Your Quick-Start Guide for 2012 Annual Benefits Enrollment Simple Steps, Easy Enrollment Take action by October 25, 2011! Powering you to live well.

What s New for 2012 Three Medical Options Healthcare 80 This option is similar to the Standard 80/20 option available today. Two Consumer Health options Consumer Health 90 Consumer Health 80 You may open a Health Savings Account (HSA) through Fidelity if you enroll in one of the Consumer Health options. The HSA is a triple-tax-advantaged savings account you can use to pay eligible out-of-pocket health care expenses now or in the future. Prescription Drug Coverage Coinsurance and minimum/maximum amounts will change Certain preventive drugs will be free Pay the Difference Program This is a new rule that will apply if you or your doctor requests brand medication when a generic equivalent is available. You will be responsible for the difference in cost between the brand and the generic medications, in addition to your applicable coinsurance. Optional Life and Accidental Death and Dismemberment (AD&D) Insurance Employee contributions will be changing for: Employee life for non-tobacco users Spouse life and AD&D Child life and AD&D Claims Administration On January 1, 2012, Empire BCBS is changing its name to Anthem BCBS. You will: Be issued a new ID number Receive new Anthem ID cards in late December, to use beginning January 1, 2012. (Then destroy your old Empire BCBS cards) Health Care and Dependent Care Reimbursement Account (HCRA and DCRA) Plans Acclaris will replace Ceridian as the health plan partner Dental/Vision Reimbursement Account Will be available to reimburse dental or vision expenses not paid by dental or vision plans for those in a Consumer Health option What s Not Changing Dental or Vision Coverage The monthly contributions and plan provisions stay the same.

STEP 1 1 2 3 Review Your Medical Plan Options Healthcare 80 This is a traditional option that offers predictability with set copays for some services, a lower deductible and the opportunity to participate in a Health Care Reimbursement Account (HCRA). But your monthly contributions will be higher and the money in your HCRA doesn t roll over from year to year. This option is similar to the current Standard 80/20 option. Consumer Health 90 Consumer Health 80 The Consumer Health options give you more control over your health care dollars and allow you to open a Health Savings Account (HSA). There are no copays, and your monthly contributions are lower. The annual deductible is higher, but there is true out-of-pocket maximum protection, and Eaton s contribution to your HSA helps offset the higher deductible. You can use the money in your HSA toward your deductible or to pay for eligible health-care expenses. With an HSA, there is no use it or lose it provision. Your account balance rolls over from year to year, and it can be invested in a variety of options. You keep this money if you retire or leave Eaton. Consumer Health 90 and Consumer Health 80 replace the current High 90/20 option and the Low 80/20 option. Both are part of this enrollment kit.

STEP 2 Decide on the Coverage You ll Need Know who is eligible. For all three of Eaton s medical plan options, you can choose to enroll eligible family members, including your spouse or domestic partner and children up to age 26. Get only what you need. You can save money by buying only what you will need or use. So before you choose coverage for you and your family members, it s a good idea to consider how you ve used health care in the past and how you expect to use it in the future. Keep reading to find out how you can get answers to questions about your coverage. STEP 3 Use Your Tools Your 2012 Enrollment Kit Take a look at the 2012 Decision Guide to explore your medical plan options in more detail. EatonBenefits.com Review the 2012 Medical Plan Option Choices Library under Eaton Learn More Links. You ll find helpful information, copies of prior communications and answers to Frequently Asked Questions (FAQs) that have been collected from Eaton employees across the United States. Check out Choose My Best Plan Option (Plan Finder) to help choose the best option for you. During the enrollment period, you can access this tool while making your elections online at Fidelity NetBenefits. After you click on Review/Update Your Election, the link to the tool is at the top of the page where you choose your medical option. Enter your estimated health care usage and this online tool will help you estimate what your cost would be for each of Eaton s medical plan options for 2012. Empire and Medco statements These statements (which you will receive shortly if they have not yet arrived) contain information about your medical and prescription drug usage, which will assist you with using the Choose My Best Plan Option (Plan Finder) tool.

