Skydive Cambridgeshire 2018 Terms & Conditions The skydive is promoted by Parkinson s UK, a charity registered in England and Wales No. 258197 and in Scotland No. SC037554. The skydive is organised in association with the North London Skydiving Centre at Chatteris Airfield. The North London Skydiving Centre is responsible for the logistics of the event on the day. These terms and conditions apply to all participants. For participants aged 16 or 17 their parent or guardian is accepting the terms and conditions on their behalf. Participation 1.1 Age restrictions apply. Participants must be 16 years or over on the day of the skydive. 16 and 17 year olds must get their parent or guardian to complete the registration form on their behalf. 1.2 Health restrictions apply. Participants must be in suitable physical condition to undertake the challenge (your height and weight should be in proportion as there is an increased risk of injury if this is not so) and be fully aware of the possible risks inherent in the challenge activity. Some of the medical conditions that could impact on your participation include, but aren t limited to: epilepsy, fits, severe head injury, recurrent blackouts or giddiness, disease of the brain or nervous system, high blood pressure, heart or lung disease, dislocated shoulder, recurrent weakness or dislocation of any limb, diabetes, mental illness, drug or alcohol addiction and some cases of asthma. If you have any concerns please contact the Events Team on 020 7932 1356 before proceeding. All participants must complete the medical information section of the registration form. Only once we have reviewed and approved this information will your place will be confirmed and your registration fee payment processed. 1.3 By providing details of your medical history and GP you are giving permission for Parkinson s UK and the British Parachute Association to discuss medical conditions relevant to this challenge with your GP or consultant as appropriate. If any of the information provided during registration is incorrect your place may be withdrawn and you may be liable to pay the challenge costs. 1.4 If your health situation changes or your weight changes dramatically it is your responsibility to inform the charity as soon as possible, and your participation may need to be re-evaluated. 1.5 All participants must complete the relevant Tandem Student Parachutist Medical Information and Declaration Form, and bring this on the day of the skydive. If you are medically fit you can self certify using Form 115A. If you have any medical conditions, you must consult your GP to sign and stamp the Form 115B. A copy of Form 115A and 115B will be sent to you in your welcome pack. Under 18s who are self certifying using Form 115A will need their form signed by their parent or guardian. If you arrive without the necessary paperwork you may forfeit your skydive. 1.6 Weight restrictions apply. Participants must be under the weight limit of 15 stone at the time of booking. This weight must be in proportion to your height and this is especially important if you weigh 13 stone or more. You

must also be able to lift your knees up to your chest for the required amount of time. 1.7 We reserve the right on reasonable grounds to decline your request to participate in the challenge. The final decision on your suitability to tandem skydive rests entirely with the airfield and their instructors. 1.8 In the event of illness or accident during the skydive medical staff will initiate treatment and Parkinson s UK will notify your next of kin as appropriate. 2) Challenge Costs & Sponsorship 2.1 Places are subject to availability and will be allocated on a first come, first served basis for those who correctly complete the booking formalities. 2.2 To book your place a registration fee of 55 must be paid to Parkinson s UK when returning the completed booking form or via our online booking process. Your registration fee is non-refundable. Payment will only be taken once your place has been approved. Once we have processed the payment we will issue you with a receipt. 2.3 You must raise a minimum of 495 in sponsorship. You will be required to make up the balance yourself should you not reach this target. There is no maximum limit. 80% of your 495 target ( 396) must be sent to Parkinson s UK at least ten weeks before the skydive (14 July 2018). Failure to receive this will mean non-participation in the challenge and forfeiture of the registration fee. The remaining 20% ( 99) must be sent to Parkinson s UK four weeks after the skydive (15 October 2018). 2.4 Your 55 registration fee is comprised of a 50 deposit that will be paid to the airfield, and a 5 administration fee. Of the 495 minimum sponsorship that you are required to raise, a maximum of 195 will be paid to cover the balance of your skydive costs. The surplus will be retained by Parkinson s UK. 3) Fundraising 3.1 You must not start to collect sponsorship until you have received confirmation of your booking from us. 3.2 When fundraising you must make it absolutely clear how much you are raising and how much of the sponsorship will contribute towards your costs. Please note due to the financial commitment made by Parkinson s UK to secure your place, the minimum sponsorship you raise cannot be redirected to local funds. 3.3 You agree to raise all funds through legal methods and activities and in compliance with the guidance received. Parkinson's UK accept no responsibility for any liability you incur by not adhering to this requirement. 3.4 You will endeavour to have all sponsors complete a Parkinson s UK sponsorship form so that the charity can claim Gift Aid on all eligible donations. Please note Gift Aid will not be counted towards your minimum sponsorship target. 3.5 Payment of the registration fee and all sponsorship money should be made to Parkinson's UK. Under no circumstances can money be raised for another charity or organisation. Sponsorship money raised for the skydive cannot be subsequently used for another event, even if the minimum

