This packet is designed to provide an overview of Frontier with key information to prepare you for a smooth transition. EMPLOYEE PAY Base Pay Your base pay will remain the same upon the closing of the transaction. CWA 1298 Represented Employees You will continue to be paid bi-weekly, following the same payroll cycle. Management Hourly Employees You will be switched from semi-monthly to bi-weekly. Management Exempt Employees You will be paid on Frontier s semi-monthly pay cycle. Payments are made on the 15th and last day of the month. Time Entry All hourly employees are required to have time entered into Frontier s time entry system (CATS). On Day 1, you will be enrolled in an online class that will provide instructions on how to use CATS. IMPORTANT: CATS does not track scheduling, so it is imperative that hourly employees enter their hours for Payroll each week. If no hours are entered, no pay will be generated. Hours of Operation are M-F, 8 AM to 6 PM Eastern. Commission Payments Commission payments will continue for eligible sales staff. You will receive further guidance on your new commission plan in a few weeks. Management Bonus Eligible Management Employees will receive their bonus in the first quarter of 2015. Your bonus will be based proportionately on both AT&T and Frontier year-end results and your individual performance. Success Sharing Plan (CWA 1298 Represented) Eligible CWA 1298 Represented Employees will receive their 2014 Success Sharing Plan in November as outlined in the collective bargaining agreement. Payment is expected to be made by Frontier on November 21, 2014. Founder s Grant All transferring employees are eligible for the Frontier Founder s Grant. Each transferring employee will receive 100 shares of Frontier restricted stock granted October 27, 2014. These shares will vest after three years. Your Founder s Grant will be administered by Fidelity. You can contact Fidelity at 1.800.544.9354 or www.fidelity.com. You will receive an email from Fidelity with instructions as to how to accept your grant, and details about the terms and conditions of the grant. TRAVEL AND REIMBURSEMENTS ProCard (Company Credit Card) ProCards will be issued and distributed on Day 1 to eligible employees. Frontier uses Citibank as its primary MasterCard provider. If you have been issued a card, you will be sent a welcome email. Cardholders are required to attend training. You will be enrolled in the online training courses. Contact the ProCard team at 1.800.790.9177 or ProCard@ftr.com. Hours of Operation are M-F, 8 AM to 5:30 PM Eastern. Reimbursements Eligible reimbursements after the closing of the transaction must follow Frontier s guidelines. HR POLICIES & CAREERS Vacation, Sick Time, and Holidays You will accrue paid time off (PTO) days on a monthly basis, earning one-twelfth of your allotment on the 16th of each month. CWA 1298 Represented Employees will continue to receive vacation in accordance with the collective bargaining agreement. CWA 1298 Represented Employees will continue the holidays provided for in the collective bargaining agreement. Management Employees will accrue PTO under Frontier s PTO policy after the closing of the transaction. Contact IT Support at 1.866.745.9122; 1.585.777.9122 or ITSupportCenter@ftr.com. SAFETY & SECURITY Security Badges Continue to use your AT&T access card for electronic entry into our buildings. Frontier will provide you with your Frontier ID Badge, which is your card for electronic building access. When you receive your Frontier ID Badge, you will give your AT&T access card to a Frontier representative who will likely be your Supervisor or someone in area management. If you have any issues regarding building access, contact the Frontier Security Operations Center (SOC) at 1.800.590.6605. The SOC is a 24/365 service that handles Security and Safety issues. Safety Reporting, Policies and Program Report any work-related injury or illness immediately to your Supervisor, who will complete a Frontier Accident Injury/Illness Report Form. This Form can be found on the Link at http://home.fcinternal.net/safety/default.asp?id=22 along with instructions, and must be submitted within 24 hours by your Supervisor to the Regional Safety Manager. Motor Vehicle Accidents (MVA) must be reported to law enforcement, our MVA vendor PHH, and your Supervisor. Motor Vehicle Accident Report Forms must be completed within 24 hours. The form is available on the Link at