1 UOB JUNE HOLIDAY FREE TRAVEL INSURANCE CAMPAIGN Terms and Conditions 1. Campaign 1.01 The Campaign is open to all UOB Cardmembers who comply with the Terms and Conditions Notwithstanding anything herein to the contrary, the Bank has the discretion at any time and from time to time to determine the eligibility and/or ineligibility of any UOB Cardmember for the Campaign and shall not be obliged to make any payment or compensation whatsoever to UOB Cardmembers rendered ineligible for participation in the Campaign nor provide any explanation for such ineligibility. 2. Registration and Participation in the Campaign 2.01 (a) An Eligible Cardmember will be entitled to the Free Travel Insurance if he/she complies with all of the following:- (i) the Eligible Cardmember must have successfully registered himself/herself for the Promotion by completing one of the following Registration Process during the Registration Period:- (1) Register via SMS in the following format a. For Single Trip Plan policy - SMS <INS>space<NRIC>space<Departure date in DDMM>space<NAME>to (2) Or, register via the Registration Site uob.com.sg/freeinsurance (3) All registrations must be submitted during the Registration Period between 21 May 2014 (0001hr) and 30 June 2014 (2359hr) or at least one (1) day before the date of your departure whichever is earlier for the insurance to be effective. (4) Successful registration via SMS will receive a confirmation SMS. (5) Successful registration via the Registration Site will be prompted with a pop up message for the Eligible Cardmember (ii) the Eligible Cardmember must charged a Qualifying Amount to the Eligible Card account of the Eligible Cardmember and/or the account of the applicable supplemental Eligible Card during the Campaign Period. 1

2 (b) (c) The Eligible Cardmember under whom any number of supplementary Eligible Cards are in use can utilise all sums charged on the said supplementary Eligible Cards towards making up the Qualifying Amount. Upon successful registration, the Eligible Cardmember will be covered with a UniTravel Single Trip Plan. The cost of the premium S$100 will be charged to the Eligible Cardmember if he/she fails to meet the Qualifying Amount of S$1,000 in foreign currency to his/her Eligible Cards. (d) (f) In the event that the Eligible Cardmember meets the Qualifying Amount of S$5,000 in foreign currency, he/she will receive a link via SMS to download the UniTravel Annual Plan certificate of insurance by 30 July Notwithstanding the fact that the Eligible Cardmember has already completed the Registration Process and furnished all required information, the Eligible Cardmember will not be entitled to the FREE Travel Insurance if he/she does not comply with any of the Terms and Conditions By participating via SMS registration, the UOB Cardmember consents to the disclosure of information including, but not limited to, the UOB Cardmembers credit/debit card number to United Overseas Insurance Limited, independent telecommunication authorities or service provider or such other third party which is engaged by the Bank for the Promotion. The UOB Cardmember authorises the Bank to disclose any information regarding the UOB Cardmember and the UOB Cardmember s relevant Eligible Card account to such parties for the purposes of the Promotion Voluntary cancellation or termination of supplementary Eligible Card account(s) will not by itself disqualify an otherwise Eligible Cardmember from the Promotion. 3. Free Travel Insurance 3.01 Each Eligible Cardmember shall be entitled to ONE Free Travel Insurance policy irrespective of the number of Eligible Cards that the Eligible Cardmember may have or the number of times he/she may have carried out the Registration Process for his/her Eligible Cards Eligible Cardmember must travel in the Trip along with or without his/her Family Members for UniTravel Single Trip Plan to be effective The Free Travel Insurance shall cover Eligible Cardmember and/or his Family Members The Free Travel Insurance; UniTravel Single Trip Plan policy shall only cover for one Trip that commences and ends within the Campaign Period 22 May 2014 to 15 July Each Trip should not exceed thirty (30) days. 2

