POS PAC 3 PRODUCT DISCLOSURE SHEET (Read this Product Disclosure Sheet before you decide to take out POS Personal Accident Cover 3 (POS PAC 3). Be sure to also read the general terms and conditions.) 1. What is this product about? POS PAC 3 is a yearly policy that provides compensation in the event of injuries, disability and death of any person who is getting into, traveling in or getting out of the Insured s vehicle that caused solely by violent, accidental, external and visible events. 2. What are the covers I benefits provided? Benefits Description Sum Insured Benefit A Death RM40,000 per person Benefit B Permanent Disablement Injuries resulting in being permanently bedridden Any other injuries causing permanent total disablement Total and irrecoverable loss of sight in one eye. Total loss of use or by physical severance of r one hand or one foot. RM40,000 per person Benefit C *Benefit D Medical and Surgical Treatment for such injury in respect of any one accident. (Inclusive of Medical Report) Daily Hospital Income up to maximum of 30 days for any one accident Limit RM2,500 per person RM30 per day Note : i. Consumer Insurance Contract - Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this insurance wholly for purposes unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering the questions in the Proposal Form (or when you apply for this insurance). You must answer the questions fully and accurately. Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. In addition to answering the questions in the Proposal Form (or

when you apply for this insurance), you are required to disclose any other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. Non-Consumer Insurance Contract - Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this insurance for a purpose related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us any of the information given in the Proposal Form (or when you applied for this insurance) is inaccurate or has changed. Cooling-off period you may cancel your policy by returning the policy within 15 days after you have received the policy. The premiums that you have paid will be refunded to you. Cash Before Cover The premium due must be paid and received by Us before insurance cover is effective. Claims Procedure You must give written notice to Us within fourteen (14) days after the occurrence of the accident. You may cancel your policy by giving a written notice to the insurance company provided that you have not made a claim on the policy. Upon cancellation, you are entitled to certain amount of refund of the premium based on the scale of short period rates (please refer to policy contract for the short period rates). 8. What do I need to do if there are changes to my contact details? It is important that you inform us of any change in your contact detail/life profile including nomination, occupation and personal pursuits which would affect the risk profile. Tunai Sebelum Perlindungan Premium yang perlu dibayar mesti dibayar dan diterima oleh pihak Kami sebelum perlindungan insurans berkuat kuasa. Prosedur Tuntutan Anda perlu memberikan notis bertulis kepada Kami dalam masa empat belas (14) hari selepas berlaku kemalangan. Kelayakan untuk membeli Anda mestilah berusia antara 16 hingga 80 tahun. Usia dikira berasaskan usia pada hari jadi berikutnya. * Senarai ini tidak menyeluruh. Bolehkah saya membatalkan polisi saya? Anda boleh membatalkan polisi anda dengan memberi notis bertulis kepada Kami. Selepas pembatalan, anda layak mendapat bayaran balik premium berasaskan skala kadar tempoh singkat(sila rujuk kepada kontrak polisi untuk kadar tempoh singkat). 8. Apa yang perlu saya lakukan jika terdapat perubahan kepada maklumat hubungan saya? Anda perlu memberitahu kami tentang sebarang pertukaran profil hayat anda termasuk pekerjaan dan kegiatan peribadi yang akan menjejaskan profil risiko.