http://home.fcinternal.net/safety/default.asp?id=22. This MVA form must be submitted to your Supervisor who, in turn, will provide a copy to the Regional Safety Manager. You are required to review the Frontier Environmental, Safety & Security Manual found on the Link at http://home.fcinternal.net/safety/default.asp?id=9. After you review, you must complete the acknowledgment page by December 31, 2014. Security and Emergency Reporting In the event of a life-threatening or serious emergency, call 911; then call the Security Operations Center at 1.800.590.6605. If you have a non-emergency security to report, visit the Corporate Security webpage on the Link at http://home.fcinternal.net/corp_security/default.asp?id=2. Click on Security Incident Report and complete the Confidential Report Form. An Environmental Health and Safety Services (EHSS) professional may be reached 24/7 by contacting the Security Operations Center at 1.800.590.6605. HEALTH & WELFARE BENEFITS Benefits Administration Your Frontier health and welfare benefits are effective beginning October 25, 2014. Health and welfare benefits are administered through Empyrean Benefit Solutions. Accessing Benefits Information You can access your health and welfare benefits information online or over the phone beginning October 25, 2014 at www.frontierbenefitscenter.com or through Frontier s HR site on the home page of the Link. o Your User ID is the first two letters of your first name, first three letters of your last name (all lower case) and the last four numbers of your Social Security Number. Your initial password is your date of birth (MMDDYYYY). You are required to change your password after you log in for the first time. To speak to a Frontier Benefits Service Center representative, call 1-855-FTR-2887, M F, 9:00 AM to 6:00 PM Eastern. 4 5

Benefits Coverage ID Cards You will receive new medical, prescription drugs and vision ID cards. These were mailed to your home address the week of October 20, 2014. Remember, these cards are effective from October 25, 2014. If you need care prior to October 25th, you are required to use your AT&T ID card(s). If you do not receive your Frontier ID card(s), you still have coverage through Frontier s plans. If you need to visit a doctor or access care before your Frontier ID card arrives, you may print a Temporary ID card. Give this information to your doctor or have your doctor s office call the carrier s Customer Service Representatives (CSRs) for plan information and coverage verification. Carrier s contact information and step-by-step information about how to print a Temporary ID card are available at Frontier Benefits Service Center at www.frontierbenefitscenter.com or the Link. Information about how to print a Temporary ID card is also available below under Medical, Prescription Drugs, and Vision Plans Administrator. Reminder: Cigna Dental does not issue ID cards unless you are enrolled in the Dental HMO Plan. Stop using your AT&T healthcare ID card after October 24th. Important: Please advise your healthcare providers that all claims incurred prior to October 25, 2014 must be submitted under your AT&T plans and claims incurred on or after October 25, 2014 will be submitted under your Frontier plans. HIPAA Notification Employees currently participating in the AT&T Health & Welfare plans will receive a HIPAA notice from AT&T s benefit administrators notifying them that their coverage through AT&T has been terminated effective as of the closing of the transaction. AT&T is required to send the HIPPA notice by law. Your coverage under the Frontier platform begins October 25th. There will be no lapse in your benefits. Medical Plans Administrator (CWA 1298 Represented Employees) CWA 1298 Represented Employees: Your medical plans administrator is transitioning from Blue Cross Blue Shield (BCBS) of IL to Anthem BCBS, effective October 25, 2014. Both BCBS of IL and Anthem BCBS utilize the same provider network, BlueCard, eliminating the risk of provider disruption for members. Accumulators (deductible, out-of-pocket max, etc.) will transfer from BCBS of IL to Anthem BCBS. Participants will receive new ID cards from Anthem BCBS. Please notify your healthcare providers that all claims prior to October 25, 2014 must be submitted to BCBS of IL and claims incurred on or after October 25, 2014 to Anthem BCBS. If You Require A Temporary ID Card: Logon to www.anthem.com. Create a User ID and Password. On