3 A. Project Overview Bridgewalk Apartments Renovation 1. Project Overview Boulder Housing Partners (BHP) seeks the services of one or possibly two General Contractor(s) to provide comprehensive renovation services for the renovation of the Bridgewalk Apartments, located at Walden Circle in South Boulder. Bridgewalk is comprised of and 2 bedroom units and a total of 21 buildings. There are 4 unit types. 2. Scope of Work Bid Package One: Exterior Envelope 1) Re-roof 19 buildings. 2) Remove and replace all siding on the site with cementitious siding product. 3) Remove and replace all windows. 4) Site work including installation of bike racks and new lighting Bid Package Two: Interior Renovation Work 5) Install new HVAC and central air conditioning in all units. 6) Electrical panel replacement 7) Renovate 110 unit interiors including some combination of new flooring, new paint, new kitchens, new bathrooms, new lighting. BHP has been replacing floors and painting unit interiors upon vacancy during the summer months. Some unit renovations will require only kitchen and bathroom renovations. A final count will be provided at the time of hard bids. Contractors may bid either Bid Package One: Exterior Envelope Work only, Bid Package Two: Interior Work only, or both Bid Packages. 3

5 C. Proposal Submission Details 1. General Submit Statement of Interest and Qualifications no later than 3:00 pm on June 1, 2011 to the following address: Boulder Housing Partners 4800 Broadway Boulder, CO Attention: Laura Sheinbaum Submit 3 hard copies (at least one unbound copy) of the Statement of Interest and Qualifications and one identical electronic copy. Limit the content of the Statement of Interest and Qualifications to ten (10) pages maximum, exclusing the AIA A305 Contractor s Qualifcation Statement portion of the submittal. Proposals will be evaluated and scored by a BHP team according to criteria outlined later in this document. Please refer questions to Laura Sheinbaum via at SheinbaumL@boulderhousingpartners.org. No phone calls please. BHP reserves the right to reject any or all proposals and to waive any irregularities deemed to be in the best interest of the organization and to be the sole judge of qualifications, criteria and final selection of Pre-Qualified bidders. It is the intent that the prequalified GCs will be notified on or around June 7, Pre-qualified bidders will be notified when 100% Construction Drawings are available (currently scheduled for June 14, 2011) and asked to submit bids. 2. Evaluation Criteria Contractors who, as Boulder Housing Partners may determine, at their sole discretion, receive a yes on all the criteria listed below will be pre-qualified to bid the Bridgewalk Renovation Project. Complete and submit the attached AIA Document A305, General Contractor s Pre- Qualification Statement 1986 Edition. If you are not already a licensed AIA software user, purchase this document here for $ Additional Requirements for AIA Document A305: Under Article 4, Section 4.3, please also include the aggregate bonding capacity of your firm for past two years, as well as a letter from your insurance carrier stating your firm, as of the date of the letter, has the available, unused bonding capacity to meet the minimum bonding requirements identified in the Evaluation Criteria, depending on your firm s desired Bid Packages. In response to Section 5.1, please include a financial statement for the first quarter of 2011, as well as for the full 2010 year. 3. Previous Multi-Family Experience 1. Provide a list of previous projects completed by the GC within the past 5 years. Include the name of the project, Project Manager, Site Superintendent, completion date, owners s contact name and phone number, and dollar value of the project. Preference is given to OCCUPIED multi-family renovation projects including both the proposed Project Manager and Site Superintendent. General Construction Experience 1. Provide a list of projects in excess of $500,000 completed by the GC Management Team. Include name of Project Manager and Site Superintendent. Include name of project, completion date, and dollar value of the project. Include reference contact information for projects completed within the past five years including names and phone numbers. Management Plan 1. Provide organization chart and information regarding GC s proposed management team. Include resumes and job descriptions of key personnel such as Project Manager, Quality Control, and Site Superintendent. Include description of other projects the key personnel are scheduled to participate in from July 2011 through February Provide information regarding your firm s proposed management plan. Identify potential risk and mitigation approaches. 3. Provide information demonstrating the ability to control sub-contractors, specifically addressing maintaining construction schedules, controlling and faily pricing change 7