Take Action 1 2 3 STEP 4 Go to EatonBenefits.com and select the Eaton Service Center at Fidelity. Sign in with your user name and password. Go to the Health & Insurance tab. Click Get Started. Click Enroll Now, then Review/Update Your Election. Then, make your elections. Helpful Hints Click on the links for more information and follow the prompts when choosing your medical plan option. If you elect life insurance coverage, you may be prompted to submit evidence of insurability. After you make your choices, take a few minutes to review and, if needed, update your beneficiary designations by clicking Beneficiaries under Your Profile tab. Prefer to Enroll By Phone? You also may enroll or get more information by phone. Call 866-EATON01 (866-328-6601) between 8:30 a.m. and midnight, Eastern time, to speak with an Eaton Service Center representative at Fidelity.

What s Ahead October 4 25 October 25 November Mid-December Late December January 1 Annual Enrollment Choose your 2012 medical, dental, vision reimbursement accounts, disability and life insurance coverage. Remember to open your Health Savings Account (HSA) if you choose a Consumer Health option. Deadline for qualifying for the Know Your Numbers and Tobacco Free Healthy Incentives credits don t leave up to $1,200 on the table! See the information mailed to your home in late July for more details or on EatonBenefits.com under Eaton Learn More Links. Receive Health Savings Account Welcome Kit You ll receive this kit if you enroll in one of the Consumer Health options and you choose to open an HSA when you make your medical plan election. Confirmation of your elections mailed to your home Receive new ID cards If you choose Healthcare 80, you ll receive a new card for Anthem BCBS. KEEP your Medco ID card for 2012, as it will still be valid and must be used with Healthcare 80 coverage. If you choose a Consumer Health option, you ll receive a new ID card from Anthem. This one card will cover BOTH medical services and prescription drugs. Please destroy your Empire BCBS ID card as it will no longer be valid after December 31, 2011. You should also discard your Medco ID card if you choose one of the Consumer Health options. Your 2012 benefit elections take effect This summary presents descriptions of the 2012 medical options that will be available to eligible Eaton employees. In the event of a conflict between this and any other descriptions of the benefit plans, the plan document will control. As with all Eaton-sponsored benefit plans, Eaton reserves the right to amend, modify, revoke or terminate the plans in whole or in part at any time. Neither this summary nor participation in any Eaton benefit plan constitutes a promise of continuing employment with Eaton.

What You Need to Do Now Eaton s annual benefits enrollment October 4 through October 25 is your opportunity to review and select your benefits for 2012. Don t miss your chance to get the coverage that best fits your needs. Make sure you take action to earn your Healthy Incentives credits to reduce your 2012 medical plan contributions. Select the medical plan option that s best for you for 2012. Open a Health Savings Account to receive Eaton s contribution and to make contributions of your own. (You must enroll in one of the Consumer Health options to open an HSA.) If you select the Healthcare 80 option, participate in the Health Care Reimbursement Account (HCRA) if you want to use pretax dollars to pay for eligible medical, dental, vision or prescription expenses. If you select a Consumer Health option, you can also contribute to the Dental/Vision Reimbursement Account if you have opened a Health Savings Account (HSA) and want to use additional pretax dollars to pay for eligible dental or vision expenses. Review and make changes, if necessary, to the dependents you cover in Eaton benefit plans. What Happens If You Don t Enroll If you are currently covered by one of Eaton s medical plan options and you miss the opportunity to enroll for 2012, you ll automatically be covered by the Healthcare 80 option (unless you waive coverage). All other 2011 benefit elections will carry over for 2012, including Dependent Care Reimbursement Accounts, as well as Health Care Reimbursement Accounts for those participating in the Healthcare 80 medical plan option. If you are not currently covered by one of Eaton s medical plan options and you do not make an election, you will not have coverage through Eaton in 2012. You won t be able to enroll or make changes for the rest of the year unless you experience a qualified life event (such as getting married or having a baby).