the MyAnthem Home page, the Print Temporary ID card link can be found under the Resources section of the screen. Click the link Print Temporary ID card to print a temporary ID card. Medical Plans Administrator for Management Employees Management Employees: Your medical plans will continue to be administered by United Healthcare (UHC). Covered employees will receive new ID cards. If You Require A Temporary ID Card: Logon to www.myuhc.com. Register as a new user (your AT&T User ID and Password are no longer valid). On the Home page, click the Account Setting link located at the top of the page. Under My Account Setting, select the option to Print a temporary ID card. Customer Service Representatives: 1.888.876.7084, available M F, 8:00 AM to 8:00 PM, Eastern. Prescription Drugs Administrator Caremark/CVS will continue to administer the Prescription Drug plans. Participants will receive a new ID card(s). If You Require A Temporary ID Card: Log on to www.caremark.com. Register using your Social Security #, followed by 01 for the primary cardholder, 02 for spouse of the cardholder, and 03 and up for dependent children. Directions under the Understanding My Benefits tab instruct you how to print a Temporary ID card. Mental Health, Substance Abuse and EAP ValueOptions will continue to administer the Mental Health/Substance Abuse and EAP Plans. Current Plan designs are replicated. ValueOptions does not issue ID cards. Customer Service Representatives: 1.877.342.3389, available 24/7. Dental Plans Administrator Cigna will continue to administer the dental plans. Your current plan design will remain the same. Cigna only issues ID cards for members enrolled in the Dental HMO Plan. Dental HMO Plan Participants: If you are enrolled in the Cigna Dental HMO plan, you will receive an ID card(s) and notification of the plan design from Cigna. Customer Service Representatives: 1.800.CIGNA24 (1.800.244.6224), available 24/7. Vision Plans Administrator EyeMed will continue to administer the vision plans. Covered employees will receive new ID cards. If You Require A Temporary ID card: Logon to www.eyemed.com. Click on Member Login. Click on the Create an Account link under New User. Register as a new member by using the last four digits of your Social Security Number. Click on View Your Benefits, from the drop down menu and then click on Print ID card. Customer Service Representatives: 1.866.723.0514, available M F, 8:00 AM to 11:00 PM, Eastern; Sat. 11:00 AM 4:00 PM, Eastern; Sun. Noon 4:00 PM, Eastern. Life Insurance Life Insurance is transitioning from MetLife to Minnesota Life (MN Life) effective October 25, 2014. AD&D Insurance AD&D Insurance is transitioning from MetLife to Zurich effective October 25, 2014. Flexible Spending Accounts (FSAs) Flexible Spending Account(s) will continue to be administered through ADP. Paper claims should continue to be submitted to ADP. Participants may use their existing log on information to access their account. FSA Participants In order for ADP to move claims and fund history from AT&T s platform to Frontier s platform, there will be a Black-out period from October 25-30, 2014. During that time, participants will be unable to access their FSA accounts and/or submit claims via the ADP Spending Account website. Effective October 31, 2014, you may log on to https://myspendingaccount.adp.com to access your FSA accounts and submit claims. Customer Service Representatives: 1.800.678.6684, available M F, 8:00 AM to 8:00 PM, Eastern. Health Reimbursement Accounts (HRAs) Health Reimbursement Accounts (HRAs) will continue to be administered through ADP. Paper claims should continue to be submitted to ADP. Participants may use their existing log on information to access their account at netbenefits.com. HRA Participants In order for ADP to move the claims and fund history from AT&T s platform to Frontier s platform, there will be a Black-out period (October 25-30, 2014). During that time, participants will be unable to access their HRA account and/or submit claims via the ADP website. Effective October 31, 2014, you may log on to https://myspendingaccount.adp.com to access your FSA accounts and submit claims. Customer Service Representatives: 1.800.678.6684, available M F, 8:00 AM to 8:00 PM, Eastern. Health Saving Accounts (HSAs) Health Saving Accounts (HSAs) will continue to be administered through Fidelity. Your current Debit Card will transfer to Frontier. Participants may use