4001 Rodney Parham Road Original Title Page Effective: August 14, 2006 TITLE PAGE This PRICE LIST applies to Long Distance Communications Service ( LDCS ) furnished by Windstream Systems of the Midwest and hereinafter referred to as the Carrier, for interstate or international communications. LDCS for interstate communications is furnished between one or more stations in the Operating Territory of the Carrier and one or more stations in the different states in the contiguous United States, Alaska, Hawaii, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, American Samoa, Guam, and the Commonwealth of the Northern Mariana Islands. LDCS for international communications is furnished between one or more stations in the Operating Territory of the Carrier and one or more stations in Canada, Mexico or International Overseas locations. LDCS is furnished by means of the wire, radio, satellite, fiber optics, or any suitable technology or combination of technologies employed by Carrier and/or Carrier's underlying carrier or carriers. LDCS is furnished subject to the availability of facilities, and subject to transmission, atmospheric and like conditions.

4001 Rodney Parham Road Original Page 3 Effective: August 14, 2006 1.1 REGULATIONS 1.1.1. Description of Service SECTION 1 - GENERAL SERVICE OFFERINGS A. Carrier provides interstate communications service to customers for their direct transmission and reception of voice, data and other types of telecommunications between various locations within the continental United States, Alaska, Hawaii, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, Guam, American Samoa, and the Commonweatlh of the Northern Mariannas. Service is available on a full-time basis seven days a week. B. Basic service is provided on the basis of two different service options (Dial-Up and Dedicated). Depending on the option(s) chosen, the customer may select the manner in which calls are originated and terminated. (1) Users who have "Feature Group D equal access" availability are considered dial-up service customers. However, when using their "equal access" lines, they simply dial 1 plus the area code and telephone number desired. In "equal access" areas, customers can also dial 10579 to reach Carrier's switching equipment. Once answered, the calling procedure is the same as for true dial-up service. Dial-Up service may also be originated when the customer dials one of the exchange telephone numbers of the Carrier. When the call is answered, the customer must key in his authorization code followed by 1 plus the area code and telephone number desired. (2) Dedicated service originates when the customer accesses the Carrier via dedicated facilities between his premises and Carrier's terminal location. When the call is acknowledged, the customer must key in 1 plus the area code and telephone number desired. C. The customer is charged no monthly fee for the first authorization code, only a charge for each individual call placed during the month. Charges for calls are both distance and time sensitive. Please refer to Section D for the schedule of rates and charges. D. Service is generally provided to customers on a subscription basis, which is termed Basic Service. E. The customer is responsible for the charge, local or toll, incurred in accessing the Carrier's switch, said charge to be billed to the originating number, by the appropriate local carrier in the absence of special billing instructions from the customer. F. To obtain Basic Service, the customer, as an "equal access" subscriber, dials 1+. At additional cost, the customer may obtain additional authorization codes or use project codes to identify the users or user groups on his account and to allocate the cost of his service accordingly. The numerical composition of such codes is set by Carrier to assure compatibility with Carrier's accounting and automation systems and to avoid duplication of codes.

