1. Purpse The GHD HSE Risk Assessment identifies the HSE hazards and risk assciated with ur business and prjects. This Prcedure identifies thse (pre-cntrl) risks that the HSE Risk Assessment determined t have the ptential t cause catastrphic utcmes t GHD staff and external suppliers / vendrs, t highlight thse GHD SAFEguards that if implemented are designed t eliminate a serius r fatal incident frm ccurring Caring fr peple s safety will guide what we d and as such, we ALL must adhere t the fllwing cre safety principles: We understand GHD s HSE expectatins and can demnstrate cmpliance with them. Refer t HSE304 Smart Behaviurs. We are all empwered t stp unsafe acts and/r cnditins. Refer t HSE369 Stp Wrk Authrity We take respnsibility fr ur wn and each ther s safety. We plan ur wrk t prevent unsafe situatins. We imprve by learning frm ur experiences. Our external suppliers and vendrs supprt ur visin and principles. Safety is a cre value at GHD and we are cmmitted t uphlding and enfrcing these principles that frm the backbne f ur HSE Management System. 2. Scpe This Prcedure applies t all GHD emplyees wh wrk fr r n behalf f - GHD n any GHD r client-cntrlled site. Refer t the Glssary f Terms fr relevant definitins. Where a jurisdictin r client s requirements differ frm the GHD SAFEguards, the mre stringent f the requirements are t be applied. GHD SOP-HSE-359 SAFEguards 1

3. Respnsibilities 3.1 Management (GHD, Reginal and Prject) (Leaders) Take all reasnable practicable steps t prtect the health and safety f GHD peple at wrk. Clearly articulate expectatins and Prcedures with staff regarding GHD s SAFEguards and manage staff behaviur where expectatins are nt achieved using HSE304 SMART Behaviurs. Ensure that peple emplyed n GHD prjects are adequately trained and where necessary accredited / certified / licensed t perfrm the required activities, including external suppliers / vendrs/. Ensure that persnnel under their cntrl have access t the SAFEguards and any resurces necessary t cmply with them. Ensure jurisdictin and client prject specific expectatins are identified and satisfied. 3.2 Emplyees D nt attempt t cmplete any wrk activity withut required the training and where necessary accreditatin, certificatin r license. Infrm manager f any newly identified hazards. Infrm manager if they are taking any medicatin (prescribed r nt), which may adversely affect ability t deliver wrk safely r affect the safety f thers. Participate in the f the risk management prcess, including identifying hazards, cntrl measures and assessing risk. Reprt incidents, near misses and hazards HSE 3.3 HSE team Prvide, advice, supprt and training t facilitate implementatin f this Prcedure. Develp and implement a schedule t prmte individual GHD SAFEguards n an nging basis, including cmpletin f assciated e-learning t keep them frnt f mind. 4. Prcedures 4.1 GHD SAFEguards The GHD SAFEguards address thse actins where nn-cmpliance may lead t death r serius injury t emplyees, external suppliers / vendrs and ther stakehlders. GHD SAFEguards ALWAYS develp; a Cmmunicatin Plan when travelling int remte r islated areas and an apprved Travel Risk Management Plan when travelling int high r extreme risk destinatins. * ALWAYS bey the rad rules (e.g. speed, use f mbile phnes, influence f alchl and/r drugs etc.) and plan t avid fatigue when driving. * ALWAYS make sure that equipment/ machinery is in a safe cnditin and yu have the necessary training and cmpetence befre use.* ALWAYS btain authrisatin thrugh risk assessment, training and entry permit befre entering a cnfined space. ALWAYS islate equipment r plant befre starting wrk and never remve a persnal danger/islatin tag belnging t anther persn.* ALWAYS btain permissin prir t entering an excavatin r barricaded area. ALWAYS btain authrisatin prir t entering a rail crridr r wrk area within the crridr. ALWAYS gain peratr visual acknwledgement and maintain safe wrking distances when entering areas with perating traffic, mbile plant r machinery. ALWAYS identify the lcatin f undergrund services befre disturbing the grund and ALWAYS maintain safe wrking distances (and authrisatin where applicable) when wrking in the prximity t verhead utilities. ALWAYS when wrking at heights ensure yu are apprpriately trained, wear specified fall prtectin equipment and dn t leave tls r equipment unsecured. Ntes There are 4 SAFEguards marked with an (*) these are applicable t all emplyees. Applicability fr thers will be determined via the Reginal Risk Assessment. While the SAFEguards identify a number f critical cntrl measures they are by n means an illustratin f the nly cntrls required t effectively reduce risk. The SAFEguards d nt negate the need t develp prject specific risk management dcumentatin e.g. JSEA and d nt replace existing Prcedures r guides. They are designed t fcus attentin n individual SAFEguard risks and must be incrprated int cntrl measures fr relevant hazards in risk management dcumentatin, after determining whether the prject can be dne anther way. E.g. Eliminate the need t g int a cnfined space, wrk at heights etc. GHD SOP-HSE-359 SAFEguards 3

