more clinically appropriate or cost-effective medication before certain high-cost medications are approved by your health plan. Prenatal Plus All BCBSND members are eligible for the Prenatal Plus program, which is a program to help promote healthy babies. You can enroll following your first prenatal visit. Once you complete a questionnaire you will receive a packet of information regarding pregnancy and prenatal care. To sign up for the prenatal plus program, you can call the member service number on the back of your ID card or enroll through online member services. Tobacco Cessation Our plan includes a tobacco cessation program. Benefits include certain over-the-counter and prescription nicotine replacement therapies. More detailed information on what is covered can be found on Firmworks in the health insurance Summary Plan Description. BLUECROSS BLUESHIELD ONLINE MEMBER SERVICES BCBSND offers online services for its members. Once you register, you will be able to access your personal information, check current and past claims, view plan information and request a new ID card. To register, go to www.bcbsnd.com and click on the Register Now link. Additional information about online member services can be found on Eide Bailly s intranet, Firmworks. DENTAL INSURANCE Maintaining good dental health through regular check-ups could prevent you from having major dental expenses in the future. Eide Bailly s dental plan covers 100 percent of preventive care. To find a participating dentist, visit www.deltadentalmn.org and use the interactive dentist search tool, or call customer service at 1.800.553.9536. Delta Dental Premier is the largest dental network with approximately 125,000 participating dentists. The Delta Dental PPO is a smaller network, but may provide greater discounts than the Premier Network. You do not need to select a network; discounts are based on which network your dentist is part of. VISION INSURANCE Eide Bailly offers a vision plan through VSP. To find a participating eyecare provider in your area, visit vsp. com, or call 800.877.7195. There s no ID card necessary on the vision plan. Coverage with VSP doctors and affiliate providers can be found below. LIFE INSURANCE The firm provides, at no cost to you, group life insurance and accidental death and dismemberment insurance for you and your family. Your coverage is one and one-half times your annual compensation, to a maximum of $150,000. The firm provides a $2,000 policy on spouses and a $1,000 policy for eligible dependents. More detailed information on what is covered can be found on Firmworks in the Life Insurance Summary Plan Description. SUPPLEMENTAL LIFE INSURANCE The firm offers supplemental life insurance coverage for you and your family. You can purchase, at your own cost, an amount equal to the lesser of 5 times your salary or $500,000. Although you are able to enroll or make changes during our open enrollment periods each year, during your first 30 days is the only time you can enroll for coverage up to a guaranteed issue of $200,000 with no medical information required. Coverage elected over this amount will require evidence of insurability. Coverage is also available for purchase for your children in the amount of $10,000 per child. If you elect coverage for yourself, you may then also choose to purchase coverage for your spouse to a maximum of $250,000. During your initial enrollment period, you can elect spouse coverage up to a guaranteed issue of $30,000 with no medical information required. Coverage cannot exceed 50% of the amount you elect for yourself. More detailed information can be found on Firmworks. DISABILITY INSURANCE Short Term Disability The firm offers short-term disability at no cost to you. This benefit provides income replacement in the event of your own serious health condition. Coverage is available after a seven consecutive calendar day elimination period with a payment of 66 2/3 percent of your base wages. Long-term Disability The firm offers long-term disability coverage at no cost to you. This coverage is available up to a maximum of $8,000 per month. VOLUNTARY BENEFITS Critical Illness The firm offers voluntary critical illness insurance coverage to you and your family. Major medical insurance may pick up most of the tab, but can still leave out-of-pocket expenses that add up quickly. Critical illness insurance can provide a lump-sum benefit upon diagnosis that can be used however you choose from expenses related to treatment to deductibles or day-to-day costs of living, such as the mortgage or your utility bills. Accident The firm offers voluntary accident insurance coverage to you and your family. With accident insurance, you ll receive payment(s) associated with a covered injury and related services. You can use the payment in any way you choose from expenses not covered by your major medical plan to day-to-day costs of living, such as the mortgage or your utility bills. STAFF 7 BENEFITS