Description of Service (Cont d) G. Carrier's optional "Call Home" service allows a customer to call back to his "home base", e.g., Lincoln or Omaha, via an 800 number terminating on the carrier's switch. After the witch answers the 800 trunk, the customer follows the same dialing instructions as those for "dial up" service to reach his desired number. H. At additional cost, the customer may obtain exclusive use of a dedicated port on Carrier's switching facility. I. Customers may receive an additional discount on "eligible" 1+ usage through "Special Calling Programs." J. Customers may obtain assistance in determining telephone numbers by calling a Directory Assistance (D. A.) number. As many as two requests may be made on each call to Directory Assistance. The D. A. charge applies whether or not the requested phone numbers(s) can be furnished, e.g., where the requested number is non-published, unlisted, or no record can be found. Those handicapped Customers who qualify for exemptions from local Directory Assistance charges under Local Exchange Company tariffs are exempted from the Directory Assistance charge. This exemption applies only to calls to Directory Assistance which are billed to the handicapped Customer's residence telephone number by The Lincoln Telephone Company. K. At additional cost customer may obtain exclusive use of an in bound 800 number which permits calls made to that number from other locations to terminate via the carrier's switch at the customer's designated premises. L. Under certain circumstances, funds provided under the American Recovery and Reinvestment Act of 2009 ("ARRA") may be subject to certain restrictions, requirements and reporting obligations. The Company may be subject to some of these restrictions, requirements and reporting obligations when services and service components are purchased with ARRA funds. In order to comply with the restrictions, requirements and reporting obligations associated with the use of ARRA funds (if any), the Company must be apprised of them before provisioning the services or service components. Accordingly, the services and service components provided under this tariff shall not be used to support the performance of any portion of a project or program which has been funded in whole or in part with grants, loans or payments made pursuant to the ARRA, without the prior written agreement of the Company and Customer regarding any specifically applicable terms, conditions and requirements. Customer shall provide the Company with prior written notice before placing any order that may be funded in whole or in part with ARRA funds. If Customer fails to provide such prior written notice of ARRA funding; or if the parties cannot agree on the terms and conditions (if any) applicable to an ARRA funded order; or if any terms, conditions or requirements (other than those to which the Company specifically agrees in such separate writing) are found to be applicable, then the Company may, in its sole discretion, reject such order or immediately terminate the provision of any affected service or service component without further liability or obligation. 1.1.2. Limitations on Service A. Service is offered subject to the availability of the necessary facilities and/or equipment and subject to the provisions of this Price List. Carrier reserves the right not to provide service to or from a location where the necessary facilities or equipment are not available. B. Carrier reserves the right to discontinue furnishing service, upon written notice, when necessitated by conditions beyond its control, or when the customer is using the service in violation of the provisions of this Price List, or in violation of the law. Universal Service Fund (USF) Charge Pursuant to Federal Communications Commission (FCC) orders, carriers may flow through the FCC s USF charge to end user customers, in order to fund the Universal Service Fund. One of the purposes of the USF is to support high cost telephone areas. This charge will be applied monthly and the charge for a given month will be derived by applying the USF percent to the total of the Interstate toll on that month s bill. The USF percentage shall be dictated by the FCC and adjusted periodically in accordance with FCC requirements. B. Payphone Surcharge A surcharge that applies to completed intrastate, interstate and international long distance calls placed from any domestic payphone used to access the Company s services. This includes calling card service, toll-free service, and prepaid calling card service. The Payphone Surcharge is in addition to any other applicable service charges or surcharges. The Payphone Surcharge does not apply to calls placed from pay telephones at which the Customer pays for the service by inserting coins during the progress of the call. Interstate Service Fee The Interstate Service Fee (ISF) allows the Company to recover costs, such as those associated with provisioning and maintaining customer accounts on the Company s network, as well as interstate and international connection charges. The ISF will be assessed per account on a monthly basis to residential and business customers who subscribe to the Company s long distance interstate service. ISF Monthly Rates D. Bill Copy Fee Residential, per account $4.76 (I) Business, per account $4.97 The Company will charge a fee of $3.00 per bill copy when a customer requests a copy of a previous month s bill.

