Now is the time to understand the benefits available to you, review your current benefit elections and enroll for coverage. You must take action between 12:01 a.m. on Monday, April 24 and 11:59 p.m. on Sunday, May 7, to confirm your benefit elections. Highlights One Medical Plan for all of UPHS All CCH and UPHS employees will have the option to enroll for coverage under one medical plan, the PennCare PPO Plan with prescription drug coverage through CVS/Caremark. Elimination of the Spousal Surcharge The spousal surcharge will be eliminated, and you will not need to complete a Spousal Surcharge Waiver during Open Enrollment. HR&You Benefits Portal: www.uphshrandyou.com Beginning April 24, you will have access to the UPHS HR&You benefits website where you can find all of your benefits-related information, tools, forms and contacts. You Must Take Action! Enroll for Supplemental Life and Accidental Death & Dismemberment (AD&D) Coverage Employee supplemental life and AD&D coverage options will be changing on June 30, 2017. If you want coverage for yourself, your spouse and/or your child(ren), you must take action during Open Enrollment.

Our benefit programs help you and your family stay well and protect you when you re not well while managing the rising costs of health care for us all. In this guide, you will find the information you need to learn about the benefit offerings available to you effective July 1, 2017. We are also excited to introduce you to the HR&You benefits website at www.uphshrandyou.com. We hope you will find it to be a helpful resource for accessing your benefit information throughout the year. Contents 1

What s Changing These changes will go into effect on July 1, 2017. Medical The Keystone HMO Plan will no longer be offered. Medical coverage is available through the PennCare PPO Plan administered by Independence Blue Cross (IBC). If you are currently enrolled in the Keystone HMO medical plan and take no action during Open Enrollment, you will default to the PennCare PPO Plan at the same level of coverage (for example, employee only or family). You will receive a new PennCare PPO ID card in the mail. If you are currently enrolled in the Keystone HMO Plan and enroll in the PennCare PPO Plan, your out-of-pocket maximum (the most you pay out-of-pocket for medical services and prescription drugs in a plan year) will start over on July 1, 2017. The new out-of-pocket maximums for medical services and prescription drugs will be in effect for the July 1, 2017 June 30, 2018 plan year. Cost of Coverage If you are currently paying the spousal surcharge, you will no longer be subject to the $100 per-pay-period spousal surcharge. There will be three salary bands for medical plan coverage instead of the previous four. The cost of dental coverage is less than last year s rates for all three dental plans and across all four coverage tiers. There is no change in the cost of vision coverage from last year s rates. Prescription Drug Prescription drug coverage will be provided through the CVS/Caremark network. Your out-of-pocket costs (copays) for prescription drugs will change. There will be no out-of-pocket costs for specialty drugs, generic hypertension and generic hyperlipidemia prescriptions when filled at a UPHS pharmacy. If you enroll in medical coverage, new CVS/Caremark prescription drug ID cards will be mailed to you after enrollment. Core Life Insurance Core life insurance, paid by UPHS, will be one times your annual base pay. This is an increase in coverage for all part-time, benefit-eligible employees. All full-time and part-time benefit-eligible employees will automatically receive this coverage. Supplemental Life Insurance Current supplemental life insurance coverage levels will be changing. You may purchase supplemental life insurance coverage of one, two or three times your base pay during Open Enrollment. If you purchase coverage during Open Enrollment, you will not be required to provide evidence of insurability (proof of good health), unless your coverage amount exceeds $500,000. Accidental Death and Dismemberment (AD&D) Core AD&D coverage will no longer be offered, and your current coverage will end on June 30, 2017. If you want AD&D coverage for yourself, your spouse, and/or your child(ren), you must elect coverage during Open Enrollment. You may purchase AD&D supplemental coverage equal to one, two, three or four times your base pay. Spouse AD&D coverage is offered at 50% of your purchased coverage amount. Child(ren) AD&D coverage is offered at 15% of your purchased coverage amount. 2

