2 You ll find some new benefits and some enhanced benefits for 2018 new benefits including short-term disability, long-term disability, spouse life insurance and child life insurance. Enhanced benefits like increased Company-paid life/ad&d coverage from $25,000 to $50,000 and additional employee supplemental life insurance up to $950,000. This year, you ll find a lot more on the menu. 3 UnitedHealthcare myuhc.com or You can choose from two BEK medical plans the BEK HSA Medical Plan and the BEK PPO Medical Plan. These are the same plans offered last year. Both plans include: In-network preventive care covered at 100% Coverage for eligible children up to age 26 Rx coverage See any in-network provider Key features of the medical plans BEK HSA MEDICAL PLAN Get a tax-free Company contribution of $1,000 (individual) or $2,000 (family) to a health savings account You can make pre-tax contributions up to $3,450 (individual) or $6,900 (family) BEK PPO MEDICAL PLAN Pay a copay and/or calendar year deductible before the plan begins to pay coinsurance Has lower calendar year deductible and out-of-pocket maximum than the BEK HSA Medical Plan Use HSA dollars to pay for eligible expenses like deductibles, vision or dental or choose to pay out of your own pocket Meet your annual calendar year deductible and out-of-pocket maximum then pay nothing for in-network care Unused HSA dollars rollover year to year YOU PAY: Lower costs per pay period YOU PAY: Higher costs per pay period $ from BEK Use HSA $ to pay for medical, dental, & Rx or pay out of your pocket $ rolls over at the end of the year Pay a copay and/or deductible Lower deductible Lower out-of-pocket maximum * The IRS has certain requirements for who can enroll in an HSA. You cannot be: 1) eligible for other medical coverage (for example, through your spouse); 2) claimed on someone else s tax return or 3) enrolled in Medicare. If you aren t eligible to enroll in an HSA, you can still enroll in the BEK PPO Medical Plan.
4 GET HELP FROM PEOPLE LIKE YOU Check out these stories and see if they help you figure out which plan to choose. MEET COURTNEY I watch my pennies. My husband and I are empty-nesters. We both have health conditions and take regular medications. I don t like surprise expenses so I d rather pay more each paycheck than when an unexpected medical problem happens. ENROLLS IN: BEK PPO Medical Plan MEET WILSON Pay later. I m single and saving for my first house. I m pretty healthy and don t expect any health problems. I want to spend the least amount each paycheck and can use the Company contribution from BEK in case I get sick. ENROLLS IN: BEK HSA Medical Plan MEET JUSTIN My wife and I are expecting our first child. We re nervous about upcoming medical expenses. I really like the peace of mind that we ll have with the PPO Plan to cover maternity expenses and unexpected costs. ENROLLS IN: BEK PPO Medical Plan MEET FELIPE I m headed for retirement. I m watching my spending. I m saving every extra dollar I can. I take one or two maintenance medications and think my 2018 medical expenses will be close to the $1,000 contribution BEK will make to my HSA, which makes this plan a great choice for me. ENROLLS IN: BEK HSA Medical Plan
5 WORKING SPOUSE SURCHARGE If your spouse is currently working and eligible for medical coverage through their employer, you will pay a $100 monthly charge in addition to the medical costs per pay period. TOBACCO SURCHARGE You ll pay $100 more per month for Ben E. Keith medical coverage if you use tobacco in any form cigarettes, e-cigarettes, cigars, pipes, snuff or chewing tobacco. If you use tobacco but want to quit, call QuitLogix at If you are unable to participate in the tobacco cessation program because your doctor has determined that it would be medically inadvisable, Ben E. Keith will work with you to provide another alternative for you to avoid the $100 per month surcharge. ID CARDS Keeping the same medical plan? 6 COMPARE THE BEK MEDICAL PLANS PLAN FEATURES BEK HSA MEDICAL PLAN BEK PPO MEDICAL PLAN Company contribution The entire BEK contribution is available on January 1, 2018 Preventive care Includes annual checkups, immunizations and screenings Annual calendar year deductible is The amount you pay each calendar year for covered expenses. Copays don t count toward your deductible. Coinsurance is The amount you pay for eligible expenses after you meet your deductible. Out-of-pocket maximum is The most you ll pay each year for eligible health care expenses, including your deductibles and copays. For doctor office visits If you enroll in the HSA, you ll pay your deductible and coinsurance (except for a virtual doctor visit). If you enroll in the PPO, you pay a fixed dollar amount for each office visit Inpatient hospital stays Emergency room visit Urgent care visit Diabetic care 20% discount when you visit a Center of Excellence for kidney and dialysis treatment. Most other health care services (physical therapy, lab work, X-rays, etc.) Wellness programs Quit tobacco, lose weight and get healthy Non-Maintenance Prescription Drugs are Commonly used to treat conditions that are considered short-term in nature. Some examples include antibiotics, analgesics and antiseptics. Maintenance Prescription Drugs are Commonly used to treat conditions that are considered chronic or long-term. PLAN FEATURES Annual deductible BEK PPO DENTAL PLAN (available everywhere) BEK DMO DENTAL PLAN (available in Texas only) IN-NETWORK OR OUT-OF-NETWORK YOU PAY: $50 individual/$150 family Maximum benefit per calendar year Preventive services Exams, routine cleanings, X-rays Basic services Fillings, extractions, periodontics, oral surgery, root canals Major services Dentures, bridgework, crowns, implants Orthodontia benefit for children up to age 19 $2,000 per person $0 for 2 exams and cleanings each year 20% after deductible 50% after deductible 50% (up to a lifetime maximum benefit of $2,000) You must select a primary care dentist after you enroll by calling MetLife All care must be delivered or referred by your primary care dentist Pay a copay each time you receive care except for preventive services When you enroll in vision coverage, get regular exams and glasses or contacts every 12 months. 