Privacy and confidentiality are major concerns for everyone in today s environment. Your wellness provider, Kersh Health, is bound by HIPAA, State and Federal law to maintain the confidentiality of your personal information. Kersh Health will not share your personal health information with anyone at Kinross. The Wellness Program is completely voluntary. Spouses and domestic partners are invited to complete the Qscore Assessment and attend onsite testing or submit doctor s results. All employees will be rewarded with 50 bonus points if their spouse or domestic partner completes both activities. Please read through this Program Guide in its entirety. Kinross and Kersh Health Pg. 1 To enroll into the Wellness Program you must complete the Qscore Assessment and attend onsite testing, or submit a Biometric Results Form by 11/30/2015. You will receive your personal Qscore Report in the mail 2-3 weeks after results are received by Kersh. Your Qscore Report has been color coded to help you quickly identify areas where improvement is needed and suggestions are given on how to improve your health. What does your Qscore mean? The higher the Qscore the healthier you are! This number represents where you are in line to see a doctor compared to 100 other individuals with similar age, race, and gender. An individual with a lower Qscore will be towards the front of the line and may need medical attention before an individual with a higher Qscore. What does your Optimum Qscore mean? This score represents where you could be in the line if changes were made in your lifestyle (i.e. stop smoking, lower cholesterol, begin exercising). What is your Risk Status? This status is based on your Qscore responses and biometric results. Depending on your status, Low Risk (Qscore of >61) or At Risk (Qscore of <60), you will be required to complete certain Health Education activities to earn incentive points.
New-Hire Enrollment Information If you are benefit eligible between February 1 and August 1, you will follow the Pro-Rated Incentive Plan. To enroll into the Program, click Complete My Qscore on the Benefitsolver landing page or logon to KershHealth.com to complete the Qscore. The access code to complete the assessment is Kinross16. Use your Kersh ID if you are re-enrolling into the program. You must also submit biometric testing results within 31 days of your hire date. Your personal Qscore Report will mailed to your home and posted at KershHealth.com. Also, be on the lookout for your personal KAM device that will be mailed to your home as well. You should begin wearing your KAM immediately to track your activity and earn incentive points. All participants may log activity at any time through use of the Activity Log located on the Kersh website. Follow the Pro-Rated Incentive Plan below to determine how many incentive points you must earn to receive a reduction in 2017. At Risk Participants (Qscore of 60 and below) Health Coaching (Q-Quest) - Speak with a Kersh Registered Dietitian (RD) at least once per quarter. Targeted participants, those with a Qscore of <60, will be proactively contacted by a Kersh RD. Refer to your Risk Letter or log into your Kersh Health account to determine your risk status for the program year. Health Education Deadlines: January 1 - March 31, April 1 - June 30, July 1 - September 30, October 1 - November 30, Pg. 3 Right Weigh is a FREE, innovative, and effective 12-week weight management program that is available to employee participants and their spouses. The Right Weigh program was designed by medical professionals to teach individuals how to Think, Eat, and Move their way to decreased weight and optimized health. There are two versions to this program: Advanced and Basic. Right Weigh Advanced provides participants with an opportunity to work one-on-one with a Kersh Registered Dietitian throughout a 12-week course. The first session begins January, and additional sessions begin approximately every six weeks throughout the program year. You will be contacted if you are selected to participate in Right Weigh Advanced, or you may call Kersh Health to see if you qualify. Participation in the Right Weigh program is voluntary. Right Weigh Basic (self-paced) materials will be provided by a Kersh RD if you are deemed ineligible for Right Weigh Advanced.
Kinetic Activity Monitor (KAM) KAM A small, durable device that uses integrated state-of-the-art accelerometer technology to measure human activity and is Bluetooth compatible. Instead of tracking steps, the KAM records intensity, duration and frequency of movement. The KAM displays KAM points earned, calories burned, and levels of activity. KAM Details The KAM device holds information for 45 consecutive days, but we recommend docking your KAM more often to avoid lost data. Please note the time displayed on the KAM correlates with the time on the computer you dock to, which may change based on time zones. The KAM should be worn on the waist, in-line with the knee. Only during an activity where your hips are stationary, i.e. using a stationary bike or elliptical trainer, should the KAM be worn on the shoe. The new model KAM is water resistant. For activities not monitored by the KAM, i.e. swimming or weight lifting, you may log your activities using the Activity Log located on the Kersh website. Lost or Malfunctioning KAM If you lose or damage your KAM, contact your Kinross Wellness/HR Team. KAM customer service is available at 800-467-3005 or Support@KershHealth.com. Kinross will pay the cost for one replacement KAM per calendar year. You will be charged $17.50 for the second replacement and $35 for the third and subsequent replacements per calendar year. Fitbit Device Integration Participants using a Fitbit have the option to use this activity device in lieu of the KAM device. Follow the instructions in your Kersh account to upload your Fitbit data and earn incentive points for the KAM portion of the Wellness Program. Please note Fitbit data storage capacity may vary from the KAM. Kersh Health and Kinross will not offer technical support or reimbursements for this device. Pg. 4

Annual Exams Form Submission Preventive visits are an important step in maintaining good health. Advantages include regular checks for disease, increasing your chance for early detection and improved recovery, the opportunity to get any needed vaccines, and to discuss recommendations with your doctor for lifestyle changes and additional screenings based on age and family history. Exams dated between January 1, and November 30, will be accepted. The deadline to submit all Annual Exam forms is November 30,. To complete the Annual Exam forms and receive incentive points, you will be required to submit the exam details listed below.