Open Enrollment Is April 24 May 7, 2017 Take action during Open Enrollment if you want to: Make changes to your current benefit elections for the 2017 2018 plan year. Add or drop a dependent from your benefits coverage. Contribute to a Health Care or Dependent Care Flexible Spending Account (FSA) for the 2017 2018 plan year. Enroll in supplemental life and/or AD&D coverage for you, your spouse and/or your child(ren). Designate a beneficiary for core life insurance. What Happens If You Don't Take Any Action? If you are enrolled in the Keystone HMO Plan, you will default to the PennCare PPO Plan at your same coverage level. You may enroll for AD&D coverage during Open Enrollment, or you must wait until a future Open Enrollment period. Benefits You Receive Automatically For full-time employees, short- and long-term disability coverage is automatically provided to you and paid by UPHS. Part-time employees, regularly scheduled a minimum of 40 hours bi-weekly, may purchase disability coverage. Visit www.uphshrandyou.com during Open Enrollment for information. Convenient Care for UPHS Employees When you need to feel better or want to take action to improve your health, UPHS provides FirstCall and PennPrevents to make that easy. You take care of patients. Let us take care of you. Not feeling well? FirstCall is a 24/7 virtual urgent care line that connects you with Penn Medicine providers 24 hours a day, seven days a week, and will be available beginning July 1, 2017. Our goal is to get you the answers and care that you need right away. Whether you need medical advice, prescriptions for acute issues or a same-day in-person appointment, make it your FirstCall! Beginning July 1, 2017, you can reach FirstCall by calling 215-615-2222. Empowering UPHS employees to take control of their health. PennPrevents offers convenient and effective treatment for hypertension and high cholesterol. In support of this program, UPHS is pleased to offer no cost generic hypertension and hyperlipidemia prescriptions when filled at a UPHS pharmacy. Visit the Perelman Center for Advanced Medicine (PCAM) and Pennsylvania Hospital to: Take control of high blood pressure. Lower your cholesterol and reduce your risk of heart disease. Screenings and care programs are provided in the workplace and are available to those covered by the PennCare PPO Plan. Call 215-615-5252 to learn more. Health Advocate The world of health care can be complex, which is why we offer support through Health Advocate. Health Advocate is a free, confidential service available to you, your spouse, dependent children, parents and parents-in-law. Their experts can: Help you get the right care at the right time, Resolve a wide range of time-consuming health care and insurance-related issues, Help you use and manage your health care benefits, and Much more. You also have the opportunity to participate in their free tobacco cessation program. Your Health Advocate benefit can be accessed 24/7 at 866-695-8622 or at www.healthadvocate.com/uphs. Normal business hours are Monday Friday, from 8:00 a.m. to 12:00 a.m. (midnight), Eastern Time. Staff is available for assistance after hours and on weekends. You can also access Health Advocate from the HR&You website. Penn Care Connect Concierge Service Penn Care Connect is a concierge service that can help you or a family member schedule an appointment with a Penn provider. Call Penn Care Connect at 267-414-2208 to schedule an appointment with a PennCare Network physician. 4

Medical The PennCare PPO Plan The Keystone HMO Plan will no longer be offered for the 2017 2018 benefit plan year. If you are enrolled in the Keystone HMO Plan, you will default to the PennCare PPO Plan at your same coverage level. The PennCare PPO Plan offers three networks of providers with different levels of coverage. You receive the highest level of benefits when you visit a PennCare Network provider, facility or pharmacy. The plan also includes prescription drug coverage through CVS/Caremark. PennCare PPO Network Provides access to best-in-class providers in many convenient locations even outside of the Philadelphia metropolitan area. When you use this network, you save on everything from doctor office visits and prescription drugs to X-rays and hospital care. IBC In-Network If you choose a provider that s not in the PennCare Network, you should consider using an in-network provider. You have to pay more out of pocket than you would if you used a PennCare Network provider, since you will have to meet a deductible for some services before the plan begins to pay for coverage. Out-of-Network This includes any providers outside the PennCare and IBC networks. You will pay more when you visit an out-of-network provider. Simply register/sign in to an account and select Find a Doctor. For provider information, visit www.ibx.com. Centers of Excellence Certain services require you to receive care from a PennCare provider under the Centers of Excellence. The benefit plan offers best-in-class services provided by the following UPHS departments: Oncology Cardiology Neuroscience Orthopaedics If you do not receive the service from a provider in the Centers of Excellence of UPHS, you will pay a $1,000 copay in addition to the regular cost share of the service (does not apply to emergency services). If you or a covered dependent lives more than 50 miles from Philadelphia, you will be eligible for consideration of waiver of the $1,000 copay if you receive the service from a provider outside of the Centers of Excellence of UPHS. Prescription Drug New for the 2017 2018 benefit plan year, prescription drug benefits will be administered through CVS/Caremark effective July 1, 2017. When you enroll in the medical plan, you will automatically receive coverage in the prescription drug program. You will have the option to fill your prescriptions at a UPHS pharmacy or through a retail pharmacy in the CVS/Caremark network. Visit www.caremark.com to find a retail location near you. Under the prescription drug program, any out-of-pocket costs, including copays, will count toward the prescription drug out-of-pocket maximum. Dental UPHS helps you maintain good dental health by providing you with three dental plan options: Delta Dental Standard, Delta Dental Premium and Penn Faculty Practice Plan.* For the 2017 2018 plan year, the cost for coverage will be reduced for all three dental plans. If you elect coverage under this plan, you must receive care at these three locations to receive plan benefits. Make sure these locations are convenient for you and your family. Save Money by Knowing Your Dental Provider s Network You can use any dentist you choose. However, the amount you pay for dental services will depend on your plan option and whether your provider participates in the Delta Dental PPO network, the Delta Dental Premium network or is a nonparticipating provider. The highest benefit level will be paid when you visit a PPO network provider. These providers offer the highest discount, so your cost is lowest when you receive services from them. Vision UPHS offers two vision plans that allow you to select the coverage level and cost that best meet your needs. You may choose either the Davis Standard Plan or the Davis Premium Plan. There are no changes to vision plan benefits for the 2017 2018 benefit plan year. 8