While GHD management are respnsible fr the implementatin f risk management strategies, effective risk cntrl als relies n individuals being respnsible empwered t intervene t help their clleagues and clients when uncntrlled risk is identified. The SAFEguards may be reviewed as new SAFEguards, and hazards identified during risk management prcesses as having pre-cntrl ptential f causing catastrphic utcmes must be managed with similar imprtance t the SAFEguards and made knwn t the lcal HSE Team, fr reprting t the Grup Manager HSE. 4.2 Implementatin f GHD SAFEguards The intent f the SAFEguards is t fcus risk management activities n basic principles, t manage surces f highest cnsequence as a pririty. The fllwing prcess will be implemented fr this SAFEguards t have the intended effect: Identify Cmmunicate Review Reginal HSE Manager is t maintain the Reginal HSE Risk Assessment and Legal Register, including identifying the jurisdictinal legislative and GHD requirements (including internal and external training) fr managing hazards in the and the relevant GHD Hazard Guides / Prcedures. Use the Risk Assessment and Legal Register t identify thse hazards with a pre-cntrl risk rating f Significant r Extreme and the GHD SAFEguards that are applicable t the Regin. Nte: The Reginal Risk Assessment and Legal Register is maintained n the HSE Frum (Knwledge Dcument: Risk Management / HSE Risk Assessment / Reginal). Reginal HSE Manager develps a GHD SAFEguards Schedule t prmte individual SAFEguards within specified timeframes. Fr example, Regins may elect t fcus n 1 SAFEguard r Hazard per mnth r 3 per quarter. This schedule is t be maintained n the HSE Frum (Knwledge Dcument: Hazards / Reginal). In accrdance with the GHD SAFEguards Schedule, cmmunicate the specific SAFEguards, alng with jurisdictinal legal / GHD guidance / training requirements (internal and external) within the Regin using agreed cmmunicatin channels (e.g. Management Teams Service Grup / Office meetings, HSE Cmmittee activities and jb specific meetings) t create a level f understanding and appreciatin within the Regin. Such briefings are t be captured as HSE Briefings in the HSE Database. Nte: The Reginal HSE Risk Assessment and Legal Register identifies the Service Grups / Offices expsed t particular hazards and is t be used t identify the target audience fr cmmunicatin f particular SAFEguards. Frmal reviews t verify that legal and GHD requirements are being implemented fr specific SAFEguards during timeframes specified in the Reginal SAFEguards Schedule. Management review f SAFEguards implementatin will be undertaken and might include: Inspectins (Office, Wrkstatin, Prject), Prject HSE Reviews, staff discussins (entered as Briefings in HSE Database). Reginal HSE Managers are t mnitr implementatin f the abve methds and review: Training recrd in Learning Management System and Reginal HSE Training Matrix. 4 GHD SOP-HSE-359 SAFEguards

Near miss / hazard / incident reprts and investigatin data t identify trends. Reginal Risk Assessment and Legal Register and update fllwing review activities, where required. Reprt Implementatin during Reginal Cmpliance Audits. All emplyee briefings shuld be captured as HSE Briefings in the HSE Database. The utcme f reviews are t be reprted t the respective Reginal Management Team. Recrds f such reviews are t be maintained n the HSE Frum (Knwledge Dcument: Mnthly Perfrmance Reprt / Year# / Reginal). Key findings relating t GHD SAFEguards (e.g. effective management, incidents etc.) will be made knwn t the EMG by the Grup Manager HSE and will als feature as a cmpnent f the annual BS HSE management review. 4.3 Managing cmpliance Management need t act decisively and transparently regarding the implementatin f the SAFEguards t achieve lng lasting change in peple s behaviur. T bring abut effective change, management shuld: Illustrate active cmmitment in the adptin and implementatin f the SAFEguards. This requires mre than just talking up the SAFEguards, it als requires demnstratin f cmmitment t them. Make HSE expectatins knwn, reward behaviur we want repeated and discurage undesirable behaviur. Actively prmte and mnitr emplyee cmpliance with GHD HSE systems. All incidents invlving GHD SAFEguards will be investigated t identify rt cause and assciated actins/behaviurs requiring management attentin (either psitive r crrective).