3 3.05 The Free Travel Insurance; UniTravel Annual Plan shall cover for unlimited number of Trips, commencing from 22 May 2014 to 21 May 2015, regardless of the date of SMS registration. Each Trip should not exceed thirty (30) days Once Eligible Cardmember has registered for the Campaign, the Free Travel Insurance is deemed to be effective and no cancellation of the Free Travel Insurance is allowed Coverage and benefit limits of the Free Travel Insurance is as shown in appendix A The Free Travel Insurance is provided by the United Overseas Insurance Limited ( UOI ) In the event of a claim, Eligible Cardmember consents to disclose or provide relevant information or document(s) to UOI and its service providers The Free Travel Insurance is not transferable or exchangeable for cash, credits, other gifts or otherwise in full or in part or refundable. The Bank assumes no liability or responsibility and shall not be responsible or liable for any injury, loss, claim or damage whatsoever or for any charges, costs or expenses of any kind whatsoever suffered or incurred as a result of the redemption or usage of the goods and services or the Free Travel Insurance or in connection with the Campaign and/or redemption or use of the Free Travel Insurance and any consequences including, but not limited to, loss of life, injury to person and/or loss or damage to property arising from or in connection with the Promotion, and/or use of the Free Travel Insurance howsoever arising Eligible Cardmembers can download the UniTravel Single Trip certificate of insurance issued in conjunction with the master policy in the name of United Overseas Bank from the Registration Site. 4. Qualifying Transactions 4.01 (a) Subject to Clause 4.01(b) and Clause 4.01(c) below, Qualifying Transactions includes all types of Retail Purchases made in foreign currency (non- Singapore dollar transactions) but shall exclude all the Excluded Transactions as set out under Clause 4.02 below. (b) For non-singapore dollar transactions charged to the Eligible Card for Retail Purchases made during the Campaign Period, the amount reflected in the account of the Eligible Card of the Eligible Cardmember (which is inclusive of the exchange rate conversion and commission, if any) in connection with such non-singapore dollar transactions will be considered as the Qualifying Transaction and taken into consideration when calculating the Qualifying Amount Excluded Transactions are: (a) balance transfers to the Eligible Card, cash advances from the Eligible Card, purchases via NETS and ongoing installment payments; 3

4 (b) (c) (d) any amount charged to the Eligible Card during the Campaign Period that is subsequently cancelled, voided or reversed; balances owing on the Eligible Card accounts from months that do not fall within the Campaign Period; and any credit/debit card transaction(s) carried out in respect of installment payments arranged prior to the Campaign Period, cash advances, balance and/or funds transfer, SmartPay, annual fees, cash advance fees, late fees, reversals, interest, any finance charges made by the Cardmember using the Eligible Card account and/or any transaction that was subsequently cancelled, voided or reversed for any reason The Bank reserves the right, at any time at its discretion, to amend or vary the list of Excluded Transactions set out under Clause 4.02 above without any reasons, prior notification and/or assuming any liability to any party and shall not be liable to pay any compensation or enter into any correspondence in connection with the same. All Eligible Cardmembers shall be bound by these amendments or variations All Qualifying Transactions must be charged to the account of the Eligible Card of the Eligible Cardmember. The Bank is not responsible for any failure or delay in the transmission of the transactions by any party including but not limited to acquiring merchants, merchant establishments, or any telecommunication provider The Bank reserves the right to determine at its discretion whether: (a) (b) (c) an Eligible Card is valid; an Eligible Cardmember has met all the requirements of the Promotion; and charges made to the Eligible Card qualify as Qualifying Transactions for the purposes of the Promotion If a Qualifying Transaction is cancelled or reversed after the Campaign Period is over and the total amount spent during the Campaign Period falls short of the Qualifying Amount, the Eligible Cardmember will not be considered to have spent the Qualifying Amount If the Eligible Cardmember fails to comply with Clause 2.01(a)(ii) above, the premium cost of the registered Free Travel Insurance; UniTravel Single Trip Plan policy or UniTravel Annual Plan policy as shown in Appendix A; will be charged to the Eligible Card account of the Eligible 5. General 5.01 (a) The Merchant is not a participant in or sponsor of the Promotion. The Merchant of the goods or services may impose conditions for redemption and use of the goods or services or the Free Travel Insurance offered in the Promotion. 4

5 (b) The Bank is neither the agent nor the principal of the Merchant and makes no representation as to the quality, condition or performance of the goods and services or the Free Travel Insurance provided. Any dispute about quality, condition, service standard or performance of the goods or services or the Free Travel Insurance is to be resolved directly between the Eligible Cardmember and the Merchant (a) The Bank shall not be liable if it is unable to perform its obligations under the Terms and Conditions, due directly or indirectly to the failure of the vendor, the telecommunication authorities or service provider or such other third party which maybe engaged for the Promotion, any machine or communication system, industrial dispute, war, Act of God, or anything outside the control of the Bank. (b) The Bank shall not be responsible or liable:- (i) for any failure or delay in the transmission or receipt of:- (1) evidence of sale transactions by MasterCard/Visa/Unionpay/JCB/CUP/American Express acquiring merchants, merchant establishments, VISA International Incorporated, postal or telecommunication authorities/provider or any other parties which may result in a charge incurred or made by the Eligible Cardmember or the applicable supplemental Cardmember being omitted from being posted to the Eligible Cardmember s account and/or captured in the Bank s system during the Campaign Period; (2) the s to the Eligible Cardmember which may result in the not being received by the Bank and therefore the Eligible Cardmember being omitted from participating in the Promotion; or (ii) (iii) (iv) (v) the breakdown or inability to access the Registration Site or any delay in the transmission or receipt of any SMS for any late posting of the Retail Purchases or for any failure in the Retails Purchases being transacted by the Eligible Cards or being captured in the Bank s system; for any notice or communication, or Free Gift which gets lost, misplaced, defaced, tampered with, stolen, damaged or misdirected; or for any breakdown or malfunction in any computer system or equipment Notwithstanding anything in the Terms and Conditions, the Bank may at any time vary, modify, add or delete any of the Terms and Conditions, including but not limited to terminating or withdrawing the Campaign on or amending the duration of 5

