3 REQUIRED ANNUAL BENEFITS ENROLLMENT 2015 It s Different This Year For 2015, rather than a passive enrollment, you are required to complete the Annual Enrollment process steps - either online or by phone with the Halliburton Benefits Center. When you complete the Required Annual Enrollment process this year, you will be required to: Confirm that the dependents you have enrolled are eligible for coverage; and Disclose whether you and your spouse (if any) have or have not used any tobacco products in the last 12 month period. The Stakes Are Higher The difference in employee contribution rates are increasing for Tobacco Users from 30% to 40% greater than the rates for Non-Tobacco Users for For example, your annual contribution for family coverage for the Global Medical Plan would be only $1,572 if you and your spouse are Non-Tobacco Users, or $2,196 if you or your spouse are Tobacco Users. Optional Life Insurance rates for you and your spouse may also be higher. IT S DIFFERENT THIS YEAR You must enroll! THE STAKES ARE HIGHER Tobacco-users pay even more! DON T LOSE OUT on what is important to you & your family! Don t Lose Out on What is Important To You and Your Family Complete the Required Annual Enrollment to ensure you certify Tobacco Use correctly for you and your spouse. Make sure you are enrolling only dependents that are eligible. Quit now if you use tobacco products. If you quit anytime during 2014 and do not use tobacco during 2015, you will be eligible to pay lower health care and life insurance costs in Select extra optional coverages if your family will need income if you are unable to work or die. 4 What is Annual Benefits Enrollment and Why Does it Matter? First, consider these headlines and why they would matter to you: What matters to you? Don t put your family AT RISK! Review your benefits coverage and the levels they provide you, your dependents or others. Enroll at levels that match your needs. Headline An employee dies unexpectedly. A wife and three young children are left without a regular income. Headline An employee is hurt in a car accident and is unable to work for 2 years. The Basic Long Term Disability payments do not provide enough income for the employee and his/her family. Headline An employee has a beautiful new daughter their first born. The employee forgets to enroll the new child within 30 days of her birth as a result she has no health plan and can t be covered until the following year. The Annual Enrollment period provides you the opportunity to review the level of benefits you are enrolled in and determine if they are still appropriate for you and your needs. For 2015 the Company is requiring you to complete your Annual Enrollment, so think about which of your family members need to be enrolled, and anyone that would need to be cared for if you cannot work or die. For example, as families mature and children leave home to marry and pursue careers of their own, you may not need to provide the protection levels necessary for a young family. Employees needs are different and evolve over time. Don t let one of the headline examples happen to you! Decide what matters to you and complete your 2015 Required Annual Enrollment by following these easy steps: 1. From the HalWorld Home Page, click on Employee Benefits (near the bottom of the page), then here in middle of page no password needed. OR from the internet, log onto and enter your password. 2. Click on the Health & Benefits icon and follow the steps. 3. Click on Submit to ensure your enrollment is completed! 4. And if you experience a life event during the year, like the birth of a child, inform the Halliburton Benefits Center before the applicable deadline. If you are unable to access the online system, you may call the Halliburton Benefits Center. 6 TOBACCO TOP 3 REASONS TO QUIT USING TOBACCO NOW! 1. Spend 40% less on your Global Medical Plan contributions in 2015 if you fit the definition of a Non-Tobacco user. 2. Avoid an even higher differential for tobacco users expected to be 50% in coming years. 3. Pay lower rates on Optional Life Insurance coverage for you and your spouse. HOW WE DEFINE TOBACCO USE Tobacco use for the Global Medical Plan includes: Smoking Vaping (e-cigarettes) Smokeless tobacco such as snuff To certify that you and your spouse (if any) are NOT tobacco users, you must certify that you both have not used any tobacco products within the last 12 months and that you both will continue to refrain from using any tobacco products while you are receiving the Non-Tobacco user discount. Your certification is subject to the Halliburton Code of Business Conduct. This code sets forth the standards, principles, laws and regulations that affect every aspect of our business. The Code also lays out the consequences for violations, noncompliance and illegal, unethical or unacceptable behavior. WE SUPPORT YOUR EFFORTS TO QUIT Prescription and non-prescription tobacco cessation products are eligible for coverage under the Global Medical Plan. The LiveWell program offers coaching and programs to help you quit. 8 FAIR IS FAIR FRAUD IS FRAUD Most employees and their family members are fair, honest and ethical when using the Global Medical and Global Dental Plans. Under the Code of Business Conduct ( COBC ), you have an obligation to protect the Company s assets and ensure their careful and legitimate use. The Global Medical and Global Dental Plans are self-insured, which means that the underlying assets belong to the Company. Company assets for these programs are to be used only for the payment of eligible health claims and administrative expenses. When fraudulent claims are submitted to the Global Medical and Global Dental Plans, you and the Company pay the price through higher contributions. Fraud occurs when a person obtains money or some other benefit by deliberate deception. Fraud is a crime and it is a disciplinary offense under the COBC. When an employee defrauds the Global Medical and Global Dental Plans by submitting fraudulent claims or enrolling ineligible persons (individuals who are not eligible dependents as defined under the Plan), this results in obtaining Company assets by deception. Fraud that has been committed against the Company is a violation of the COBC and may result in disciplinary action up to and including termination. Help us keep the costs of our health care programs down. Contact Cigna Global s customer service center or the Halliburton Benefits Center if you have questions about the definition of eligible dependent. Contact the Company s Ethics Helpline to report any possible COBC violation. 07
