If so, there are a couple things you need to know about your prescription coverage. Medicare prescription drug coverage is available to everyone with Medicare and it provides a standard level of coverage set by Medicare. The LCPS prescription drug plan, through Express Scripts/Medco, provides coverage that exceeds the standard set by Medicare. This is called Creditable Coverage and means you will not pay a penalty if you elect a Medicare drug plan later during a special enrollment period. For the full Creditable Coverage disclosure please visit the Employee Health, Wellness and Benefits homepage for a link to the document. Health Insurance Plan Documents Summary of Benefits and Coverage (SBC) and Summary Plan Document (SPD) Looking for more detailed information regarding your coverage? The official coverage documents can be found on the Employee Health, Wellness and Benefits website under Health Insurance/Forms and Publications. The Summary of Benefits and Coverage (SBC) summarizes your coverage and provides practical cost examples. The Summary Plan document (SPD) is the official coverage document that details what is covered, how it is covered, exclusions and limitations. This document takes precedence over any summary information published by LCPS or the insurance providers.

4 Open Enrollment Benefit Changes P A G E 4 Changes for 2015 Important Notes About Your 2015 Benefits Health Insurance - LCPS is unbundling the health insurance plans for active employees. Currently, when you elect health insurance, you are enrolled in medical/prescription, dental, and vision coverage - and all dependents you choose to cover are also enrolled in each of these plans. Beginning with this year s Open Enrollment, you will be able to elect dental and vision separate from medical/prescription as well as the dependents that will be covered under each plan that you are enrolled in. We understand that most employees will want to maintain the same coverage so, if you do not want to make a change, you do not have to do anything. Your current election will automatically continue and you and all of your covered dependents will keep the same coverage. If you do want to make a change to the plans that you are enrolled in or the dependents covered under a particular plan, you will need to complete a Health Insurance Enrollment/Change form for These forms are available on the Employee Health, Wellness and Benefits website, by request by calling Employee Health, Wellness and Benefits and at Open Enrollment events. Prescription Insurance - Effective 9/15/14, compounded prescriptions will be undergoing additional clinical review if they fall into any of the following categories: 1. The medication represents a significant cost and/or is within the top 200 most expensive compounds 2. The medication is considered over the counter as defined by our formulary 3. The medication has commercially available alternatives 4. The medication has had significant and/or continuous price increases 5. The medication is comprised of products lacking clinical evidence of efficacy The U.S. Food and Drug Administration (FDA) defines a compounded medication as one that requires a licensed pharmacist to combine, mix or alter the ingredients of a medication when filling a prescription. The FDA does not verify the quality, safety and/or effectiveness of compounded medications and they do not undergo the rigorous clinical review process required of FDA approved products. If you ve been notified by Express Scripts that your prescription is under review, here s what you must do to avoid paying the full cost of your medication: If you ve never tried an FDA-approved, commercially available drug, ask your doctor if one may be right for you. If yes, ask for a new prescription. If you have tried an FDA-approved, commercially available drug and it didn t work, or your doctor decided it wasn t appropriate, ask your doctor to call Express Scripts to arrange for a review of your prescription. If you continue taking medication that is no longer covered or your doctor doesn t call and get pre-approval, you ll be responsible for the full cost of the medication. So, please, ask for a new prescription or have your doctor call Express Scripts at Flexible Spending Accounts (FSA) - We will no longer require a minimum dollar requirement to enroll in a Healthcare or Dependent Care Flexible Spending Account (historically, the minimum has been $300). You can elect any dollar amount you wish, not to exceed $2,500 for Healthcare and $5,000 for Dependent Care. AN ANNUAL ELECTION IS REQUIRED FOR FSA S. IF YOU WISH TO PARTICIPATE IN 2015, YOU MUST ENROLL; YOUR ELECTION WILL NOT CARRY FORWARD AS IT DOES WITH HEALTH INSURANCE.

