An Introduction to VF Benefits Meeting the Diverse Needs of Our Employees At VF, we embrace diversity. That s why VF offers a benefits program tailored to reflect the unique cultures of each of our brands and the lifestyles of our employees. Our benefits program is built around the things that matter to our employees: health care, retirement, well-being, time away from work and more. When our employees are healthy, secure and well, they can bring their best selves to work every day. This brochure provides an overview of the benefits plans offered to VF employees. Hopefully you will see the rewards of becoming part of the One VF team. Benefits Eligibility All employees who work an average of 30 hours or more per week are eligible for benefits under the VF Medical, Dental, Vision, Health Savings Account (HSA), Flexible Spending Accounts (FSAs), Disability Insurance, Life and Accidental Death & Dismemberment (AD&D) Insurance, and other benefit plans. 2

Health Care Benefits At VF, we offer high-quality health care benefits options to ensure that you and your family have the coverage you need. While you pay for a portion of the cost, VF pays for the majority of your health care benefits. Medical Plan Options You have access to two medical plans: the PPO Plan (through BlueCross BlueShield of Alabama or UnitedHealthcare) or the HSA Medical Plan (through BlueCross BlueShield of Alabama). HMOs are also available in California (see page 5). Both plans offer comprehensive medical and prescription drug coverage, including in-network preventive care at no cost to you. You have the option of visiting in-network or out-of-network providers, but you ll save considerably when you visit in-network providers. With the HSA Medical Plan, you ll also have access to a Health Savings Account (HSA) a tax-advantaged health care spending account. VF will contribute $500 or $1,000 to your HSA,* based on your coverage level and when you enroll in the plan; you can think of that as a direct reduction to the deductible for the HSA Medical Plan! If you enroll after January 1, you will receive VF s contribution on July 1 (50% of the total contribution). If you enroll after July 1, you will not receive a VF contribution for that year. Access Care 24/7 with Teladoc If you enroll in a medical plan through BlueCross BlueShield of Alabama or UnitedHealthcare, you ll also receive access to Teladoc. Teladoc provides 24/7 access to a national network of physicians who provide quality health care through convenient telephone or online video consultations. Teladoc physicians can diagnose, treat and write prescriptions for routine medical conditions, such as a sore throat and stuffy nose, allergies, cold and flu, and respiratory infection. 3

Prescription Drug Coverage When you enroll in the PPO or HSA Medical Plan, you also receive prescription drug coverage through Express Scripts. If you enroll in the PPO Plan, based on the type of drug, you will be responsible to pay either 30% or 50% of the cost of your prescription; however, there are minimum and maximum amounts in place (see chart to the right). You are responsible for the cost of prescriptions up to the prescription drug out-of-pocket maximum: $4,100 employee only $5,700 employee + dependents If you enroll in the HSA Medical Plan, you pay 20% of the prescription drug cost, regardless of drug type, after you meet your deductible. Medical and prescription drug expenses have a combined out-ofpocket maximum. Most diabetic medications and many preventive drugs are covered at 100% under the HSA Medical Plan. However, not all brands of preventive medication are covered at 100%. A Dental Maintenance Organization (DMO) is also offered in California (see page 6). Both plans offer comprehensive coverage, give you the freedom to see in- or out-of-network dentists, and cover innetwork preventive care at 100%. A major difference between the two plans is that the Basic Option plan does not cover Type C (major restorative) and Type D (orthodontia) services. Vision Plans VF offers two vision plans: one through VSP and one through UnitedHealthcare. With both plans, you can see any provider you choose, but you will maximize your savings when you go to an in-network provider. VSP network providers are typically private-practice optometrists and ophthalmologists, including affiliate providers Costco and Eye Care Centers of America. UnitedHealthcare s network of providers is a national eye care network that includes private practices and leading retail locations such as Walmart, Costco, Visionworks, etc. 4