6 the Campaign without giving any reason, prior notice and/or assuming any liability to any party and shall not be liable to pay any compensation or enter into any correspondence in connection with the same and all Cardmembers shall be bound by these amendments or variations The Bank s decision in all matters arising out of or in connection with the Promotion is final and conclusive. The Bank shall not be obliged to enter into any correspondence with any persons and no appeal, correspondence or claims will be entertained By participating in the Promotion and completing the Registration Process, each Cardmember agrees to be bound by the Terms and Conditions. If there is any inconsistency between the Terms and Conditions and the UOB Cardmembers Agreement, the Terms and Conditions prevail only to the extent of such inconsistency The Free Travel Insurance is provided by the Merchant, under such terms and conditions determined by the Merchant, and therefore, the Bank will neither be responsible or liable for any defect, the quality, merchantability, the fitness for purpose or any other implied term or condition or any other aspect with respect to the Free Travel Insurance. The Bank also assumes no liability or responsibility for the acts or defaults of the Merchant. The Bank will not be responsible or liable for any injury, loss or damage suffered as a result of the redemption or usage of the goods or services or the Free Travel Insurance provided by the Merchant or arising from or in connection with the Promotion howsoever arising A person who is not a party to the Terms and Conditions and/or any agreement governed by the Terms and Conditions shall have no rights under the Contracts (Rights of Third Parties) Act (Cap. 53B) of Singapore to enforce any term of such agreement or any of the Terms and Conditions The Terms and Conditions are governed by the laws of Singapore and all Cardmembers participating shall be deemed to have agreed to submit to the exclusive jurisdiction of the courts of Singapore All information is correct at the time of publishing and the Bank makes no representation or warranty whether express or implied, and accepts no responsibility or liability for its completeness or accuracy. In the event of any inconsistency or discrepancies between these Terms and Conditions and any advertising, promotional, publicity and other materials relating to or in connection with the Promotion, the Terms and Conditions shall prevail. 6. Definitions 6.01 Bank or UOB refers to United Overseas Bank Limited and its successors in title Campaign refers to the UOB June Holiday Free Travel Insurance Campaign Campaign Period refers to the period commencing 23 May 2014 to 15 July 2014, both dates inclusive. 6

7 6.04 Cardmember shall mean any existing and new principal and supplementary holders of an Eligible Card, whose card account(s) is/are valid, subsisting, in good standing and satisfactorily conducted as determined by the Bank in its discretion Disqualified Cardmembers shall mean any Cardmember, including Eligible Cardmembers, who are disqualified from participating in the Promotion including those set out below:- (a) (b) (c) (d) any Cardmember whose single Eligible Card account and/or multiple Eligible Card accounts, as the case may be, is/are voluntarily or involuntarily suspended, cancelled, closed or terminated anytime during the Registration Period and Campaign Period; whose Eligible Card account(s) is/are not active, valid, subsisting or in good standing or which are otherwise determined by the Bank as being delinquent or unsatisfactorily conducted for any reasons as may be determined by the Bank at its sole discretion; who are deemed mentally unsound by Order of the Courts of Singapore, facing legal incapacity or is incapable of handling their affairs, deceased, insolvent, bankrupt or have any legal proceedings or investigations by any government authority (or any threat of the same) of any nature instituted against them; or who the Bank may decide to exclude, at its discretion, without prior notice at any time during the Registration Period and Campaign Period Eligible Card shall mean an existing Visa, MasterCard, American Express, JCB or China Unionpay credit / debit card or a Corporate credit card on personal liability which is: (a) (b) issued by the Bank in Singapore; and owned by the Eligible Cardmember, BUT shall exclude any Corporate Business/Purchasing credit/debit cards issued by the Bank Eligible Cardmember shall mean a UOB Cardmember who has complied with all the conditions set out under Clause 2.01(a) above Excluded Transactions shall mean the transactions listed under Clause 4.02 above a may be amended from time to time in accordance with the Terms and Conditions Family Members shall mean the following : (a) the Eligible Cardmember's legally married spouse who has not been legally separated or divorced from the Cardholder; and (b) all the Eligible Cardmember's legally dependent children including step-children and legally adopted children, each of whom is: under the age of eighteen (18) years or up to twenty-three (23) years on his/her next birthday; 7