9 Participating in the Halliburton Retirement and Savings Plan Halliburton wants to help you prepare for your financial future. By providing you with the Halliburton Retirement and Savings Plan, the Company has made it easier than ever for you to save for retirement and other long-term financial goals. How to Maximize Your Benefit If you currently earn $40,000 per year and have elected to contribute 4% of your eligible pay on a tax-deferred basis to the Plan, by increasing your contribution to 6% you may increase your overall account balance by approximately $19,400 in 10 years ($67,000 in 20 years; $174,000 in 30 years)*. The Company match occurs on a per pay period basis so plan accordingly to maximize this benefit by contributing at least 6% of your eligible pay in order to receive the full Company match of 5%. The IRS limits the amount of tax-deferred contributions you can make in a year (currently $17,500 for 2014) and limits the amount of compensation recognized for Plan purposes. If you are age 50 or over (or attaining age 50 during the year), you may also elect to make catch-up contributions to the Plan. The Internal Revenue Code (IRC) maximum allowable catch-up contribution for 2014 is $5,500. *Assumes annual salary increases of 4% and annual investment returns of 7%. Halliburton makes it easy to gradually increase your savings with the Plan s Automatic Escalation Feature. The Automatic Escalation Feature increases deferrals each year and can help you get closer to your retirement income goals. Under this feature, you can elect an annual percentage increase for your tax-deferred savings contributions to the Plan and set a target deferral percentage of your salary goal. This feature automatically increases your annual tax-deferred savings contributions each year in January until your target deferral percentage of salary is reached. Naming or Changing Your Beneficiaries If you haven t reviewed them in a while, now is a good time to take a look at your beneficiary designations. Log on to the Halliburton Total Rewards Website and click on the Wealth icon. From the NetBenefits home page click on the Your Profile link at the top of the page, and then select the Beneficiaries link. You can receive Halliburton contributions to your Retirement and Savings Plan account equal to 9% of your eligible Plan compensation for the year. The Company match* is 5% (if you contribute 6% or more) and the basic contribution* is 4% (*subject to Plan provisions and applicable limitations). You have the option to select from a set of Target Date Portfolios that automatically adjust their investment mix, or asset allocation, as you approach the expected retirement date at age 65. Retirement portfolios are professionally managed for you based on your age and expected retirement date. Alternatively, you may choose from 10 custom investment options to build your own retirement portfolio. To access your Halliburton Retirement and Savings Plan information online, visit the Halliburton Total Rewards website, and click on the Wealth icon. You may also call the Halliburton Benefits Center at (international: use your country s AT&T access number) or (select Option 2), Monday through Friday, from 7:30 a.m. to 7:30 p.m. Central Time. Learn more about Retirement Planning. With Fidelity as the Halliburton Retirement and Savings Plan provider, you can choose what works best for you. The Halliburton Benefits Center at Fidelity offers you the guidance, information and tools you need for indepth financial assistance. Phone Speak directly with a Fidelity Planning and Guidance Consultant through the Halliburton Benefits Center automated phone system. 10 When Does Health Care Coverage for Your Children End? Your Global Medical and Dental Plans allow coverage of your dependent children up to age 26*. We do our best to alert you in advance if dependent coverage is about to expire. Here is what you can expect to receive if coverage for your child will end. ALERT from the HALLIBURTON BENEFITS CENTER Dependent Child turns 26 within 30 days NOTE: COVERAGE WILL TERMINATE at end of month. *Note coverage may extend past this age if your child is dependent upon you due to a disability or handicap. However, even if we do not alert you, dependent coverage will terminate according to the terms of the Global Medical and Global Dental Plans. 09

The Women s Health and Cancer Rights Act of 1998 Under Halliburton s Global Medical Plan, coverage will be provided to an employee or covered dependent who is receiving benefits for a medically necessary mastectomy and who elects breast reconstruction after the mastectomy for: All stages of reconstruction of the breast on which a mastectomy has been performed Surgery and reconstruction of the other breast to produce a symmetrical appearance Prostheses Treatment of physical complications of all states of mastectomy, including lymphedemas. This coverage will provide benefits in consultation with the attending physician and the patient, and will be subject to the same deductibles and co-insurance provided for the mastectomy.