sponsorship for the first event has been exceeded, subject to the following exceptions: Should an event be cancelled or a participant be unable to take part and produce a doctor s or other valid certificate justifying withdrawal from the first event, money pledged or given towards that event may be transferred. 3.6 From the time of the transfer all subsequent sponsorship must be for the second event. 3.7 No money will be repaid to the participant. 3.8 If you cancel, or are required to withdraw from the skydive, you will be required to send all sponsorship forms and money collected directly to Parkinson s UK. This money will be retained by Parkinson s UK. If your sponsors wish to request a refund of their donations, they must apply in writing to Parkinson's UK. 4) If you amend or cancel your participation 4.1 All changes will be subject to availability. 4.2 If Parkinson s UK refuses to authorise your participation in the skydive after you have registered, your registration fee won t be processed. 4.3 If you choose to postpone your participation in the skydive and the skydive you wish to move to has a higher registration fee or minimum sponsorship level, you will be responsible to pay the difference. 4.4 If you wish to change dates but still jump at the same airfield there will be an amendment fee of 50. 4.5 If you wish to cancel the jump and not re-book, and you inform us at least eight weeks prior to the challenge you will only lose your registration fee. If you inform us less than eight weeks prior to the challenge, you may lose the full cost of the challenge. 4.6 If bad weather (or circumstances beyond the airfield s control) prevents you from jumping on the selected date, Parkinson s UK will do everything possible to arrange a new date for the whole group to participate together at the same location, at no extra cost. If this date isn t suitable you can arrange an alternative date at the same location, at no extra cost. In order to rebook, you will need to speak directly with the airfield concerned. Restrictions apply. 4.7 All amendments and cancellations should be confirmed to us in writing. Please note cancellations are only effective from the day that they are received by us. 4.8 If you are unable to take part, you must notify Parkinson s UK at the earliest opportunity. 5) If we amend the event 5.1 We cannot accept responsibility for airfields changing or postponing operating days however we will transfer your booking to an alternative date at the same location at no charge if required. We are not liable for any penalty charges associated with travel or accommodation, in the event of a change to the date of the skydive.

5.2 Force Majeure: We are not responsible for changes which arise as a result of situations outside of our control such as technical or maintenance problems with means of transportation, war or threat of war, civil strife, industrial disputes, natural disasters, bad weather, bureaucratic obstacles or terrorist activity. 6) If we cancel the event 6.1 Participants understand that Parkinson's UK reserves the right to change the details of the skydive, including the venue, or delay, postpone or cancel in the event of adverse weather conditions or other circumstances outside the control of Parkinson's UK. The determination of what constitutes such conditions is reserved solely by Parkinson's UK. 6.2 If we have to cancel your skydive (other than where Force Majeure applies) we will offer you either (i) an alternative challenge of comparable type (if the alternative offered is at additional cost, the difference will be payable by the Charity) or (ii) a full refund of your registration fee, in either case being the only recompense which will be due to you. Refunds of sponsorship money will be dealt with in accordance with Clause 3.8. 7) Our liability 7.1 We will only place your skydive challenge at an affiliated British Parachute Association (BPA) airfield. The BPA is the National Governing Body of the sport. 7.2 Parkinson's UK is unable to accept liability for any loss or damage, however arising, or for cancellation of the event for any reason outside its control. We cannot be held responsible for delays and changes to arrangements that are caused by force majeure. 7.3 Parkinson's UK shall not be liable for any injury, illness or death of any person or persons nor loss or damage to any property nor for any indirect or consequential loss however arising other than from injury, illness or death as a direct result of negligent act or omission of their servants, agents or employees acting in the course of their employment. 8) Insurance 8.1 Included in the challenge is a British Parachute Association Liability Insurance Policy which is co-ordinated by the airfield. Please refer to the airfield s terms and conditions for further details. Participants may also wish to take out a separate policy that provides additional cover which they would have to arrange themselves. 9) Risks 9.1 Skydiving is an adventure sport and participation in such sports necessarily involves a risk of injury or death regardless of the standard of training, supervision and equipment employed. You must be adequately fit to undertake the challenge. You therefore take part entirely at your own risk and agree to indemnify Parkinson s UK, our employees, agents, subcontractors and suppliers against claims for loss or damage to personal

property or for loss or consequential losses or claims through your participation in the skydive arising from your own actions. All participants must wear safety clothing or equipment such as may be required by the airfield as required by the British Parachute Association. You must not consume alcohol on the day of your challenge or to excess the night before. 9.2 You must comply with all reasonable instructions of the skydiving instructors or airfield staff relating to the safety and organisation of the challenge. If in our opinion, any member of staff or other person in authority feels that you are behaving in such a manner as to cause danger, distress or annoyance to others or cause damage to property, your skydive arrangements may be terminated by us or the airfield. In such an event, we shall have no liability to you and will not be responsible for making any refunds, paying any compensation or meeting any costs or expenses you incur as a result. Furthermore, you must meet any expenses we incur as a result of your behaviour. 9.3 The participant is fully responsible for their actions during the event. Parkinson s UK does not accept responsibility for the actions of the participant nor the consequences of such actions. 10) Personal data and images 10.1 I will not use the Parkinson s UK logo without permission and will obtain all fundraising materials from Parkinson s UK. 10.2 I consent to photographs, video, quotes and any other record of my participation in the event being used for marketing and publicity purposes, both online and in print. If I do not wish my photos or quotes to be used for any purpose, I will email events@parkinsons.org.uk or call 020 7932 1356. 10.3 By registering for this I agree to receive emails, mailings, and marketing from Parkinson s UK in relation to my participation in the event. I have also confirmed with those registered by me that they agree to receive emails, mailings, and marketing from Parkinson s UK in relation to their participation in the event. 10. 4 We will process the data you have provided for this event in line with our fundraising promise and privacy policy. You can find more details here. If you wish to discuss how we process your data please contact our Supporter Services Team on 0800 1386593 or email them at supporterservices@parkinsons.org.uk Sensitive information, which includes any medical information that you provide, will not be used for marketing purposes but will be held in accordance with current data protection regulations for up to 6 years.