8 Unum unum.com or Financial protection when you need it. Ben E. Keith has enhanced these benefits to offer you financial protection. BEK BASIC LIFE/AD&D INSURANCE PLAN Great news! BEK is increasing the amount of Company-paid life/ad&d insurance for all fulltime employees from $25,000 to $50,000. You do not need to enroll, and there is no cost to you. Please take time to review your designated beneficiary(ies) and make any changes. BEK SUPPLEMENTAL LIFE/AD&D INSURANCE PLAN HIGHER COVERAGE AMOUNTS FOR 2018 Great news! BEK is increasing the amount of supplemental life/ad&d insurance that fulltime employees can purchase for themselves. If you d like your beneficiaries to receive a higher payment amount than the BEK-Paid life/ AD&D insurance provides, purchase additional life insurance of $50,000 to $950,000. WHAT YOU NEED TO KNOW Choose a coverage amount of $50,000 to $950,000 in $50,000 increments. If you enroll during 2018 Annual Enrollment, you are guaranteed coverage up to $300,000. Amounts over $300,000 will require Evidence of Insurability (EOI) and must be approved by Unum to be effective. Rates are now based on the age of the employee as of January 1, BEK SPOUSE AND CHILD LIFE/AD&D INSURANCE PLANS NEW OPTIONAL COVERAGE FOR 2018 While you and your family are dealing with the emotional burden that the loss of a family member brings, you can receive financial help through optional dependent life insurance coverage. SPOUSE LIFE/AD&D WHAT YOU NEED TO KNOW Choose a coverage amount of $50,000 to $250,000 in $50,000 increments. You must enroll in supplemental life/ad&d if you want to cover your spouse. Spouse coverage can be up to 50% of the employee life/ad&d amount, not to exceed $250,000. For instance, if an employee chooses $100,000, the spouse can enroll in $50,000. Amounts over $50,000 require Evidence of Insurability (EOI) and must be approved by Unum to be effective. Rates are based on the age of the employee as of January 1, CHILD LIFE/AD&D WHAT YOU NEED TO KNOW Each child receives $10,000 in life/ad&d coverage. Children up to age 26 can be covered. Be sure to list the names of each child during enrollment when requested; otherwise, they will not have coverage. Pay the same low rate of $1.50 a month no matter how many children you enroll.
9 Unum unum.com or NEW COVERAGE FOR 2018 WHAT YOU NEED TO KNOW STD is provided at no cost to you by Ben E. Keith Company. Coverage becomes available on January 1, You are automatically covered as a full-time employee no enrollment needed. Starting January 1, 2018, STD will be administered by Unum. Once approved by Unum, STD replaces a portion of your pay for the first 26 weeks of short-term injury or illness. After 26 weeks, LTD begins if approved by Unum. Look for a welcome kit in the mail at home from Unum soon with more information about STD including how to file a claim. HOW BEK STD WORKS FIRST 26 WEEKS OF APPROVED STD Replaces a portion of your pay AFTER 26 WEEKS BEK LTD begins if approved by Unum See the Decision Guide you received in the mail at home for details.
10 Unum unum.com or NEW COVERAGE FOR 2018 Great news! BEK is providing a Company-paid LTD plan to all full-time employees effective January 1, This benefit starts after your short-term disability benefits end. LTD makes sure you have some income replacement if you can t work for an extended period of time, due to a non-work-related illness or injury. WHAT YOU NEED TO KNOW LTD is provided at no cost to you by Ben E. Keith Company. Coverage becomes available on January 1, You are automatically covered as a full-time employee no enrollment needed. Once approved by Unum, LTD coverage begins after your 26th week of disability. LTD replaces a portion of your pay. This coverage is coordinated with other benefits you may receive while disabled, such as Social Security and Worker s Compensation. Coverage may continue until you reach your normal Social Security retirement age. Benefits are paid monthly. Look for a welcome kit in the mail at home from Unum soon with more information about LTD including how to file a claim. 12 RetiremenT SAvings PlaN Empower Retirement empowermyretirement.com or BEK-SAVE ( ) (k) A 401(k) allows you to contribute a percentage of your paycheck and receive matching contributions from Ben E. Keith. If you are a full-time employee, you are eligible to participate in the 401(k) on the first day of the month following 60 days of Company service. You can manage the investments of both your personal contributions and Company-matching contributions to your account. GET MATCHING DOLLARS FROM BEN E. INVEST YOUR WAY OR BASED ON YOUR RETIREMENT AGE Your contributions will be automatically invested in a Vanguard Target Date Fund based on your retirement age, but you can choose other activelyor passively-managed funds if you prefer. CATCH-UP CONTRIBUTIONS HELP YOU SAVE MORE Participants turning age 50 or older in 2018 may contribute an additional $6,000, for a total contribution amount of $24,500. YOU SAVE IT YOU KEEP IT You are always 100% vested in your personal contributions and any rollover contributions plus any earnings they generate. You will be 100% vested in Ben E. Keith s matching contributions after two years of Company service. ROLLOVERS ARE WELCOME Contributions from any eligible retirement plan and type (including Roth) can be rolled over into your 401(k) account. Consult with your financial advisor or contact Empower Retirement to learn more. PROFIT SHARING Ben E. Keith has contributed $40 million to the Profit Sharing plan during the last 10 years. Full-time employees with one year of Company service are eligible to receive a Profit Sharing contribution if Ben E. Keith makes a discretionary contribution. INVEST YOUR WAY OR BASED ON YOUR RETIREMENT AGE Your Profit Sharing account will be automatically invested in a Target Date Fund based on your retirement age or choose other activelyor passively-managed funds if you prefer.