8 orders; and promptly responding to site call backs during start-up and warranty periods. 4. Describe your approach to closing out a project. Safety Plan and Experience 1. Submit a copy of your firm s safety protocol and the name of the person who oversees your safety program. 2. Submit your firm s Interstate Experience Modification Rate for Workmen s Compensation Insurance used by your insurance carrier for each of the last three years. E. About BHP BHP is the provider largest provider of affordable housing in city of Boulder. Please refer to our website at for more details on the organization. F. General Information 1. Proposal Rejection: BHP reserves the right to reject any and all proposals and waive any irregularities therein if it is found to be in the best interest of BHP. Proposals not received by the required deadline are ineligible for consideration and will not be opened, however BHP may change the deadline at any time. 2. Contract and Method of Payment: BHP s preferred form of construction contract is attached to this document. Funding for this project is provided by refinance proceeds available August 1, Limitation of Liability: BHP assumes no liability for costs incurred by candidates in responding to the Request for Statement of Interest and Qualifications. All submittals become a matter of public record upon submission. 4. Insurance Requirements: Insurance requirements are attached. 5. Davis/Bacon Wages: Davis/Bacon wages will not apply this project. 9 Attachment #1: Insurance Requirements A. General Contractor s Liability Insurance: The General Contractor shall purchase from and maintain in a company or companies lawfully authorized to do business in the jurisdiction in which the Project is located such insurance as will protect the General Contractor from claims set forth below which may arise out of or result from the General Contractor s operations for which the General Contractor may be legally liable. The insurance required shall be written for not less than the limits of liability specified below or required by law, whichever coverage is greater. Coverages, whether written on an occurrence or claims-made basis, shall be maintained without interruption from date of commencement of operations until date of final payment and termination of any coverage required to be maintained after final payment. General Contractor shall submit a certificate of insurance and also any notices of renewal of the policies as they occur. All policies of insurance shall be written on a per occurrence basis unless otherwise agreed in writing by Boulder Housing Partners (BHP). The Insurance policies shall be written on a claims made basis. The Insurance policies shall remain in effect for the duration of the Project and for at least three years beyond the completion and acceptance of the Work. B. Certificates of insurance and/or insurance policies required shall be subject to the following stipulations and additional requirements; 1. The clause entitled Other Insurance Provisions contained in any policy including BHP as an additional insured shall not apply to BHP. 2. Any and all deductions contained in any insurance policy shall be assumed by and at the sole risk of the General Contractor. 3. If any of the said policies shall fail at any time to meet these requirements as to form or substance, or if a company issuing any such policy shall be or at any time cease to be approved by the Division of Insurance of the State of Colorado, or be or cease to be in compliance with any stricter requirements, the General Contractor shall promptly obtain a new policy, submit the same to BHP for approval if requested, and submit a new certificate of insurance. Upon failure of the General Contractor to furnish, deliver and maintain such insurance as provided herein, in the sole discretion of BHP, the Contractual Agreement may be immediately declared suspended, discontinued or terminated. Failure of the General Contractor in obtaining and/or maintaining any required insurance shall not relieve the General Contractor from any liability, nor shall the insurance requirements be construed to conflict with the obligations of the General Contractor concerning indemnification; 4. Receipt, review or acceptance by BHP of any insurance policies or certificates of insurance required, shall not be construed as a waiver or relieve the General Contractor from its obligation to meet the insurance requirements. 9

10 5. All requisite insurance shall be obtained from financial responsible insurance companies, authorized to do business in the State of Colorado and acceptable to BHP; 6. Insurance shall not be voided by reason of the General Contractor s waiving rights. C. Other insurance requirements for Boulder Housing Partners and General Contractor are: 7. BHP shall be named as an additional insured on all Commercial General Liability policies. 8. The General Contractor s Commercial General Liability Insurance shall include premises operations (including explosion, collapse and underground coverage), independent contractors, completed operations, and blanket contractual liability on all written contracts, all including broad form property damage coverages. D. The General Contractor s Commercial General Insurance Liability and Automobile Liability Insurance, Workers Compensation Insurance and Professional Liability Insurance shall be written for not less than limits of liability as follows: 1. Comprehensive General Liability a. Bodily Injury $1,000,000 Each Occurrence $2,000,000 Aggregate (Completed Operations) b. Property Damage $1,000,000 Each Occurrence $2,000,000 Aggregate 2. Comprehensive Automobile Liability a. Bodily Injury $1,000,000 Each Occurrence b. Property Damage $1,000,000 Each Occurrence 3. Worker s Compensation Insurance and Employers Liability a. Bodily Injury $100,000 Each Occurrence b. Employers Liability Insurance 4. Umbrella Insurance a. Each Occurrence $1,000,000 10
