Houseowner / Householder Insurance PRODUCT DISCLOSURE SHEET (Read this Product Disclosure Sheet before you decide to take out the Houseowner / Householder Insurance. Be sure to also read the general terms and conditions.) Date SCHEDULE 9 OF THE FINANCIAL SERVICES ACT 2013 (FSA) IMPORTANT NOTICE Consumer Insurance Contract Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance wholly for purposes unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form (or when you apply for this insurance). You must answer the questions fully and accurately. Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. In addition to answering the questions in the Proposal Form (or when you apply for this insurance), you are required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. What is this product about? This policy provides you with coverage for loss or damage to your building and/or contents of Private Dwelling Houses, Flats and Apartments solely for residential purposes caused by the perils mentioned in the policy. 2. What are the covers / benefits provided There are three sections in the houseowner / householder policy. You can opt to purchase either Section 1 or 2 or both Sections. Liability to the Public RM50,000/- any one accident or series of accidents constituting one occurrence This policy covers loss or damage to your building and/or contents caused by any of the undermentioned Perils fire, lightning, thunderbolt, subterranean fire, explosion, aircraft and other aerial devices and/or articles dropped therefrom, impact with any of the buildings by any road vehicles or animals not belonging to or under the control of the insured or any member of his family, bursting or overflowing of domestic water tanks, apparatus or pipes, theft but only if accompanied by actual forcible and violent breaking into or out of a building or attempt thereat, hurricane, cyclone, typhoon, windstorm, earthquake, volcanic eruption flood Subject to exclusions and excesses mentioned in the policy. You may extend to cover the following perils by paying an additional premium a) subsidence and landslip b) riot strike and malicious damage 3. How much premium do I have to pay? Houseowner / householder insurance is tariff - rated. The total premium that you have to pay may vary depending on the underwriting requirements of the insurance company. Please refer to the agent or the insurance company directly for more information. You must ensure that your property is adequately insured at the appropriate amount taking into account the renovations made to your property. Insured Value / Sum Insured Building sum insured = market value or reinstatement value Contents sum insured New items = purchase price Other items = market value which shall mean the value of the property insured less due allowances for wear and tear and / or depreciation Average Clause If at the time of loss, the property insured be of greater value than the sum insured, you shall have to bear a rateable proportion of the loss eg. Value at Risk Sum Insured Loss or Damage Insurer pays You will have to bear RM50,000/RM30,000/RM7,000/30,000/-/50,000/- x 7,000/- = RM4,200/RM2,800/-

Excess - It is the amount of loss you have to bear while your insurance company will pay for the balance of the loss or damage claim. Period of Coverage - Not exceeding 12 months unless to coincide with the expiry of other policies effected by you. Payment of Premium The premium due must be paid to Zurich General Insurance Malaysia Berhad within 60 days from the inception date of policy failing which the contract is automatically cancelled. Payment can be made by cash or cheques (cheques should be made payable only in the name of Zurich General Insurance Malaysia Berhad) Insist on a receipt for the premium paid to your insurance agent or insurance company. Contact Zurich General Insurance Malaysia Berhad if you have not received the insurance policy after one month of purchase. This list is non-exhaustive. Please refer to the policy contract for the term and conditions under this policy. 6. What are the major exclusion This standard houseowner / householder policy does not cover certain losses occasioned by War, Terrorism, Nuclear and Radioactivity Risks Subsidence and landslip Riot, Strike and Malicious Damage All losses / claims related to asbestos All other exclusions as per the houseowner / householder policy. However, you may pay additional premium to cover the above peril exclusions as mentioned in question 2. Note This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy. 7. Can I cancel my policy? You may cancel your policy by giving written notice to the insurance company. Upon cancellation, you are entitled to a refund of the premium based on short period calculation on the unexpired period of insurance subject to the minimum premium to be retained by the insurance company. 8. What do I need to do if there are changes to my contact details? It is important that you inform us of any change in your contact details to ensure that all correspondences reach you in a timely manner. 9. Where can I get further information? Goods and Services Tax Clause The amount of premium payable by You for this Policy includes an amount on account of the GST on the premium. When We pay a claim, Your GST status will determine the amount We pay. When You are (a) non-gst registered person, We will pay in full (including 6% GST) up to sum insured/limit of liability or the other limits of insurance cover. (b) GST registered person, We will pay (excluding 6% GST) up to sum insured/limit of liability or the other limits of insurance cover. You are to claim Your Input Tax Credit entitlement from the Royal Malaysian Customs Department directly. You must advise Us of Your correct entitlement to an Input Tax Credit on Your Premium and the correct entitlement to an Input Tax Credit on each item of the property to be insured.

Definitons under the Section on Glossary GST means goods and services tax and has the meaning assigned to it in the Goods and Services Tax Act 2014 ( GST Act ). Registered person means a person who is registered under Part IV of the GST Act and a non registered person shall mean a person who is not registered under the GST Act. Input Tax means the GST incurred on any purchase or acquisition of goods and services by a taxable person for the purpose of making a taxable supply in the course or furtherance of business. Input Tax Credit means the input tax claimable by a registered person. Polisi ini menawarkan perlindungan bagi kerugian atau kerosakan kepada bangunan dan/atau kandungan Rumah Kediaman Persendirian, Flat dan Pangsapuri yang digunakan semata-mata untuk tujuan kediaman akibat bencana yang disebutkan dalam polisi. 2. Apakah perlindungan / manfaat yang diberikan? Terdapat tiga bahagian polisi pemilik rumah/isi rumah. Anda boleh memilih untuk membeli sama ada Bahagian 1 atau 2 atau kedua-dua Bahagian. 11. Fasal Cukai Barangan dan Perkhidmatan Jumlah premium yang perlu dibayar oleh Anda bagi Polisi ini termasuk jumlah GST yang dikenakan pada premium. Apabila Kami membayar tuntutan, status GST Anda akan menentukan jumlah pembayaran oleh Kami. Apabila Anda (a) orang tidak berdaftar dibawah GST, Kami akan membayar sepenuhnya (termasuk 6% GST) sehingga kepada jumlah diinsuranskan/had liabiliti atau had lain perlindungan insurans. (b) orang berdaftar dibawah GST, Kami akan membayar (tidak termasuk 6% GST) sehingga jumlah diinsuranskan/had liabiliti atau had lain perlindungan insurans. Anda hendaklah menuntut secara langsung Kredit Cukai Input yang layak daripada Jabatan Kastam Diraja Malaysia. Anda hendaklah memaklumkan kepada Kami kelayakan Kredit Cukai Input Anda yang betul atas Premium Anda dan kelayakan Kredit Cukai Input yang betul pada setiap item harta benda yang diinsurans.
