23 HIPAA Special Enrollment Rights Notice If you are declining enrollment for yourself or your dependents (including your spouse) because of other health insurance coverage or group health plan coverage, you may be able to enroll yourself and your dependents in this plan if you or your dependents lose eligibility for that other coverage (or if the employer stops contributing towards you or your dependent s other coverage). However, you must request enrollment within 30 days after you or your dependent s other coverage ends (or after the employer stops contributing toward the other coverage). In addition, if you have a new dependent as a result of marriage, birth, adoption or placement for adoption, you may be able to enroll yourself and your dependents. However, you must request enrollment within 30 days after the marriage, birth, adoption or placement for adoption. Effective April 1, 2009, group health plans will recognize the following as additional special enrollment opportunities for employees and dependents: * Termination of Medicaid or Children s Health Insurance Program (CHIP) due to loss of eligibility. * Eligibility for state premium assistance under Medicaid or CHIP. However, you must request enrollment within 60 days after the date of termination of Medicaid or CHIP; or the date the employee or dependent is determined eligible for premium assistance under Medicaid or CHIP. Insurance Information The plan has contracted with Metropolitan Life Insurance Company and AFLAC Insurance Company to pay all claims incurred under the terms of the plan. The total premiums paid for the plan year ending May 31st 2017 were $460,047. Your Rights to Additional Information You have the right to receive a copy of the full annual report, or any part thereof, on request. The items listed below are included in that report: 1. Insurance information including sales commissions paid by insurance carriers To obtain a copy of the full annual report, or any part thereof, write or call the office of SPMI: MyHRProfessionals who is the plan administrator, at P.O.
