. SUPPLY OF PEST CONTROL SERVICES TENDER DETAILS: DURATION: Tenders should be prepared on the basis of a twelve (12) month arrangement commencing 1 st July 2018. CLOSING DATE: Monday 28th May 2018. TENDER APPLICATION REQUIREMENTS: Provide pricing information for the items detailed in the pricing section. Provide a brief company profile addressing History, Service, Experience and commitment to quality assurance and Work Health and Safety. Provide certificates of currency for public liability insurance and work cover Provide a National Criminal History check for all contractors working on site (can be organised through The Home, fee applies) The successful tender will provide to all of The Good Shepherd Home s buildings (see attached site plan) and also to Tumbetin Tea Rooms (located next to the Home): SCOPE OF WORK Quarterly Pest Control for the treatment of cockroaches, silverfish and ants. First quarterly treatment is due July 2018 Rodent Management. Services calls as and when required due to reinfestation. One termite inspection of all buildings on site in November 2018. RESPONSIBILITIES To maintain the Home in a condition that is free of infestations of insects and rodents to the extent as set out in the scope of works. PRICING Pricing is to remain fixed for the period specified above. Please provide fixed pricing individually for each of the following; The Good Shepherd Home Quarterly Pest Control fee Rodent Management fee Service call out fees Tumbetin Tea Rooms Quarterly Pest Control fee Rodent Management fee

EVALUATION OF OFFER All submissions will be evaluated using our matrix selection method to ensure fair evaluation. Both price and non-price factors will be considered. QUALITY ASSURANCE The successful supplier shall be committed to Quality Procedures. Please provide details of your commitment to quality assurance as part of your company profile. Please include details of your accreditation and your willingness to participate in the Home s Quality Assurance Program. WORK HEALTH & SAFETY Work Health & Safety regulations must be complied with at all times in the provision of products and services. The Homes Work Health & Safety details are available on request. At all times the Homes Work Health and Safety guidelines (Contractors Site Requirements) should be referred to for compliance to The Homes policies. INSURANCE: The successful supplier agrees to keep the following insurances current for the duration of the agreement. WorkCover Policy, and Public Liability Policy NATIONAL CRIMINAL HISTORY CHECKS In accordance with the Commonwealth Aged Care Act 1997, The Good Shepherd Home now requires all contractors that regularly provide services to the Home to supply a certified copy of a current National Criminal History Check. These NCH checks must be renewed every 3 years from the date of issue. It is the responsibility of the contractor to apply and provide a current Police Certificate to The Home. Contractors are able to obtain a National Criminal History check through their local police station at a small cost. Any employees of your business that may complete works on site regularly must provide a current police check. COMPLAINTS PROCESS All complaints must be investigated and responded to within 14 days. Please nominate a Contact Person through which any complaints should be directed in the first instance. Should you wish to furnish a complaint to the Home for any reason the following procedure should be followed: (i) Initial complaint should be directed to our Hotel Services Manager (ii) If the complaint is not dealt with satisfactorily it should then be referred to

the Home s CEO. TERMINATION If either party wish to terminate the tender agreement, 60 days notice in writing must be given. The Home has the right to terminate the agreement forth with if the supplier does not comply with the specifications with in this document. PRIVACY This information is being collected for the purpose of processing your tender. The intended recipient of this information will be The Good Shepherd Home. Please note that the provision of this information by you is voluntary. However, if you do not provide the information requested The Good Shepherd Home may be unable to process your tender. You have the right of access to, and alteration of, personal information concerning yourself held by The Good Shepherd Home in accordance with the Act. The information is being collected and will be held by The Good Shepherd Home. You may request a copy of The Good Shepherd Home s Privacy Statement for details related to this statement including information collection, usage, dissemination and access. CONTRACTOR SITE REQUIREMENTS INTRODUCTION The procedures and conditions set out in this document are mandatory site requirements and a precondition for permission to work at The Good Shepherd Home. SIGNING IN/SIGNING OUT All Contractors/Visitors must sign in at reception prior to commencing works. (Contractors who arrive on site prior to or after business hours must make prior arrangements with the Maintenance Supervisor). The process of signing in to the Contractor/visitor s Book is acknowledging that you (Contractor/Visitor) will comply with the Home s safety responsibilities. Any changes to access times must be agreed to by the Maintenance Supervisor. Contractor/Visitor ID badges must be worn at all times whilst working in or around the Home EMERGENCIES It is important that you familiarise yourself with the emergency & evacuation plans for The Good Shepherd Home. Evacuation plans are shown in all of the Home s areas. In case of emergency contact the Workplace Health & Safety Officer on 4772 9912 or Facilities Maintenance Manager on 4772 9954 or Reception on 4772 9900.

