2 What is Crusader Club Crusader Club (CC) is a before and after school program providing Mission Viejo Christian School students a positive and engaging extension of their school experience. Before and after school activities are provided in a fun, structured environment and facilitated by loving Christian staff. Safety, in a caring environment, is the main priority! Crusader Club staff serve as positive Christian role models in the children s lives. Program Hours Before School: 7:00-8:00am & 7:00-9:30am on late start school days Students checked into Crusader Club must remain until being walked to the playground at 7:45am After School: 3:00-6:00pm Students NOT picked up from school by 3:15pm will be checked into Crusader Club Half Days: 12:30-6:00pm Note: CC is open full days during most school breaks. Special sign-ups are required. Sign In/Out Procedures CRUSADER CLUB Before and After School Care Crusader Club is located in the gated building adjacent to the parking lot. Students will be escorted by their teachers to the building and will be signed in by Crusader Club staff. Students MUST be signed out by a parent or authorized adult every day. If you do not sign your child out with the time of pick up, you may be charged until 6:00pm. Crusader Club Daily Schedule Before School: 7:00-8:00am Free Choice Activities (Board Games, Reading, Art, Puzzles, Homework, etc.) After School: 3:00-3:45pm Sign in, outside play, and snack provided. 3:45-4:00pm Devotions: The children are grouped according to age level and a portion of scripture is read and they have a discussion of its meaning and application. 4:00-5:00pm Mandatory Homework Time for all ages: Students are required to work on homework for at least half minutes. Crusader Club staff will be available for homework assistance. If your child has no homework, they are expected to read during this time. 5:00-6:00pm Game/Activity Time: Students may choose from a variety of different enrichment activities, including but not limited to: Outside Games, Inside Games, Crafts, and Gardening. Special Events Special events are planned throughout the year for the children to participate in. Some holiday events include Seasonal Crafts, St. Patrick s Day Leprechaun Hunt, Easter Egg Hunt, Occasional BBQs, etc.

3 MVCS Crusader Club Schedule of Fees 2018/2019 School Year REGISTRATION FEE: $25.00 per child or maximum $50.00 per family. Students must register each year. MONTHLY PLAN OPTIONS: Monthly plans, i.e. Before School Care, After School Care, or Before & After School Care, are available and charged a flat monthly rate. November, December, April, and June are charged a reduced rate due to the shortened month (see * below). The monthly rate remains the same regardless of minimum days or absences. These plans are cost effective to families who use Crusader Club on a maximum daily basis. Monthly plans can only be paid by credit card or FACTS and are due & payable on the 1 st of each month. Your preferred method of payment will be automatically charged. Please note, holidays & non-school days are not included in any monthly and/or hourly plan but will be charged the holiday rate of $35.00 per day. *November: Rate adjusted based on 1-week vacation. December: Rate adjusted based on 2-week vacation. April: Rate adjusted based on 1-week vacation. June: Rate adjusted based on 1-week school attendance. MONTHLY PLAN OPTIONS (For Registered Students Only) Before School Care 7:00-8:00am $90/$40/$15 per month** After School Care 3:00-6:00pm $210/$160/$135 per month** Before & After School Care: 7:00-8:00am, 3:00-6:00pm $240/$190/$165 per month** **Sibling Discounts: Families with registered students, on monthly plans, will receive a discount for multiple children, as follows: $50.00 discount for 2nd child and $75.00 discount for each additional child. HOURLY RATE OPTION: For both registered and non-registered students, hours charged are calculated and determined at the end of each month by our Business Office. When calculating your child s hours, a 15- minute grace period from 3:00 to 3:15pm only, will be honored. If you go over any additional hour, for any length of time, you are charged a full hour. If a child arrives prior to 8:00am, there will be a 1hour charge. HOURLY RATE OPTIONS Registered Students Non-Registered Students $7.00 PER HOUR $10.00 PER HOUR OVERTIME: Any student(s) picked up from Crusader Club after 6:00pm will be charged $1.00 per each additional minute. WITHDRAWAL: Should you desire to withdraw your student from a payment plan, please notify the Crusader Club Coordinator or the Business Office at celston@mvcschool.org.

5 MVCS CRUSADER CLUB AUTOMATIC PAYMENT PLAN Automatic credit card/debit card or FACTS account payments, for MVCS Crusader Club, are recommended. To activate the automatic payment plan, please complete the form below. AUTOMATIC PAYMENT PLAN TERMS AND CONDITION FOR CREDIT CARDS / DEBIT CARDS: I give permission to MVCS to automatically charge the amount due, to my authorized credit/debit card or FACTS account, unless my written withdrawal or change has been received by MVCS Crusader Club prior to the payment date. If my credit/debit card is declined or does not go through, and MVCS is unable to process the charge, the MVCS Business Office will contact me and give me the opportunity to make another payment option. For payment changes, I will contact Chris Elston, celston@mvcschool.org or x 142, in the Business Office. I understand that this authorization will remain in effect until I cancel it in writing. I agree to notify MVCS in writing of any changes in my account information or termination of this authorization. I certify that I am an authorized user of this credit/debit card and/or FACTS account, and I will not dispute these scheduled transactions with my credit card company, my bank, or FACTS; so long as the transactions correspond to the terms indicated in this authorization form. 6 Medical Release and Waiver of Liability Children Name(s): As consideration for being permitted by the Mission Viejo Christian School to participate in Crusader Club, I hereby release and hold harmless Mission Viejo Christian School and Crusader Club staff from all liability, and from all actions or claims that I or my child now or hereafter have for damage or injury to my child, or to property, resulting from my child s participation in the program. I further agree that this waiver, release and assumption of risks are to be binding on the undersigned. I further agree to accept any cost or expense which they may incur, as a result of any injury and/or property damage that I or my child may cause or sustain while participating in the program. In case of a medical emergency, I hereby give permission to Mission Viejo Christian School personnel to order treatment for my child, including any necessary medical treatment and x-rays. I also hereby give permission to Mission Viejo Christian School to disclose the information contained on the Emergency Medical Form to medical personnel. I understand that an attempt will be made to reach me by phone when an emergency occurs. I agree to pay all medical, hospital, or other expenses which my child or I may incur, as a result of such treatment. Mission Viejo Christian School does not disclose your non-public personal medical or financial information, except as required by law. Mission Viejo Christian School Accident Insurance acts as secondary coverage to parents primary medical insurance. I HAVE CAREFULLY READ THIS RELEASE AND FULLY UNDERSTAND ITS CONTENTS.