RATE DETERMINATION - The chargeable time for a call is determined by the duration of the call. Chargeable time includes the initial period plus the additional time involved, if any, and is determined as follows: 2.1.1. The per-minute rates and related one-time and recurring charges applicable to Carrier's "Dial up", "Dedicated" and "Call Home" services are as specified in the following rate tables. (Monthly recurring charges will not be prorated for partial months.) (C) (C) 2.1.2. Each customer is charged individually for each call placed through the carrier since the last monthly billing. The duration of a call, which involves a fractional part of a billing increment will be rounded up to the next full billing increment. 2.1.2. Time Determination - On all calls chargeable time begins when connection is established between the calling station and the called station, PBX or a mobile radio system for Public Land Mobile Service, Coastal Harbor Service, VHF Service, or Air-Ground Service. 2.1.4. Disconnect Time - Chargeable time ends when the calling station hangs up." If the called station "hangs up" but the calling station does not, chargeable time ends when the connection is released either by automatic timing equipment in the telecommunications network or by the Carrier operator. 2.1.5. Customer-Systems - When LDCS is directly connected to a Customer-provided communications system at a Customer's or User's premises, chargeable time for all classes of service begins when an LDCS call terminates in, or passes through, the first Customer Equipment on that Customer-provided communications system. It is the Customer's responsibility to furnish appropriate answer supervision to the point of interface with the LDCS so that chargeable time may begin. 2.1.6. Dedicated Port - Use, if any, of dedicated port(s) on Carriers switching facility by the customer is subject to a one-time installation charge per port, payable in advance of installation, and a monthly use fee per port per month. These charges are in addition to the other charges in this PRICE LIST. The customer is solely responsible for furnishing and paying other common carriers for the customer's private line connecting the dedicated port to the customer's premises.

Mainland - For calls originated and terminated in the Mainland, the rate in effect at the calling station at the time the connection is established applies to the entire call. 2.2.2. All Other Calls - For all other calls, except for calls that use a special access line, when a call is established in one rate period and ends in another rate period, the rate in effect at the calling station for each rate period applies to the portion of the call occurring within that rate period. When a calling station uses a special access line, the rate in effect at the Carrier's Central Office for each rate period applies to the portion of the call occurring within that rate period. When a unit of time is split between two rate periods, the rate applicable to that unit of time is based on the rate period in which it began. 2.2.3 Time Continuum - Chargeable time for a rate period (e.g., 8 AM - 5 PM) begins with the first stated hour (e.g., 8 AM) and continues to, but does not include, the second stated hour (e.g., 5 PM). 2.3. DISTANCE CALCULATION - Usage charges for all mileage sensitive products are based on the airline distance between the serving wire center locations associated with the originating and terminating points of the call. The distance between the originating and terminating points is calculated by using the V and H coordinates of the serving wire centers as defined by BellCore (Bell Communications Research), in the following manner: A. Obtain the "V" and "H" coordinates for the serving wire center of the Customer s switch and the destination point. B. Obtain the difference between the "V" coordinates of each of the Rate Centers. Obtain the difference between the "H" coordinates. C. Square the difference obtained in Step B. D. Add the squares of the "V" difference and "H" difference obtained in Step C. E. Divide the sum of the squares obtained in Step D by ten (10). Round to the next higher whole number if any fraction results from the division. F. Obtain the square root of the whole number obtained in Step E. Round to the next higher whole number if any fraction is obtained. SPECIFIC RATING REGULATIONS - This section contains regulations governing application of the LDCS: in the Mainland; between the Mainland and Alaska, Hawaii, the District of Columbia, Puerto Rico, the US Virgin Islands, Guam, American Samoa, and the Commonwealth of the Northern Mariannas. Rates, regulations and material in this section are for two-point service only. 2.4.1. This plan cannot be used for and does not include long distance Internet access, fax machines, softphones or data devices, transcript services, business (whether for-profit or not-for-profit), telemarketing, auto-dialed calling, multi-party conference calls (including 3-way calls), party lines, chat lines, adult entertainment lines, calls to 900 and 976 numbers, and calls to access voice mail or information services. Calls to numbers identified as contrary to or in violation of the above policy will be billed at $0.10 per