This brochure highlights what s changing in 2019 and provides you with information you need to make informed decisions that prioritize your health. Use this guide and all your resources to learn about your Medtronic benefits and make thoughtful choices during this year s open enrollment.

Take a second look It can be easy to fall back on your same elections year after year. But, are you enrolling in the best benefits for your current situation? Does your family look different from last year a new baby, spouse, or college-aged child, or are you recently widowed or divorced? Have your family s health needs changed, such as a surgery, braces, a new baby, or a health condition like diabetes or cancer? Would you like to save money by choosing more cost-effective benefits or paying less in taxes? Would you like to start saving for healthcare costs in retirement? Has it been over a year since you ve made changes to your benefits? BENEFIT CHANGES FOR 2019 In the spirit of taking healthcare Further, Together, every year Medtronic looks to innovate our benefits program, respond to your needs, and deliver the best possible healthcare experience. For 2019, there are a few changes you should review before you enroll. If you answer yes to any of these questions, now is the time to take another look at the complete package of benefits we offer.

Higher Health Savings and Spending Account Limits The IRS sets annual limits for how much you can contribute to a Health Savings Account (HSA) and Health Care Flexible Spending Account (FSA). Below are the contribution limits that apply to each type of account for 2019. New to the HSA? If you are enrolling in the Consumer Health Plan (CHP) with Health Savings Account (HSA) for the first time, be sure to enroll in Medtronic HSA contributions (elect the Employer Plan ), in addition to enrolling in the CHP medical option. At the same time, you can choose to make personal pre-tax contributions to your HSA. You also have a choice of plan administrator UnitedHealthcare (UHC) or BlueCross BlueShield of Minnesota (BCBS). Check out the provider networks and select the administrator that includes your preferred doctors and facilities. Health Savings Account (HSA) Limits Health Care Flexible Spending Account (FSA) Limit 2018 2019 2018 2019 Employee only $3,450 $3,500 Employee + spouse/ child(ren)/family $6,850 $7,000 $2,600 $2,650 The limits for the HSA include both contributions from Medtronic and any contributions you choose to make to the account. DID YOU KNOW? If you enroll in the CHP with HSA, Medtronic helps you save with an annual HSA contribution: $500/employee only $750/employee + spouse or employee + child(ren) $1,250/family

Healthier Together Update For the past several years, employees (and their covered spouses) who participate in Healthier Together and achieve Level 2 pay less for their medical coverage than those who don t. By achieving Level 2, employees avoided paying a premium differential. Now the premium differential is going away no one will pay a premium differential in 2019. Going forward, all employees will make the same contributions for medical coverage regardless of Healthier Together level achievement. We are committed to our partnership to build the physical, financial, and emotional well-being of our employees through a wellness program that is motivating and valuable to you. Without the premium differential, employees can engage in Healthier Together to enjoy exciting new rewards. Watch for more information early next year. If you and your covered spouse (if applicable) complete Level 2 by Dec. 23, 2018, you ll be entered into drawings for free medical premiums* and Recognize points. If you complete Level 4, you ll be entered into drawings to win prizes such as custom NIKEiD shoes, personal spa/fitness packages, wireless earbuds, and grand prize drawings for a travel package. *The free medical premiums will be based on the medical plan election made during the open enrollment period and does not include the spousal surcharge, if applicable. ALEX: Benefits Minus the Boring If it s your first time enrolling, or if you want new ideas from your go-to benefits, using ALEX, your online benefits guide, takes the decision-making process to a new level. Not only does ALEX do the math for you, but it helps you better understand your benefit options and shows cost comparisons and tax savings of available plans at Medtronic.

UHC Medical Plan Network Changes Beginning Jan. 1, 2019, employees enrolled in the Consumer Health Plan (CHP) through UnitedHealthcare (UHC) will have access to a higher level of benefit when they visit a UHCdesignated Tier 1 Provider. Tier 1 physicians, specialists, and facilities are evaluated by UHC based on their quality and cost-efficiency. Receiving care from a Tier 1 Provider may result in lower overall health costs for you and your family. You will pay the lowest coinsurance when visiting a Tier 1 Provider from UHC s network. If you choose an in-network doctor or facility who is not a Tier 1 Provider, your coinsurance and out-ofpocket costs will be higher. No benefits will be covered for out-of-network providers. Follow the steps below to verify your doctor s Tier 1 status during open enrollment: 1. Visit myuhc.com 2. Choose Find a Doctor and enter search criteria 3. Look for two blue hearts next to the provider s name to confirm their Tier 1 status effective Jan. 1, 2019 New Medical Plan for Select Locations Medtronic is offering a new on-demand medical plan option called Bind for employees in Colorado, Florida, and Minnesota in 2019. These locations were selected to pilot this plan for many reasons including their large employee populations, the discounts available from the network of providers, and the potential opportunities for employees and their families to save money on their healthcare services. Medtronic may offer the Bind plan more broadly in the future, based on the experience with employees in these three locations. Additional Benefit Changes More details about changes and your 2019 benefit options are available on benefits.medtronic.com. The calendar year plan maximum per person limit for the Basic Dental Plan will increase from $1,000 to $1,500. The CHP with HSA in-network deductible will increase by $100 for employee only coverage and $200 for employee + spouse, employee + child(ren), and family coverage.

ENROLLMENT IS AS EASY AS 1, 2, 3! Don t miss a thing this open enrollment. Keep this to-do list to check off each step of the way. Your 2019 Open Enrollment To-Do List: NOW NOV. 1-14: OPEN ENROLLMENT LATE NOVEMBER/ EARLY DECEMBER 1. Review your benefit options by visiting benefits.medtronic.com. Use ALEX our online benefits guide helps you assess your needs, compares available plans, and offers recommendations. 2. Visit Workday from the home page of benefits.medtronic.com to enroll in, update, or view your current benefits. Make sure you have your dependents Social Security numbers listed. Answering the spouse surcharge medical coverage question just got easier this year. If you are covering a spouse under a Medtronic medical plan, answer the medical coverage question in your open enrollment benefit event in Workday. 3. Review your confirmation statement that will be mailed to your home the week of Nov. 19. Make any corrections to your benefits, if necessary, during the correction period from Nov. 26-Dec. 4. What happens if I don t enroll? If you don t enroll by Nov. 14 or make corrections by Dec. 4: 1. Your current coverage will continue both the plans and the dependents you currently cover medical, HSA contributions (if applicable), dental, vision, life and accident insurances, and disability benefits. 2. You will not be able to take advantage of the Health Care and/or Dependent Daycare FSAs. You must indicate a contribution amount each year. 3. You won t be able to make changes to your benefits until the next open enrollment, unless you have a family status change (such as the birth of a child or marriage) that allows for coverage changes. 4. If you don t answer the spouse surcharge question between Nov. 1 and Nov. 14 and you cover your spouse, you will pay the surcharge of $92.31 per pay period. Keep in mind All benefits are effective Jan. 1, 2019. You will be mailed a new ID card to your home in late December if: You are a UHC member for 2019. You enrolled in your benefits for the first time. You recently made a change to your benefit elections.