Additional Medical and Dental Plan Options If You Live in California VF offers additional medical and dental options to employees living in California, including: Health Maintenance Organizations (HMOs) offered through Kaiser Permanente and Anthem BlueCross, and A Dental Maintenance Organization (DMO) offered through Delta Dental. If you enroll in one of these plans, you will only receive benefits if you use in-network providers (except for emergencies). The DeltaCare USA network consists of private-practice dental facilities that have been carefully screened for quality. Under the DeltaCare USA plan, many services are covered at no cost, while others have a copay for certain benefits. Services covered at no cost include cleanings, topical fluoride, oral exams, X-rays and fillings. Services that require a copay include sealants, preventive resin restoration, space maintainers, crowns and extractions. Orthodontics are also covered. Dental services that are not performed by your selected contract dentist, or are not covered under provisions for emergency care, must be preauthorized by Delta Dental to be covered. 5

Health Care Spending Accounts VF provides access to different types of tax-advantaged spending accounts so you can be smart about the money you spend on care. Health Savings Account (HSA) An HSA is a special tax-exempt savings account that you own and can use to pay for eligible out-of-pocket health care expenses, such as deductibles, coinsurance, and dental and vision expenses, now and in the future. An HSA is only available if you enroll in the HSA Medical Plan. VF contributes to your HSA twice each year (half on January 1 and half on July 1), up to a total of: $500 for employee only coverage $1,000 for all other coverage levels You may also contribute to your HSA through payroll deductions, up to the IRS annual maximum. Health Care Flexible Spending Account (FSA) The Health Care FSA is completely employee-funded; you elect how much you want to contribute via payroll deductions. You can use the Health Care FSA to pay for eligible expenses such as medical deductibles, copays, coinsurance and out-of-pocket expenses. You may not contribute to a Health Care FSA if you enrolled in an HSA. Dependent Care Flexible Spending Account Put aside pre-tax money to pay for eligible dependent day care expenses throughout the year so that you (or your spouse, if you are married) can work, look for work or go to school full-time. Disease Management Coaching Program Disease management coaching is offered through our health care providers, either BlueCross BlueShield of Alabama or UnitedHealthcare, when you enroll in VF medical coverage. A qualified Health Coach will work one-on-one over the phone to help you or your spouse/domestic partner effectively communicate with your doctor, offer advice and strategies for following your treatment plan, and suggest ways you can make lifestyle changes to better manage your condition and avoid complications. Health Coaching is available for: Asthma Chronic Obstructive Pulmonary Disease (COPD) Congestive Heart Failure Coronary Artery Disease Diabetes 6

Be Fit for Life Be Fit for Life is VF s well-being program and reflects our commitment to your health. Be Fit for Life is about more than just health the program is committed to helping you be physically healthy, financially secure, and engaged with your community, as well as happy, confident and productive. Be Fit for Life helps you work toward an overall sense that you re thriving in life. Be Fit for Life includes our incentive-based wellness program that helps you understand your health and rewards you for taking healthy actions and achieving healthy results. The program utilizes an interactive, personalized platform that helps each eligible employee and their spouse/domestic partner develop their own journey toward health. On this path, you move through three levels with the opportunity to earn points and rewards (including medical plan premium discounts and Visa gift cards) at each step. Eligible employees not enrolled in VF medical coverage can participate in the program, but are not eligible to earn rewards. Be Fit for Life also provides you and your family with the resources, tools and motivation to make living well easier. Services available include health coaching, biometric screenings, tobacco cessation and an employee assistance program. Diabetes Management Program The Livongo for Diabetes Program is offered at no cost to eligible employees and spouses/domestic partners enrolled in a VF medical plan offered through BlueCross BlueShield of Alabama or UnitedHealthcare. Participants receive: A Livongo connected glucometer that uses cellular technology to automatically upload blood glucose readings that can be accessed online. Personalized support provided from the Livongo mobile app and secure website to help manage diabetes better. Certified diabetes educators are also available by phone to provide advice and answer nutrition and lifestyle questions. Unlimited free test strips and lancets shipped at no cost. 7

