1 Requirement: Frequency: Projects for Assistance in Transition from Homelessness (PATH) Grant Contract 42 U.S.C. 290cc 21 et. seq. 42 C.F.R., Part 54 Annual Monitoring Annual Report Quarterly Report Due Date: Ongoing The Projects for Assistance in Transition from Homelessness (PATH) is a formula based, SAMHSA funded grant created to reduce or eliminate homelessness for individuals with serious mental illnesses or cooccuring substance use disorders who are homeless or at imminent risk of becoming homeless. The PATH Grant will be administered according to the Florida Department of Children and Families (DCF) guidance document, or the latest version thereof. COVERED COSTS: Allowable administrative and general program costs that are incurred under the Projects for Assistance in Transition from Homelessness (PATH) Grant. RECIPIENTS: Individuals with serious mental illness or serious mental illness and substance use disorders, and are homeless or at imminent risk of becoming homeless. AUTHORIZED SERVICES: Outreach; screening and diagnostic treatment; habilitation and rehabilitation; community mental health services; alcohol or drug treatment; staff training, including the training of individuals who work in shelters, mental health clinics, substance abuse programs, and other sites where homeless individuals require services; case management services; assistance in obtaining income support services, including housing assistance, food stamps, and supplemental security income benefits; referral for other appropriate services; representative payee services in accordance with Section 1631(a)(2) of the Social Security Act if the eligible homeless individual is receiving aid under Title XVI of such act and if the applicant is designated by the Secretary to provide such services; supportive and supervisory services in residential settings; referrals for primary health services, job training, and educational services; relevant housing services (subject to Section 522(h)(1)), including minor renovation, expansion, and repair of housing, planning of housing, technical assistance in applying for housing assistance, improving the coordination of housing services, security deposits, matching individuals with appropriate housing situations, and one time rental payments to prevent eviction. FUNDING: Costs associated with this BE/OCA combination are directly charged to the PATH Grant and require a 25%, or one third of the total fiscal year allocation, in kind or cash match. Network Service Providers are required to: 1) Partner with local resources to link people with safe, affordable housing of their choice. 2) Implement an approved and signed Local Intended Use Plan that demonstrates compliance with Title V, Part C, Section 522 of the Public Health Services Act. 2 3) Expend PATH local matching funds in the provision of PATH eligible services to PATH eligible persons. The expenditures must match the types of services outlined in the Local Intended Use Plan. The formula to be followed is cited in Title V, Part C, Section 524 of the Public Health Services Act (42 U.S.C. 290cc 21 et. Seq). 4) Allocate and expend no more than 25% of the PATH award for administrative and supervisory expenses; a minimum of 75% of PATH funding must be used for front line staff and direct services to eligible consumers. These allocations shall be outlined in the annual budget. 5) Participate in the peer based fidelity assessment process to assess the quality, appropriateness, and efficacy of treatment services provided to individuals under this contract pursuant to 45 CFR ) Get approval of the Intended Use Plan (IUP) from the local Continuum of Care (CoC) Governing Body in their area, prior to submission to the Managing Entity (include how the PATH services fit with the priorities of the local CoC plan). 7) Submit an annual PATH application Intended Use Plan (IUP) to the Managing Entity as requested in the format and per the instructions provided by the Managing Entity. Instructions for the application will be distributed each year, subject to change on a yearly basis. The IUP must establish PATH funded program priorities including, at a minimum: a. Targeting persons who are experiencing homelessness as a priority population and maximizing services to vulnerable adults who are literally and chronically homeless; b. Conducting street outreach and case management as priority services 8) Provide support services for individuals who have a serious mental illness or serious mental illness and substance use disorder, and are homeless or at imminent risk of becoming homeless. 9) Expend eligible PATH local matching funds in the provision of PATH eligible services to PATH eligible persons. The expenditures must match the types of services outlined in the Local Intended Use Plan. The formula to be followed is cited in Section 524 of the Public Health Services Act, as amended by Public Law ) Complete a program questionnaire by January of each year outlining agency implementation of PATH. 11) Utilize a Homeless Management Information System (HMIS) for documentation of services provided under PATH. 12) Be a member in good standing with their local Lead Agency Coalition or CoC. 13) Work with the Managing Entity staff and prioritize efforts to locate and house individuals identified as High Utilizers of the SAMH System. 14) Establish a service plan for all PATH enrolled individuals including: a. Goals to obtain community mental health services for the individual; b. Coordinating and obtaining needed services for the individual, including services relating to shelter, daily living activities, personal and benefits planning, transportation, habilitation and rehabilitation services, prevocational and employment services, and permanent housing; c. Assistance to obtain income and income support services, including housing assistance, Supplemental Nutrition Assistance Program (SNAP) benefits, and Supplemental Security Income/Social Security Disability Insurance (SSI/SSDI); d. Referrals to other appropriate services; and e. Review of the plan not less than once every three months. 15) Maintain individual client files containing an intake form, a determination of eligibility for PATHfunded services, a service plan, and progress notes for each person served with PATH funds. 3 Required Reports: 1) Monthly Reports a. The Network Service Providers must submit the Monthly LSF Homeless High Utilizer Search and Update List no later than the 10th of the month following the end of each quarter. The Template for this report is incorporated herein. 2) Annual Reports a. The Network Service Provider must submit annual reports into the PATH Data Exchange website or as directed by the Managing Entity. More information and guidelines for annual report submission may be located at: 3) Quarterly Reports a. The Network Service Provider must file a progress report to the Managing Entity on a Quarterly Basis. This report will include: i. Number of people housed during the Quarter ii. Names of people housed during the Quarter iii. SSN of people housed during the Quarter iv. Type of housing for each person housed during the Quarter v. Residence Prior Night to Enrollment for each person housed during the Quarter vi. Type of funding used to house the individual 1. This Quarterly Report is due to the Managing Entity no later than 5:00 pm on the 10th day of the month following the end of each quarter. The Template for this report is incorporated herein. b. The Network Service Providers must enter quarterly summary information about PATH programs and services into the PATH Data Exchange (PDX) at no later than the 10th of the month following the end of each quarter.