minute, plus applicable taxes, surcharges and fees. If the Company determines that usage is not consistent with residential voice service, includes excessive usage or usage predominantly during business hours the Company may immediately restrict use or change the customer s long distance plan to the Windstream 10 Plan. If a customer disconnects the minimum required components for the bundled service plans, the customer s account will be converted to the Windstream 10 Plan. This plan does not include directory assistance, calling cards, collect calls, operator services, international calling or toll-free calling services, and additional charges may apply for these services. The per minute rate for Windstream calling card calls under this plan will be as stated below. Monthly Fee, Unlimited Calling $20.00 Calling Card Calls, Rate Per Minute (No Surcharge) $0.35 This plan also includes the International Discount Plan. The monthly fee applies per account not per line. This plan is limited to a maximum of two residential lines. A $5.00 discount will apply when purchased with Windstream broadband. Taxes, fees and other charges apply. R.6 Residential Offering Option 28 (Windstream 7) The following flat rate is for residential users of outbound interstate LDMTS. This rate is applicable at all times for all interstate calls where technically available. Monthly Fee: $4.95 Rate per minute: $.07 (I) Calling card calls will be billed at $.35 per minute with no surcharge. This plan is only offered in conjunction with a corresponding intrastate plan which includes a per minute rate for intrastate calling. R.7 Residential Offering Option 29 (Windstream Flex 100) The following block-of time plan provides residential customers with blocks of 100 minutes of combined intrastate and interstate direct dialed outbound calling for a flat monthly rate. Usage in excess of the 100 minute block will be billed as an additional block. Unused minutes in a block do not carry over to the following month. Toll-Free calling service, Operator Service and Calling Card calls are not included in the block. Calling Card service is available at an additional charge as specified below. Residential Toll-Free Service Plan 1 This plan is only available to existing customers at existing locations. The following flat rate is for residential users of inbound toll-free service. Per Minute Rate $.20 This plan is available to customers who also subscribe to Windstream Systems of the Midwest Long Distance. Plan 2 The following flat rate and monthly fee is for residential users of inbound toll-free service. This rate is applicable for all times for all calls where this service is available. Calls will be rated at one minute minimum with one minute increments. Rate per Minute: $.15 *Monthly Fee: $2.50 T. Residential Account Code Service Residential Account Code Service is an optional service for direct intrastate and\or interstate long distance calls that offers customers a way to track long distance usage. This Service allows customers the ability to have calls separated and displayed on their telephone bill. Account Code Service requires callers to enter a unique code for completion of a long distance call. The Company s network will authorize calls only if the unique code is entered. This service is offered where technically available. *Monthly fee per account $2.50 *These charges are the same as the Monthly Fees identified in Windstream s state specific (N) tariffs/price lists. Bottom Line New Business Customer Introductory Program This program is offered to introduce the Carrier s service to small and medium sized businesses that are not current customers of the Carrier. This plan and its rates do not apply to any other calling plans which have been subscribed to. Prepaid Calling Card Service The prepaid calling cards are available in various denominations of units as specified by the Company. The prices are inclusive of all taxes. Prepaid cards will be sold at prices rounded to the nearest cent. The customer dials the toll-free number listed on the card and is prompted to enter the personal identification number (PIN) listed on the back of the card. When the card balance nears zero during a call, an automated voice announces the remaining time to the customer. Calls are automatically disconnected after the card balance reaches zero. Per minute charge: Maximum of $0.15 W. Calling Card Surcharge Calling card calls placed from a payphone will be charged a surcharge as set forth in Section 1.1.3. X. Bottom Line Chamber Advantage This program is offered to businesses that are members of a participating Chamber of Commerce. The Chamber of Commerce commits to the program for a term of two years or longer. Each business member of the Chamber of Commerce, that subscribes to one of the following services; 1. Bottom Line WATS 2. Bottom Line 1+ WATS 3. Bottom Line 800 Dedicated Access Line Program 4. Bottom Line 800 Central Office Line Program will receive a 5% discount on their domestic usage in addition to the respective discounts applicable to the specific plan subscribed to. The participating Chamber of Commerce receives a rebate from the Carrier equal to 2% of the aggregate, net-billed domestic usage of all the participating businesses.
