The interested parties may collect the empanelment formats from Bank Maharashtra, Zonal Office, General Administration Department (GAD), Kolhapur or download from www.bankmaharashtra.in. 2. Cost empanelment Document/Form: Rs. 1000/- (Rs. One Thousand only-nonrefundable) to be paid in the form Demand Draft /PO favouring Bank Maharashtra payable at Kolhapur. 3. Sector/IDRBT, in appropriate class. Pressionals/Contractors & Service providers shall apply for empanelment in Prescribed format. Application forms are available at above-mentioned address i.e. Bank Maharashtra Kolhapur Zonal Office,Mahabank Building, 517/A/1 Plot 7, Kawala Naka, Tararani Chowk, Kolhapur-416001 between 11.00 am to 3.00 p.m. on all ing days except Sundays & holidays (from 04.07.2013 to 20.07.2013.) INSTRUCTIONS TO APPLICANTS: 1) Interested pressional/ contractors/vendors are requested to furnish details about their firm, technical experience, competence and evidence their financial standing as per enclosed format for considering their names for empanelment. 2) The pressional/ contractors/vendors should have minimum five years experience in the respective field. 3) The pressional/ contractors/vendors applying for electrical s should have valid electrical licenses. 4) The pressional/ contractors/vendors who intend to apply for more than one category have to apply for each category separately using appropriate application form by paying requite fee which is non refundable. Intending pressional/contractors/vendors are requested to read the format carefully before filling the particulars. Incomplete applications will not be considered. 5) Bank Maharashtra shall approach the pressional/ contractors/vendor s clients, corporations, organizations, etc. to verify their general reputation / competence. Selection contractors will be based on the ability and competence required for good quality jobs to be performed by them. 6) The empanelment shall be made for a specific category. However, pressional /contractors/vendors empanelled in higher category shall be eligible for the s in the lower category. 7) If the space provided in the format is insufficient for giving full details, the same may be given on a separate sheet paper. 8) Information/details furnished by selected pressional/ contractors/vendors, if found to be false at any time in future or any information affecting empanelment is willingly /unwillingly withheld & if it comes to the notice the Bank at any point time, the empanelment pressional/ contractors/vendors can be cancelled immediately. 9) Where copies documents are required to be furnished, these should be certified
true copies. 10) Cost the application form is Rs.1000.00 which is not refundable & to be paid by way Pay Order/Demand Draft favoring Bank Maharashtra payable at Kolhapur. Application form is not transferable. 11) The evaluation will be based on the experience the pressional/ Contractors/ vendors and their financial capabilities and may also include inspecting the s furnished by them for which, necessary co-ordination shall be made by them. Based on the details furnished and inspection s, eligibility criteria, pressional/ contractors/vendors will be empanelled. Decision the Bank regarding selection rejection for empanelment will be final and binding and no further correspondence will be entertained. 12) The empanelled contractors will only be informed by post. 13) Decision Bank Maharashtra regarding selection pressional/contractors /vendors for empanelment will be final and binding and no further correspondence will be entertained. 14) Pressional/ contractors /vendors empanelled will be informed separately. Empanelment the firm, if selected, will be valid for one year and subject to review, bank may decide to extend it for another 2 years or as decided by the bank. 15) Bank Maharashtra reserves the right to accept or reject any application without assigning any reason. 16) The application shall be submitted strictly in the format as mentioned along with the supporting documents. Information furnished to Bank will be kept as strictly confidential. 17) The application shall be signed by the person/s on behalf the organization having necessary Authority/ Power Attorney to do so. Each page the application shall be signed as token acceptance the contents mentioned therein and copy Power Attorney / Memorandum association (Wherever applicable) shall be furnished along with application. 18) Completed empanelment application/ document shall be submitted up to 3.00 pm on or before 20.07.2013 to the Zonal Manager, Bank Maharashtra Kolhapur Zonal Office,Mahabank Building, 517/A/1 Plot 7, Kawala Naka, Tararani Chowk, Kolhapur-416001. The application shall not be accepted by post / courier. Each application/form must accompany Pay Order / Demand Draft favoring Bank Maharashtra payable at Kolhapur for Rs.1000/-. 19) It is necessary that pressional/ contractors/vendors must have full fice set up at Kolhapur or a full-fledged branch fice at Kolhapur if its head/registered fice is at somewhere else. Preference will be given to Kolhapur based pressional/ contractors/vendors who are on the panel Nationalized/Public Sector Banks in Kolhapur.