Financial Protection Well-being at VF is more than just being physically fit; it s about managing your finances in a way that reduces stress. We offer income protection benefits that ensure your family is financially stable in the event you become ill, are injured or die. Income Protection VF offers you flexibility and choice when it comes to safeguarding finances with Life Insurance, Accidental Death & Dismemberment (AD&D) Insurance and Disability Insurance. Life Insurance VF pays the cost of basic life insurance. You are covered at one times your eligible earnings, with a minimum benefit of $50,000 and a maximum benefit of $1,000,000. You may purchase optional life insurance up to seven times your eligible earnings (rounded to the next higher $1,000), with a maximum benefit of $1,500,000. You may also purchase spouse and/or child life insurance. Dependent Coverage For: Spouse/domestic partner $5,000, $10,000, $20,000, $30,000, $40,000, $50,000, $75,000 or $100,000 Child(ren) $2,500, $5,000 or $10,000 Additional Protection You Can Buy Whole life insurance builds cash value that you can use throughout the life of the policy and then provides a benefit to your beneficiary if you pass away. You may purchase coverage for yourself, your spouse and your children. Group accident insurance helps you pay outof-pocket expenses and extra bills that can follow an accidental injury, whether minor or catastrophic. Examples of accidental injury include broken bone, concussion, lacerations, burns, etc. Accidental Death & Dismemberment AD&D insurance can help you pay expenses if you or your spouse/ domestic partner is seriously injured or dies in a covered accident. You may select from $10,000 to $500,000 of coverage in $10,000 increments. There are three coverage tiers: employee only, employee + children, and employee + family. If you elect to cover others, the maximum benefit available is $250,000 for your spouse/domestic partner and $25,000 for each child. Disability Insurance Short-Term Disability (STD): VF pays for STD to cover an eligible injury or illness, and provides you with 100% of your pay for the first six weeks of disability and 60% of your pay for the next 20 weeks. Benefits begin on the first day you are injured or hospitalized, or the eighth day of an illness. Long-Term Disability (LTD): You may purchase LTD coverage that will replace 60% of covered earnings, to a maximum of $12,000 per month, if you have an eligible illness or injury that causes you to be disabled for more than 180 days. 8

Retirement Benefits Invest in your retirement and yourself with help from the VF 401k Savings Plan! After three months of service, you are eligible to participate in VF s competitive 401k plan administered by Fidelity. The plan offers a convenient, pre-tax way to save for retirement. You are always 100% vested, which means you own all of the money in your 401k account. Your savings can grow in several ways: Your pre-tax contributions You may contribute from 1% to 50% of your pay on a pre-tax basis through automatic payroll deductions, up to the annual IRS dollar limit. The amount you contribute is automatically deducted before federal and state taxes are withheld. This lowers your taxable income, so you pay less in taxes now. Plus, your contributions and their earnings are not taxed until you withdraw the money which means your savings can grow more quickly than they would in a regular bank account. Company matching contributions VF helps your contributions grow through a generous match. VF will match 100% of the first 6% you contribute to your 401k per pay period. In other words, for every dollar you contribute (up to 6% of pay), VF will contribute another dollar to your account. It s like getting free money. Earnings from your investments You choose how to invest your contributions and Company matching contributions. The plan offers a range of investment options so you can choose funds that best suit you. You can choose from: Core investment funds Lifestage funds, or Fidelity BrokerageLink (a self-directed brokerage account). VF also offers you independent, objective investment advice and management from Financial Engines Advisors LLC, a registered investment advisor. Through this advisor, you ll have access to an online advice service and a professional management program to assist you in managing your funds. Well-being at VF is more than just being physically fit. It s working toward an overall sense that you re thriving in life which includes saving and planning for a financially secure retirement. The VF 401k Savings Plan, along with the tools and resources VF makes available to help you manage your account, can help you do just that! 9

Vacation You earn paid vacation annually based on your length of service with VF. VF Smart Shopping Access an online marketplace that provides discounts at brand-name retailers and local merchants. Employee Assistance Program (EAP) The EAP provides resources to help resolve issues that may be affecting your personal or professional life. You and your dependents are eligible for free telephone or in-person counseling sessions. Work/life benefits also give you free access to useful information, resources and customized referrals for child care, adoption and education resources. Holidays VF observes core holidays that vary by location. VF Product Discounts Group Legal Access a network of attorneys to help with common situations requiring legal counsel, including: Will and estate planning Document preparation Buying and selling your primary residence Traffic issues Bankruptcies Debt collection Personal property issues Family law, including divorce Critical Illness Insurance Medical plans don t cover all the costs associated with a critical illness. You can purchase coverage for yourself, your spouse or your children to help fill the gaps and cover costs related to recovery from a major illness such as a heart attack, stroke, cancer, etc. Out-of-Country Medical Coverage Get reimbursed for eligible medical expenses not covered by a VF medical plan if you become ill or injured while traveling abroad on VF business.