5 Open Enrollment Benefit Changes P A G E 5 Changes for 2015 What else do you need to know about 2015? Health Insurance Plan Design We have developed health insurance rates for and have presented them by benefit on the following page. Please pay close attention to our s and other communications as we may be announcing additional changes by the end of October. Marriage Equality in the Commonwealth of Virginia Effective October 6, 2014, same-sex marriage became legal in Virginia. As a result, Loudoun County Public Schools (LCPS) will begin offering benefits to the same-sex spouse of any employee to whom they are legally married. LCPS requires documentation demonstrating all insured dependents meet eligibility criteria. Employees who are able to provide proof of same-sex marriage prior to October 6, 2014 will have 30 calendar days from October 6, 2014 (the qualifying event date) to submit enrollment forms and applicable documentation. Coverage would become effective the first of the month following the timely receipt of this paperwork in the Employee Health, Wellness and Benefits office. Employees marrying after October 6, 2014 will have 30 days from the date of their marriage to submit the required forms and documentation. Forms and guidance on adding a dependent can be found on the Employee Health, Wellness and Benefits website. Out-of-Network Health Insurance Benefits Out-of-Network WILL cost you more. Through the LCPS health insurance program you may access providers who do not partner with Cigna, however, YOU will pay more to do so. When using your out-ofnetwork benefits, you will be responsible for paying the provider for the services and then submitting your own claim to Cigna. Also, if you are choosing to access out-of-network benefits, this could result in providers passing along additional charges to you through balance billing (a provider s way of collecting amounts not reimbursed by Cigna). IT IS IMPORTANT TO NOTE THAT OUT-OF-NETWORK BENEFITS WILL COST YOU MORE IN EVERY SCENARIO. Cigna s network is a full complement of providers for all levels of care. If you are currently accessing out-of-network benefits, you may want to revisit your choice.

6 2015 Premium Chart - Active Employees January 1, December 31, 2015 The rates for Medical/Prescription, Dental and Vision coverage are listed separately. If you keep your same coverage from 2014 (Medical/Prescription, Dental and Vision) you must add the three costs together to determine your total monthly health insurance cost. See the example to the right. Employee Only who works 30+ hours per week and enrolls in OAP coverage with Dental and Vision. 7 Flexible Spending Accounts Healthcare and Dependent Care Flexible Spending Accounts P A G E 7 Flexible Spending Accounts What is a Flexible Spending Account (FSA)? Our Flexible Spending Account program consists of two accounts, a Healthcare Expense Reimbursement Account and a Dependent Care Expense Reimbursement Account. For both accounts, money deducted from your pay is not subject to payroll taxes; and the end result is that the amount LCPS reports as taxable income on your W-2 is less than if you did not participate in an FSA. Compusys/Erisa Group, Inc. is the plan administrator for LCPS. The Healthcare FSA allows you to put aside up to $2,500 per calendar year (beginning January 1), pre-tax, to be reimbursed for eligible healthcare expenses not reimbursed under your family s health care plan. Premiums are NOT eligible expenses. The Dependent Care FSA allows you to put aside up to $5,000 per household, per calendar year, pre-tax, to be reimbursed for daycare expenses for your child(ren) under age 13 and/or other qualified dependents. How it Works: Determine how much money you wish to contribute based on your estimates of health and/or dependent care expenses for the upcoming calendar year. This is your annual election. Your annual election will be divided evenly over your pays in that plan year. This is deducted on a pre-tax basis and contributed to your healthcare and/or dependent care account. When you have eligible expenses to be reimbursed, complete a paper claim form or the online claim form and submit appropriate receipts to Compusys/Erisa. You will be reimbursed for eligible expenses up to the maximum you elected for the healthcare reimbursement account for the plan year. You will only be reimbursed up to your account balance for the dependent care account. Reimbursements can be issued via a check mailed to your home or by direct deposit if banking information is provided to Compusys/Erisa. All employees enrolled in the healthcare reimbursement account will receive a Flex Debit Card which they can use to pay for medical services directly from their flex account. In almost all cases, documentation substantiating the expense must still be provided to Compusys/Erisa afterwards.

Regardless of life phase we all experience stress and life difficulties from time to time. This workshop will review the resources available through this program that can help manage stress leading to a greater well-being. 1:00 pm 1:30 pm Basic and Optional Life Insurance Plans Do you know how much basic life insurance you have with LCPS? Did you know that the basic life insurance plan consists of accidental death and dismemberment benefits, a safety belt, felonious assault and repatriation benefits? Did you know that you may be able to purchase additional life insurance on your spouse or children, as well as yourself? 1:30 pm 2:00 pm LCPS Disability Plans Short-term and Long-term Disability insurance protects one's income in the case that he or she becomes disabled. Although you may have enough money in the bank to meet your short-term needs, what would happen if you were unable to work for months, or even years? Just like life insurance, disability insurance is protection for your future income. If you are involved in a serious accident or develop a disabling disease, how will you continue to support yourself, let alone your dependents? The bottom line: If you depend on your income to pay the bills, you should consider disability insurance. During Open Enrollment, you ll have opportunity to learn about and enroll in: LCPS Disability Plans Family Sick Leave Bank Optional Life Insurance DONATED FAMILY SICK LEAVE BANK The Donated Family Sick Leave Bank provides paid emergency sick leave of up to 30 days to eligible employees when their spouse, child or parent has suffered a serious illness or injury. Enrollment is October 15 to November 15 each year.

