PDSSN's Buddy Walk & 5K Race Saturday, October 4, 2014 Re: Buddy Walk & 5K for Down Syndrome Awareness Saturday, October 4, 2014 5K Timed Race: 8:45am Buddy Walk Festivities: 9am - 12:30pm The Buddy Walk was established in 1995 by the National Down Syndrome Society to celebrate Down Syndrome Awareness Month in October and to promote acceptance and inclusion of people with Down syndrome. Today, the Buddy Walk program is supported nationally by NDSS and organized at the local level by parent support groups, schools and other organizations and individuals. It is an inspirational and educational event that celebrates the many abilities and accomplishments of people with Down syndrome. We invite you to join us as a sponsor of the Buddy Walk. PDSSN (Piedmont Down Syndrome Support Network) is a local nonprofit organization that serves to enrich the lives of individuals with Down syndrome and to provide information and support to families and professionals in our community. Your support of this event would help our efforts to advocate and educate throughout the year. We are a 501(c)(3) organization and all donations are tax deductible. For your reference, our Tax ID Number is 20-3131948. This year will be our 14th Annual Buddy Walk and our first 5K Race. In 2013, we had over 1,000 members of the community come out to support Down Syndrome Awareness and raised over $55,000 for programs provided by PDSSN. This fun filled event is designed to foster community involvement and produces visible, tangible, unifying outcomes that you and your employees can be proud of for years to come. Remember that this community event will have long-term, positive repercussions in the city where your customers live and work. We are asking for your help! Impact of Your Sponsorship Did you know that the money raised at the PDSSN Buddy Walk pays for the No Limit II Dances? Here are just a few of the programs PDSSN is able to provide with your support! No Limits II Dances Each quarter, PDSSN hosts a No Limits II Dance for adults (18 years or older) with intellectual, developmental, and physical challenges. No Limits II Dances provide opportunities for adults with disabilities to enjoy food, fun and fellowship with activities such as dancing, bingo, refreshments and door prizes at no cost to attendees. More than 500 people come from six counties to enjoy the dances. One guest said, "I want the world to know this is the best place you could ever come!" First Call Program Next Chapter Book Club Our First Call program is a volunteer group of experienced parents who make themselves available to parents of children newly diagnosed with Down syndrome. Whether people have received a diagnosis prenatally or after the birth of their children, we are here to help. We answer questions honestly and do our best to describe the joys and challenges we have experienced in our own personal journeys. We provide a Resource Notebook to each family containing up-to-date information about Down syndrome and resources available to the family. One grateful parent said, "It was very reassuring to hear another family's stories and to learn about all of PDSSN's programs." The Next Chapter Book Club (NCBC) promotes literacy and social connection experiences for adolescents and adults with intellectual disabilities that encourage friendship and lifelong learning within a community-based setting. Participants, regardless of their reading skills, gather weekly with volunteer facilitators in a local bookstore, coffee shop, or cafe to read aloud and discuss a book. NCBC received the Prevention, Outreach, and Wellness Program of Excellence Award from NC Council of Community Programs in 2013. When asked what she thought about book club, one young woman exclaimed, "It's fun to learn how to read all together!" Family Events & Parent Networking Exceptional Families From monthly Ladies' Night Out & D.A.D.S. Group meetings to quarterly family social gatherings, funds raised through the Buddy Walk allow PDSSN to provide frequent opportunities for members and friends to get together. One dad summed it up by saying, "whether you're just beginning the journey or you ve been on this road for a few decades, it s a time to laugh, tell stories, receive affirmation, be involved and enjoy this beautiful gift called life." PDSSN partners with the Family Support Network of Greater Forsyth to host monthly educational meetings with dinner and child care provided at no cost to attendees. Guest facilitators lead discussions regarding topics of interest for parents of children who have intellectual and developmental disabilities. Past topics have included: IEP Preparation; Guardianship; SSI benefits, Dental Care for Kids with Special Needs; Assistive Technology; and more. One parent remarked, "I was so grateful to get my questions answered!" We hope to count on your support as PDSSN expands its programs and services. Future developments. The IRS has created a page on IRS.gov for information about Form W-9, at www.irs.gov/w9. Information about any future developments affecting Form W-9 (such as legislation enacted after we release it) will be posted on that page. Purpose of Form A person who is required to file an information return with the IRS must obtain your correct taxpayer identification number (TIN) to report, for example, income paid to you, payments made to you in settlement of payment card and third party network transactions, real estate transactions, mortgage interest you paid, acquisition or abandonment of secured property, cancellation of debt, or contributions you made to an IRA. Use Form W-9 only if you are a U.S. person (including a resident alien), to provide your correct TIN to the person requesting it (the requester) and, when applicable, to: 1. Certify that the TIN you are giving is correct (or you are waiting for a number to be issued), 2. Certify that you are not subject to backup withholding, or 3. Claim exemption from backup withholding if you are a U.S. exempt payee. If applicable, you are also certifying that as a U.S. person, your allocable share of any partnership income from a U.S. trade or business is not subject to the Date withholding tax on foreign partners share of effectively connected income, and 4. Certify that FATCA code(s) entered on this form (if any) indicating that you are exempt from the FATCA reporting, is correct. Note. If you are a U.S. person and a requester gives you a form other than Form W-9 to request your TIN, you must use the requester s form if it is substantially similar to this Form W-9. Definition of a U.S. person. For federal tax purposes, you are considered a U.S. person if you are: An individual who is a U.S. citizen or U.S. resident alien, A partnership, corporation, company, or association created or organized in the United States or under the laws of the United States, An estate (other than a foreign estate), or A domestic trust (as defined in Regulations section 301.7701-7). Special rules for partnerships. Partnerships that conduct a trade or business in the United States are generally required to pay a withholding tax under section 1446 on any foreign partners share of effectively connected taxable income from such business. Further, in certain cases where a Form W-9 has not been received, the rules under section 1446 require a partnership to presume that a partner is a foreign person, and pay the section 1446 withholding tax. Therefore, if you are a U.S. person that is a partner in a partnership conducting a trade or business in the United States, provide Form W-9 to the partnership to establish your U.S. status and avoid section 1446 withholding on your share of partnership income.