FIRE SAFETY Contractors/Visitors should take the time to observe the fire exits and fire safety equipment closest to their place of work in The Good Shepherd Home. Fire exit doors are located throughout the home and are clearly signed. The Home is monitored by an electronic fire detection system. This is also monitored by the local Fire Brigade. Costs relating to false alarms that are caused by a contractor are the responsibility of the contractor. Costs exceed $1000 per call out. All work likely to interfere with the Fire Alarm System are to be identified by the contractor and reported to the Maintenance Department PRIOR to work commencing so that detectors can be isolated. HAZARDOUS MATERIALS In case of spills involving hazardous material the Contractor is to immediately notify the Maintenance Supervisor on Ph.: 4772 9954 and the Safety Officer on Ph.: 4772 9957 and advise where the spill has occurred and what substance has been spilt. It is the responsibility of the Leading Hand or Foreman for each Contractor Company or independent contractor to advise The Good Shepherd Home if they will be undertaking work requiring any of the following; *Electrical Isolation *Confined Spaces *Working at Heights *Dangerous Goods or Hazardous Materials *Fire Services Isolation *Hot Work *Use of Plant & Equipment *Noise from works or equipment FIRST AID First Aid Officers and Registered Nurses are available throughout The Good Shepherd Home. There is a first aid room located in the Day Therapy Centre. MEDICAL EMERGENCY Immediately seek assistance from a Good Shepherd Home staff member for the purpose of contacting the nearest Registered Nurse or First Aid Officer. NOTE: if you are aware of any further risks or hazards that have the potential to harm attending medical personnel please advise them prior to their commencing any emergency medical assessment or treatment. OH&S POLICY AND RESPONSIBILITIES All Contractors/Visitors performing work at The Good Shepherd Home are required to comply with the Home s WH&S policy, procedures and work instructions and manuals. The Contractor/Visitor is responsible to ensure that all works undertaken comply with

the relevant Workplace Health and Safety Act, Legislation, Regulations and Codes of Practice. RISK MANAGEMENT PROGRAM The Contractor/Visitor is responsible for identifying risks or hazards in their work area, advising of same to the Maintenance Supervisor and/or the Workplace Health & Safety Office INTERACTION WITH THE HOME S STAFF, VISITORS AND RESIDENTS Residents and their Visitors have right of way at all times. Contractors should ensure that they are not working unsupervised in Resident living areas and permission to enter wings must always be obtained from the Registered Nurse responsible for that wing. Particular care should be taken when working in dementia wings and further induction should be sought prior to entering these areas. Noise should be kept to a minimum whilst working in the vicinity of Resident living areas. SAFE WORK METHOD STATEMENT / JOB SAFETY ANALYSIS Contractors/Visitors will be asked to complete a Safe Work Method Statement or Job Safety Analysis (JSA) for all work being performed. HOUSEKEEPING AND WASTE All work areas must be kept clean, tidy, free and clear of hazards at all times, this is of importance in resident s rooms, public access areas and all thoroughfares and emergency exits. At no time shall any waste, chemical, cement or sediment be allowed to enter storm water drains. At no time are tools or waste or equipment to be left unsupervised. SMOKE FREE POLICY The Good Shepherd Home is a no smoking workplace. CONSUMPTION OF ALCOHOL The consumption of alcohol, and or the bringing of alcohol onto the premises whilst performing work or operating equipment at The Good Shepherd Home, is strictly prohibited. AMENITIES For designated eating areas and toilets, please check with the Maintenance Supervisor for relevant information. ACCIDENT INVESTIGATION All accidents, injuries and hazards should be brought to the attention of The Good Shepherd Home. The Contractor/Visitor may be required to complete an Incident or Hazard Form.

TRAFFIC REGULATIONS For the protection of our Residents and Staff we have a 15km per hour speed limit within our grounds. In addition the traffic flow, when accessing the delivery points is one way, around the top car park. All contractors and suppliers are required to adhere to this while on site. Do Not park outside the kitchen/laundry area. NON COMPLIANCE If any Contractor does not satisfy all of the conditions outlined in this document, this will be reported to the Contractor Company. The Contractor Company may need to respond to the report in writing and take steps to ensure the breach does not occur again. Failure to respond could lead to termination of current contract and/or any future work at the Contract Company s expense. All self-employed contractors, upon occurrence of the breach, will be put on notice and may need to provide a written response. In the case of all other unsafe practices, the Contractor Company or the self-employed contractor will be advised of the unsafe practices and directed to correct the condition immediately and/or cease the work being performed. The Good Shepherd Home reserves the right to require work to stop immediately and to require the contract worker to leave the site if any safety procedures are violated.
