TERM LIFE INSURANCE & AD&D Term life and accidental death and dismemberment insurance from Lincoln helps employees take care of their loved ones by providing benefits in the event of their death or catastrophic injury. Lincoln provides swift, responsive claims service with a goal of a claims decision within five days of receiving complete claims information. A life insurance claim can be started over the phone by the beneficiary, employer or funeral home. Our LINKS claims management process allows automatic transfer of information from a life waiver claim to the start of a death claim; no separate claim form needs to be submitted for term and supplemental life insurance. Full Benefits The Waiver of Premium option helps employees keep their life coverage if they re disabled and unable to work for more than six months in a row. Accelerated Death Benefit enables the insured to withdraw a percentage of their life insurance coverage when diagnosed as terminally ill, reducing the death benefit by the amount withdrawn. Portability lets employees keep their coverage, even if they leave their company. Conversion allows employees to convert all or part of their coverage to an individual life policy if they leave their company, without Evidence of Insurability. SHORT TERM DISABILITY Our short-term disability product includes features to help employees return to work. With a variety of standard and optional benefits, our plans are available with benefit durations of 13 to 26 weeks or longer. Full Benefits Employees can earn up to 99% of pre-disability income through partial-disability employment combined with the short-term disability benefit payments for the duration of the claim. Multiple return-to-work incentives include allowing days of total or partial disability to be used to satisfy the elimination period. Vocational rehabilitation programs include personalized plans, benefits to help employers make changes to the workplace so the employee can return to work, and payment for approved services, such as vocational evaluation, training and job placement. Our standard reporting package provides key claims information 24/7 online, or by email or mail. Flexible options A choice of plan designs with generous maximums Employer- or employee-paid coverage options, including buy-up plans The Family Income Benefit provides an additional cash payment to the insured s eligible survivors, if the insured dies while receiving total disability benefits A simple claims process Many employees use more than one type of disability coverage, and a short-term disability leave may become a long-term disability and may also require a life Waiver of Premium. We proactively identify potential transitions from one coverage to another, and make the process smooth and easy to negotiate. 24-hour claim turnaround time for many common diagnoses, including delivery Seamless transition from short-term to long-term disability claim, if needed, with no paperwork During the claims process, employers and employees have online access to enrollments, claims and certificates, bill payment and claims management.

LONG TERM DISABILITY When disability keeps an employee out of work for an extended period of time, longterm disability insurance helps protect their finances and provide support as they return to the workplace. Employers can choose from a variety of plans and customize coverage with cost-containment features and other options. Full Benefits Employees can earn up to 99% of pre-disability income through partialdisability employment. Total or partial disability can satisfy the elimination period. When appropriate, vocational rehabilitation professionals create personalized programs to help claimants return to work. The Recovery Benefit provides a six-month supplemental benefit for commissioned employees, while they rebuild their commission income to predisability income. (Not available in California.) Our standard reporting package provides key claims information, accessible 24/7 through the web, email or mail. Employers and employees enjoy easy access to enrollments, claims and certificates, bill and claims management by phone, IVR, web, fax, email or mail. Flexible options A choice of plan designs with generous maximums, including the full-featured Premier plan, the flexible Advantage Plan and the cost-conscious Value plan Employer- or employee-paid coverage Optional reimbursement benefits allow employees to: o Pay towards COBRA health insurance o Add to their retirement savings even when a disability prevents them from earning an income o Protect the value of their income replacement from rising prices o Receive long-term disability coverage for a spouse A Conversion Privilege allows employees to purchase individual long-term disability coverage when they leave their job for reasons other than disability or retirement. Joining the program is only a two-step process. 1. Request a proposal To receive an FEI Employee Benefits Program proposal from our partners at Lincoln Financial, please provide the FEI EB Program Account Manager with the following: Group census in excel format including: o Date of Birth o Gender o Salary o Occupation o Current Elections Plan Information - Benefit Summaries or Booklets Current and renewal rates Claims Experience o Mandatory for STD groups over 200 lives o Mandatory for LTD groups over 1,000 lives 2. Join the FEI Employee Benefits Program Your FEI EB Program team will work with you to implement the move to the FEI Program. Once you have agreed to the proposal and upon confirmation of your move to FEI/Lincoln Financial you will receive an Implementation and Next Steps email. The Program Account Manager will be your contact to guide you through the implementation process. We will send you the forms and documents that are required to be completed, they are: FEI Member Agreement (if the group is not already a current member) Application for Group Insurance Administrative & Benefit Form W2 Form (for Disability Lines of Coverage) Final Census Unlike other carriers, Lincoln Financial does not require a binder check so no payment will be needed during implementation.
