If yu are already signed in t the Stryker netwrk, yu will autmatically be lgged in t the Benefits Enrllment Site. 2. Review yur infrmatin If any f yur persnal infrmatin is utdated r inaccurate, visit My Stryker Inf t update it nce yur enrllment is cmpleted. Nte: Yu must be lgged int Stryker s netwrk in rder t access My Stryker Inf and t update yur persnal infrmatin. Once updated, it will take a few days t reflect in the Benefits Enrllment Site. Read the Legal Disclaimer and if yu agree t the terms and cnditins, click Accept. If all f yur infrmatin is crrect, click Save and Cntinue t prceed t the next step. 3. Enrll in yur benefits Enrllment steps The system will give yu benefit ptins based n yur eligibility. Select the benefits that best fit yur needs and thse f yur family. Once yu re ready t enrll, frm the Enrll page, either click View/Change next t each plan if yu wish t make individual plan changes, r click Take Me Thrugh Each Benefit if yu wish t be guided thrugh each eligible benefit ffered fr 2018. If yu re nt sure which medical plan is the right chice, stp and estimate yur 2018 healthcare csts using the Medical Expense Estimatr by clicking the link t the right n the medical enrllment screen. If yu wish t add a dependent click Add Dependent and fill ut the required infrmatin. After yu are finished, click Cntinue t Step 2. If yu d nt want t enrll in medical insurance, click Decline Cverage t waive this benefit. Step 3: Review the medical plan ptins and select Chse This Optin alngside the medical plan yu want t elect. If yu wish t waive medical cverage, select Chse N Cverage t waive this benefit. Once yu have made yur selectin, click Save and Cntinue. If yu skip this step yu will autmatically be defaulted as a tbacc user and assessed the Tbacc Surcharge. Health Savings Accunt (HSA) Yu must elect the UHC Premium HSA Plan r the UHC Basic HSA Plan t be eligible fr the HSA. If yu want t make a cntributin fr 2018, enter yur HSA annual cntributin amunt. Read the Terms and Cnditins and if yu agree, check the accept bx. Then, click Save and Cntinue. Healthcare Flexible Spending Accunt (HCFSA): Yu must elect a traditinal PPO r HMO plan t be eligible fr the HCFSA. If yu want t make a cntributin fr 2018, enter yur HCFSA annual cntributin amunt then, click Save and Cntinue. Dependent Care Flexible Spending Accunt (DCFSA) Yu can elect a DCFSA regardless f the medical plan yu chse during enrllment. If yu want t make a cntributin fr 2018, enter yur DCFSA annual cntributin amunt. Then, click Save and Cntinue. Dental: Step 1: Click the check bxes next t yurself and each dependent that yu wish t cver. If yu wish t add a new dependent, click Add Dependent and fill ut the required infrmatin. After yu are finished click, Cntinue t Step 2. If yu d nt want t enrll in dental insurance, click Decline Cverage t waive this benefit. Please be sure t verify yur beneficiary infrmatin is crrect befre prceeding. Click the Manage Beneficiaries link t add r update yur beneficiaries. Nte that the beneficiaries yu assign t this benefit will als apply t yur Basic Life and Accidental Death and Dismemberment (AD&D) cverage as well as yur ptinal Supplemental Life Benefit cverage. Once yu have made yur selectin, click Save and Cntinue. Supplemental Life Make yur ptinal Supplemental Life insurance electin by clicking n the Chse This Optin buttn next t yur chice, r click the Chse N Cverage buttn t waive the cverage. Yur electin f Supplemental Life Insurance may require Evidence f Insurability (EOI). If EOI is required, yu will receive infrmatin in the mail frm The Hartfrd regarding hw t cmplete the EOI prcess. Nte: Until The Hartfrd receives yur EOI and makes the final decisin abut yur cverage, yu will be cvered at the maximum level available t yu withut prviding EOI. After making yur electin, click Save and Cntinue. Spuse/declared dmestic partner Life Insurance Yu als have the ptin t elect Life Insurance fr yur spuse/declared dmestic partner. Cverage purchased fr yur spuse/declared dmestic partner will pay a benefit t yu if yur spuse/declared dmestic partner shuld die. Yu will autmatically be designated the beneficiary fr these plicies. If yur spuse/declared dmestic partner is nt listed, click Add Dependent and fill ut the required infrmatin, then click Save. T elect cverage, click Chse This Optin next t yur selectin, r Chse N Cverage t waive. When yu have cmpleted making yur selectins, click Save and Cntinue. Child(ren) Life Insurance Yu als have the ptin t elect Life Insurance fr yur child(ren). Cverage purchased fr yur child(ren) will pay a benefit t yu if yur child shuld die.