9 Aflac & Lincoln Financial Group 403(b) P A G E 9 Open Enrollment for 2015 Aflac Cancer Income Protection Plan This plan is designed to provide the cash necessary to get the best care possible while helping to cover the ever-increasing costs of cancer treatment, but more importantly, the non-medical costs. These may include the normal household bills, childcare, travel, lodging and loss of income for the extended period of time for which you (or a loved one helping to provide care) may be unable to work. To enroll in Aflac you must contact Debbie Smith, the LCPS Aflac Agent, during the Open Enrollment period of October 15 - November 15. 10 Wellness Works! Updates P A G E 10 Important Information The Wellness Works! Passport must be completed by December 31, 2014 and returned to Employee Health, Wellness and Benefits no later than January 31, Did you know that Loudoun County Public Schools will reward you for being aware of your physical, emotional and financial health? First, you need a Wellness Works! Passport. If you don t have one, ask your worksite Wellness Ambassador, print one from our website or wellness@lcps.org to request one. Next, complete the requirements listed on Page 22 of the Wellness Handbook by December 31, 2014 (turn your passport in early so you don t forget during the holiday season!). For your convenience, you have until January 31, 2015 to turn in your completed passport, however, all requirements must be completed between the dates of January 1, December 31, Biometric Information You will NOT be awarded $120 if you fail to provide biometric information (blood pressure, cholesterol, waist measurement, height and weight) in your health assessment. If you mark unsure you will get a completed note at the end of the assessment - BUT THIS DOES NOT QUALIFY YOU FOR THE REWARD. Please take the assessment again if you are in doubt. If you ve previously submitted the information you can simply click through the information. If you did not, you will have the ability to update from unsure. Our reports indicate that MANY OF YOU are completing the health assessment - but not including biometric data. The data is critical to ensuring that we are aware of and meeting the needs of our employees relating to wellness initiatives and disease management programs, and it is mandatory this year in order to receive the reward. When do I get my $120 reward? If you complete all required steps before December 31, 2014, and your submission is received by Employee Health, Wellness and Benefits by January 31, 2015, you will receive your reward in your payroll direct deposit account on or before March 30, 2015, but no earlier than February 1, This option is taxable and the reward will be included on your W-2.

11 Emergency Room Helpful Hints P A G E 11 Emergency Room Information your Emergency Room alternatives Know before you go! When you need care right away, you have choices. In a health emergency, always dial 911 or go to the nearest emergency room. But when your health problem is not an emergency, you can get the care you need and save time and money. Simply see your doctor or go to a convenience care clinic or urgent care center. These facilities typically all have: Shorter wait times Convenient hours Lower costs Walk-in office visits Facts to consider when deciding where to get care: Your doctors know you best. They may have evening or weekend office hours. Make an appointment or ask them about your care choices. Your copay is $15 for a Primary Care Physician or $30 for a Specialist under both the OAP and POS plans. Convenience Care Clinics are located in pharmacies, grocery stores and retail stores. Their nurses or assistants treat common conditions like earaches, sore throats and the flu. Your copay is $15 under both the OAP and POS plans at a Convenience Care Clinic. Urgent Care Centers are most like emergency rooms. Their doctors, assistants and nurses treat less critical conditions such as sprained ankles, cuts, fevers and other conditions that need care right away. Your copay is $50 for the OAP plan and $25 for the POS plan at an Urgent Care center. Emergency Room copay is $100 for both the OAP and POS plans. Our utilization stats show that ER usage by LCPS employees is above the normal benchmark. Based on diagnosis and access to care, 19% of these visits could have been treated effectively at the lower costs facilities mentioned above. You can expect that this co-pay will increase significantly in coming years if we don t all work together to make more cost efficient choices.