FLEXIBLE SPENDING ACCOUNTS We have a Flexible Benefits Program, which allows you to use pre-tax dollars to pay for certain medical and dependent care expenses. There are three components of our Flexible Benefits Program: Medical Spending Account allows you to set aside before-tax dollars to pay medical expenses not paid by insurance or reimbursed by another source. The annual maximum you can contribute to this account is $2,650. - You may carry over up to $500 to the next plan year. If you make a Flex Medical elections for the current year, and are unable to use the funds by December 31, up to $500 will be carried over into the following plan year. This does not affect how much you can elect for the upcoming plan year. For example, you could elect $2,650 for the upcoming plan year and have up to $500 carry over from the previous year, bringing your total Flex Medical balance to $3,150. Dependent Care Spending Account allows you to set aside before-tax dollars to pay for day care expenses for children under the age of 13, or for adult day care for a disabled spouse or other disabled dependent. The annual maximum you can contribute to this account is $5,000. Premium Conversion we automatically deduct your portion of the health, dental and vision insurance premiums on a pre-tax basis. Limited Purpose Flex A limited purpose medical spending account allows an employee who is participating in or covered by a high deductible health plan (HDHP) and/or is also participating in a health savings account (HSA) the ability to set aside before-tax dollars to pay for specific expenses that are not paid for by the insurance, the employer or by any other source. For this type of account, eligible expenses are limited to those that fall into the following three categories as allowed by the IRS: dental, vision or preventitive. Debit Card All staff that enroll will receive a Flexible Spending Account debit card. This allows some of your expenses to be paid without using a claim form. Plan Year The plan year runs from January 1 through December 31. Participants have until April 30 of the following year to file claims for expenses incurred during the plan year. 401(K) RETIREMENT PLAN You may defer a portion of your salary into the 401(k) plan, subject to IRS limitations. You will be able to enroll in the 401(k) plan immediately and have the option to elect pre-tax contributions as well as Roth contributions. The firm has an automatic enrollment provision as part of the 401(k) plan. On your first paycheck, a 3% deferral will automatically be contributed to your 401(k) account. If you do not want the 3% contribution or wish to contribute a different percentage, you will need to go through the online enrollment process at www.alerusretirementsolutions.com and elect zero or your desired percentage at least 2 weeks prior to your first paycheck. You will receive an enrollment packet in the mail, including your login information, within the first week of employment. If you do not receive the packet, please contact your Office Administrator. The firm has a profit sharing contribution, which occurs yearly in January. To be eligible for the plan, you must have attained the age of 21, complete 12 months of employment (during which you work at least 1,000 hours), and must be employed by the firm on 12/31. If you meet the eligibility requirements, the firm will contribute an amount equal to four (4) percent of your annual compensation into the profit sharing plan on January 15 of the following year. Each year the requirements are reassessed to be sure you are still eligible for the contribution. There is a five-year vesting schedule for the profit sharing contributions. STAFF 9 BENEFITS
CORPORATE RESPONSIBILITY Volunteer Time Eide Bailly supports giving back to our communities. The Firm provides eight hours of paid volunteer time annually so staff can give back to the organization of their choice when they want and how they want. The volunteer time will be prorated based on your status of 3/4 time of 1/2 time. Matching Donations In addition to volunteer time, we support you in your charitable financial donations. The firm will match up to $200 annually to each staff member s 501c3 organization of choice. OTHER BENEFITS Paid Time-Off (PTO) You will accrue PTO hours on a monthly basis. The accrual increases according to your position within the firm and length of service. Holidays We recognize the following holidays: New Year s Day; Memorial Day; Independence Day; Labor Day; Thanksgiving, the day after Thanksgiving; Christmas Eve (or other religious holiday); and Christmas Day (or other religious holiday). Direct Deposit Eide Bailly allows you to have your paychecks direct deposited into your bank account. You are allowed to designate up to five separate accounts, with one as the default. Employee Assistance Program (EAP) In this day and age there are many stressors in our lives. To help you cope with day-to-day problems, or major life crises, the firm offers confidential and professional counseling services through the Village Family Service Center. Maternity/Paternity Leave To help maintain that balance during some of the most important transitions in life, the firm will offer new moms eight weeks of paid maternity leave and new dads two weeks of paid paternity leave following the birth or adoption of a child. *We offer Milk Stork, a delivery service providing express shipping &/or easy toting of breastmilk. Business Travel Insurance This is a $50,000 life and disabling accident insurance policy for business travel paid for entirely by the firm. You have the choice of designating a beneficiary through myhr or using the beneficiary hierarchy contained in the policy. If you do not formally designate a beneficiary, the benefit paid for loss of life will be paid to the first of the following surviving beneficiaries: your (a) lawful spouse; (b) child or children, jointly; (c) parents, jointly if both are living or the surviving parent if only one survives; (d) brothers and sisters, jointly; (e) estate. Savvy Benefits SavvyBenefits provides access to telemedicine through Teledoc. Teladoc is a national network of U.S. board-certified doctors available on-demand 24/7 to diagnose, treat, and prescribe medication if necessary for many of your medical issues.