their existing log on information to access their account at www.netbenefits.com. Customer Service Representatives: 1.800.835.5095, available M F, 8:30 AM to 8:00 PM, Eastern. Commuter Account(s) Commuter Accounts will continue to be administered through WageWorks. You will continue to enroll on the WageWorks site and Payroll deductions will continue. Participants are required to re-register as a new user. Your AT&T login information is no longer valid. Log on to www.wageworks.com. Click on Login/Register, Employee Registration and follow the prompt to create a new User ID and Password. Important: Your ticket for the following month must be purchased by the 10th of the current month (e.g., December s ticket must be purchased by November 10th). Mercer Voluntary Plans Voluntary benefits will continue to be offered. The Mercer platform utilized by AT&T is designed for larger employers. As a result, Mercer will be utilizing their mid-size enrollment platform for Frontier, managed by Paylogix. Not all products currently offered to AT&T can be replicated for Frontier. Travelers Insurance Company declined to replicate the Home & Auto insurance plan they offered to AT&T to Frontier due to lack of participation. Currently, there are no employees participating in the Travelers plans. ID Theft, Voluntary Accident and Cancer Expense policies will not be converting. Participants will receive a written notification from Mercer that may continue their policies through direct billing. Financial Planning will not be converting and will not be transitioning to direct billing; this benefit will end. Access your information through the new website at http://frontiercommunications.e.paylogix.com. Customer Service Representatives: 1.866.796.2774, available M F, 9:00 AM to 6:00 PM, Eastern. Long-Term Care (LTC) John Hancock declined to contract with Frontier due to lack of participation by Connecticut AT&T. Current participants will receive a written notification from John Hancock that may continue their LTC plan with John Hancock through direct billing. WorkLife WorkLife will continue to be administered through LifeCare. Employees with existing policies must re-register with a new Registration Code. Log on to http://member.lifecare.com, click the Start Now link and enter the new Registration Code: Frontier. Customer Service Representatives: 1.888.473.9123, available 24/7. DISABILITY Short-Term Disability (STD) If you are on disability or a disability-related injury at the time of the closing of the transaction, you will transition to Frontier once you have been cleared to return to work. Frontier s short-term disability program is administered by Sedgwick and Prudential. Starting October 25, 2014, new STD claims and Family Medical Leave Act claims will be reported to Prudential. If you have a relapse of a recent STD claim, your new absence period must be reported to Sedgwick. If you have any further questions with regards to STD, please contact Prudential at 1.855.FTR.4PRU or www.prudential.com. Long-Term Disability (LTD) Frontier s long-term disability (LTD) is administered by Prudential. In the event you exhaust your STD benefits for a claim that was initiated after October 25, 2014, you may be eligible for LTD. Prudential will manage that process. Contact Prudential at 1.855.FTR.4PRU or online at www.prudential.com if you have questions. 401(k) & PENSION 401(k) Plan Your Frontier 401(k) benefits will be administered by Fidelity. Your 401(k) payroll deductions, including loans, will remain the same following the closing of the transaction. Your account balance is scheduled to transfer to the Frontier 401(k) Plan on the second Friday after the closing of the transaction. Your account will be available to access in mid-november; at that time you can make changes to your account. Investments will map to similar funds in the Frontier 401(k) Savings Plan. Pension Plan and Retiree Health Care Your Frontier Pension benefits will be administered by Milliman. Your Pension balance can be viewed at www.millimanbenefits.com after the closing of the transaction. Your Pension history will be available on www.millimanbenefits.com approximately 60 days after the transaction close. A packet of information will be mailed to your home with Milliman website information. If you want to retire prior to January 1, 2015 please contact Milliman at 1.866.333.2074, option 1, for Pension information and option 2 for Retiree Health Care.