Life and Accidental Death & Dismemberment (AD&D) Life and AD&D benefits, offered through Sun Life, provide you and your family with an added layer of financial protection. For the 2017 2018 benefit plan year, you will have new coverage options. You must take action and enroll in coverage during the Open Enrollment period to have coverage beginning July 1, 2017. Imputed Income Under IRS rules, you must pay taxes on the premium value of the UPHS-paid core coverage over $50,000. This is called imputed income and will be shown on your bi-weekly pay stub. The IRS determines the amount of this tax. Core Life Insurance Full-time and part-time benefit-eligible employees will automatically be provided with core life insurance coverage, paid by UPHS. New for the 2017 2018 benefit plan year, coverage will be one times your annual base pay. This is an increase in coverage for all benefits-eligible part-time employees and many full-time employees. Supplemental Life Insurance During Open Enrollment, you may purchase supplemental life insurance of one, two or three times your base pay. You may purchase up to $500,000 in coverage without providing proof of good health, also known as evidence of insurability. The maximum amount of coverage you can elect is $1,000,000 when combined with core life insurance. Your benefits under the core and supplemental life insurance plans will reduce beginning when you turn age 70 (40% of the total benefit), then age 75 (25% of the total benefit), and then age 80 (15% of the total benefit). Spouse Life Insurance Spouse life insurance provides benefits to you, as the beneficiary, in the event of the death of your spouse. If your spouse works for UPHS, only one of you can elect spouse life insurance. During Open Enrollment, you may purchase coverage of $2,000, $5,000 or $10,000. Child Life Insurance Child life insurance provides benefits to you, as the beneficiary, so you can pay expenses in the event of the death of your child(ren). If your spouse works for UPHS, only one of you can elect child life insurance. During Open Enrollment, you may purchase coverage of $1,000, $2,000 or $5,000. 9

Evidence of Insurability If you enroll in supplemental life insurance coverage during Open Enrollment, you will automatically qualify for coverage, unless your coverage is greater than $500,000. In this case, you may be required to provide proof of your good health also known as evidence of insurability (EOI) to Sun Life. AD&D Core AD&D coverage will end on June 30, 2017. You may elect to purchase AD&D coverage during Open Enrollment for yourself, your spouse and your child(ren). AD&D coverage pays benefits in addition to any life insurance benefits you receive if you die or are dismembered in an accident. You are automatically the beneficiary for any dependent AD&D coverage you purchase. If your spouse works for UPHS, only one of you can elect dependent AD&D insurance. If you choose to enroll for AD&D coverage, the cost of this coverage will be deducted from your paycheck on a pre-tax basis. Your options for AD&D coverage are: Type Employee AD&D Family AD&D Coverage 1, 2, 3 or 4 times your annual base pay 50% of the purchased employee coverage for spouse and 15% of purchased employee coverage for child Additional Benefits Disability Disability benefits can replace a portion of your base pay if you are unable to work because of illness or injury. Short-term and long-term disability benefits are fully paid by UPHS. Part-time employees, scheduled to work a minimum of 40 hours bi-weekly, may purchase disability coverage on a voluntary basis. Flexible Spending Accounts (FSAs) FSAs allow you to contribute money on a pre-tax basis to help pay for eligible health care and/or dependent care expenses. For the 2017 2018 plan year, you can contribute from $60 to $2,600 in pre-tax earnings to the Health Care FSA and from $260 to $5,000* in pre-tax earnings to the Dependent Care FSA. For the Health Care FSA, any unused funds remaining after September 15, 2018, will be forfeited. You can submit expenses until December 31 after the end of the plan year. * Employees making $115,000 or more will be able to contribute a maximum of only $1,700 this plan year to a Dependent Care FSA to meet IRS regulations. 10

Employee Assistance Program (EAP) The EAP is provided by Penn Behavioral Health. All UPHS employees are eligible for the EAP, which is 100% paid by UPHS. The EAP, available to you 24 hours a day, seven days a week, offers confidential assistance to all employees and their dependents. How to Enroll To enroll for benefits, visit the EnrollOne website at www.enrollone.com/uphs, starting April 24. Log on to our secure site using your network username and password the same information you use when logging on to your work computer. Once you are logged on to EnrollOne, you can: Enroll for benefits. During the Open Enrollment window, April 24 May 7, 2017, you may enroll for benefits 24 hours a day, seven days a week. Your elections will be in effect for July 1, 2017, through June 30, 2018. Print your online confirmation statement. If you want a copy of your elections, you must print your confirmation page after you complete your enrollment. View your benefits profile page. Your profile will show your benefit elections for the 2017 2018 plan year. Designate Your Beneficiaries During Open Enrollment, please review your beneficiary information for your life insurance benefits and make any necessary changes. Beneficiaries are those person(s) who receive your benefits if you die. If you elect life insurance for your spouse, make sure his/her information is also on file. For the UPHS 403(b) Plan If you are married, your spouse is automatically your beneficiary. To choose a beneficiary other than your spouse, you must obtain your spouse s consent in writing. Are You Saving Enough for Retirement? UPHS offers retirement savings plans for all employees. You can contribute on a pre-tax or after-tax (Roth) basis.
