3 What s New for 2018 (Effective January 1, 2018) Dental Health Now Part of the Shift into Better Health Premium Incentive Program Tobacco Cessation Update New Spousal Surcharge Requirement Research shows the growing importance of good dental hygiene and its effect on the heart and overall health. That s why BorgWarner has updated its premium discounts to incentivize you for good dental health. You can now earn a premium discount point for completing a dental preventive visit. Keep in mind, the BorgWarner dental plan pays 100% of a preventive visit, which includes an exam, cleaning and bitewing X-rays. This discount point replaces the current tobacco-free discount point. Although the premium discount for living a tobacco-free lifestyle is being replaced with dental preventive care, BorgWarner still supports a tobacco-free lifestyle through its $100 reward for completion of the Cigna Tobacco Cessation program. If your spouse is covered under a BorgWarner medical plan, he or she will need to complete an additional activity to avoid paying the $100 monthly surcharge. In addition to completing the Cigna HRQ (within 60 days if new to the plan), your spouse also will need to complete one or more of the following preventive visits by Oct. 31, 2018: Preventive annual physical (with their Primary Care Physician or OB/GYN) Preventive colon cancer screening (age 50+) Preventive mammogram (age 40+) Cigna will apply credit through claims data. Spouses with BorgWarner secondary medical coverage may submit a qualifying preventive visit/screening EOB directly to Cigna to meet this qualification. Enhanced Autism Benefit Coverage New Diabetes Care Management Benefit Pharmacy Updates Employee Contributions and Out-of-Pocket Maximums Applied Behavior Analysis (ABA) therapy for an autism diagnosis has been added to both the Basic and Buy-Up medical plans. In addition, age and day limits for Short Term Therapy (STR) have been removed. Medical necessity is the only criteria applied for the number for physical therapy, speech therapy and occupational therapy visits for the treatment of autism. LIVONGO is a new benefit available to employees and their dependents who are enrolled in a BorgWarner medical plan and who have been diagnosed with Type I or Type II diabetes. This benefit includes unlimited lancets, testing strips and a glucose meter, along with comprehensive diabetes care management support all covered at 100%. Details about LIVONGO will be available during open enrollment meetings. With the implementation of the Livongo diabetes care management benefit, diabetic supplies are now subject to copays as outlined in the 2018 Benefits Reference Guide. Employee contribution rates for medical and vision coverage are changing for The dental contribution rates, however, will remain the same. Refer to the next page for details. Medical out-of-pocket maximums will be adjusted to comply with 2018 federal mandates. For details, refer to the 2018 Benefits Reference Guide posted at Key Dates Early November November 13 - November 30, 2017 January 1, 2018 Onsite employee enrollment meetings Online enrollment period 2018 benefit elections are effective; check coverage through
