OTHER IMPORTANT BENEFITS For Full-time Hourly and Salaried Associates Short-Term Disability (STD) & Long-Term Disability (LTD) Receive additional financial protection if you become disabled and are unable to work because of illness or injury. IMPORTANT: Full-time hourly associates are automatically enrolled into coverage. Full-time hourly associates will have payroll deductions begin on your 91st day of employment unless you decline coverage. If you decline, Evidence of Insurability will be required for future enrollments. Salaried associates are provided Company-paid STD & LTD coverage. Salaried associates are defaulted into the LTD Tax on Plan Cost option. This means that the cost of the LTD coverage that the Company pays for you will be charged to you as income and you will pay income taxes on the LTD cost. By doing this, if you were approved for LTD, then your benefit would be a tax-free benefit. You can change this option during your enrollment session. Life Insurance and Accidental Death and Dismemberment (AD&D) You can enroll in life insurance and AD&D coverage at any time by calling the Benefits Choice Center but not more than once during a 12-month period or if you experience a family status change. Company-Paid Basic Term Life Insurance The Company provides you and your dependents with basic life insurance if you and your dependents participate in a Company medical plan. Associate and Dependent Voluntary Term Life Insurance You ll have an opportunity to purchase additional life insurance coverage, in multiples of your pay, that best meets your needs. As a new associate, you have a one-time opportunity to enroll in Voluntary Life Insurance coverage of up to three times your pay without having to provide an Evidence of Insurability form*. Company-Paid Basic AD&D The Company provides all full-time hourly and salaried associates with basic AD&D insurance. Voluntary AD&D You ll have an opportunity to purchase additional AD&D coverage for yourself or yourself and your family, in multiples of your pay. Evidence of Insurability is not required for AD&D coverage. * An Evidence of Insurability form is required if you elect a coverage amount above $500,000. 4

For Part-time Hourly Associates Short-Term Disability (STD) IMPORTANT: Part-time hourly associates are automatically enrolled into coverage. When you are covered under the short-term disability plan, you ll continue to receive part of your income (50% of your weekly base pay up to $125 per week) if a qualified disability due to illness or injury leaves you unable to work. Benefits begin after you have been unable to work for 14 consecutive calendar days and continue for an approved period of disability for up to 26 weeks (your 14-day elimination period plus your 24 weeks of coverage). Term Life Insurance for You and Your Dependents IMPORTANT: Part-time hourly associates are automatically enrolled in Term Life Insurance for yourself. If you don t want this coverage, you must decline it before your 91st day of employment or payroll deductions will begin. Term Life Insurance provides a $20,000 benefit for you, the associate.
