Please replace with the attached Addendum No. 1 Attachment C Fee Submittal. The PCCP in the fee table for PCCP Transmission Main Condition Assessment stands for Prestressed Cylinder Concrete Pipe.

2 Attachment C Fee Submittal

3 The Fee submittal shall consist of the fee tables and all required forms described in Attachment B. The Fee submittal shall be submitted in a sealed envelope separate from the qualifications proposal. There is no limit to the number of pages submitted as part of the Fee submittal. The proposer must submit an original and nine copies with it. 1. Fee Submit hourly rates and multipliers by discipline as listed in the attached Fee Table. (Overtime shall be treated as straight time, unless required otherwise by State law.) Compensation for labor shall be based NOT on the actual salary rates, but on category rates and multipliers quoted by the proposer in the Fee Table. Reimbursable items will be negotiated on a per Task basis. A fixed reimbursable is included in the Fee Table to allow for other direct costs, which are reimbursed without a markup on submission of receipts with the consultant s monthly invoice. Fee Table Instructions The columns for initial Average Hourly Rate (Column B) and Average Multiplier (Column C), on Fee Table are average values. The City will pay the EP for hours expended by using these average rates and multipliers, regardless of what each person is actually paid by the firm or what each firm s multiplier actually is. Proposer shall fill in Column B and C with rates and multipliers it is willing to accept as payment for services performed by any proposer team member. Proposer shall complete only one such form for its entire team, and not submit separate forms for different subconsultant firms. An allowance for reimbursables for Water Mains Renewal Program contract has been fixed by the City at $150, Proposers are not to submit a different proposal for reimbursables, language anywhere else in this RFP notwithstanding.

4 NOTE: The hours indicated in the table are the CWD s best estimate of hours for one contract. The hours are for proposal purposes only and do not necessarily reflect the actual hours or mix of hours that will be used. CWD will allow fee rate increases once every year, after the completion of the first year. The rate increase shall be limited to the CPI (Consumer Price Index) for that calendar year. No increase shall be allowed in the multiplier. 2. Office of Equal Opportunity (OEO) Forms and Other Required City Forms These are attached and explained in Attachment B.