8 studying full-time in a recognised institution of higher learning; unemployed; and unmarried Free Travel Insurance refers to UniTravel Single Trip Plan policy or UniTravel Annual Plan policy, underwritten by UOI as shown in Appendix A, provided that Travel Insurance can be redeemed by the Eligible Cardmember in accordance with the Terms and Conditions UniTravel Single Trip Plan policy refers to a certificate of insurance issued in conjunction with the master policy in the name of United Overseas Bank, where one Trip commences and ends between 22 May 2014 to 15 July Each Trip should not exceed thirty (30) days UniTravel Annual Plan policy refers to a certificate of insurance issued in conjunction with the master policy in the name of United Overseas Bank, where you can make unlimited number of Trips to any country, between 22 May 2014 to 21 May Each Trip should not exceed thirty (30) days Insured Person means the Eligible Cardmember or any one of his Family Members as defined herein Merchant refers to United Overseas Insurance Limited Registration Period refers to the period commencing 21 May 2014 to 30 June 2014, both dates inclusive Registration Process refers to the registration process described under Clause 2.01(a)(i) above Registration Site refers to Retail Purchases means all transactions made by a Cardmember using the Eligible Card or the applicable supplementary Eligible Card Qualifying Amount refers to the total minimum sum of Singapore Dollars One Thousand (S$1,000) worth of Qualifying Transactions for UniTravel Single Trip Plan and Singapore Dollars Five Thousand (SS$5,000) for UniTravel Annual Plan, charged either to the account of the Eligible Card of an Eligible Cardmember and/or to the account of the applicable supplementary Eligible Card during the CampaignPeriod Qualifying Transactions shall carry refer to the transactions referred to under Clause 4.01 above Terms and Conditions means the terms and conditions relating to the Campaign as may be amended from time to time Trip means the period commencing from the time you leave your place of residence or business to proceed directly to the place of embarkation in Singapore and ceases when you return to your place of residence or business or three (3) hours upon your arrival in Singapore after clearing the Immigration or the expiry of the 8

9 period of insurance shown on the Certificate of Insurance, whichever is the earlier. Subject to maximum duration of not more than thirty (30) days per Trip for Single Trip Plan policy and Annual Plan policy UOB Cardmember shall mean a principal Cardmember or Supplementary Cardmember but shall exclude Cardmembers:- (a) (b) who hold any Business Debit Cards, Corporate Cards (not on personal liability), Purchasing Cards, Empire World Mastercard, Platinum Business Cards, Distribution Cards and Business Platinum Cards; and whose Eligible Card accounts have been suspended, cancelled or terminated (excluding voluntary cancellation or termination of Card accounts where there are more than one (1) accounts and at least one (1) account remains) for any reason during the Registration Period UOB Cardmember Agreement refers to the prevailing UOB Cardmember Agreement which is available at Except where the context otherwise requires, words denoting the singular include the plural and vice versa. 9

10 Appendix A Table of Benefits Covers and Limits Maximum Limit 1. Personal Accident We will pay you or your estate the benefits according to the plan selected for death or permanent disablement arising from an accident. a) Each Insured Person below Age 70 years old 100,000 b) Each Insured Person Age 70 years old and above 85,000 c) Each Child as Defined 15,000 d) In aggregate as a Family 250, Medical, Dental & Other Expenses Incurred Overseas We will reimburse you for the medical, hospital, treatment and emergency dental expenses incurred including a sub-limit benefit for treatment for miscarriage arising from an accident. a) Each Insured Person below the Age 70 years old 100,000 b) Insured Person Age 70 years old and above 85,000 c) Each Child as defined 50,000 d) In aggregate as a Family 250, Follow-up Medical Expenses Incurred Upon Return to Singapore We will reimburse you for the necessary follow-up medical, hospital and treatment expenses reasonably incurred by you in Singapore within 31 days after your return to Singapore. 10,000 per Family 10

Emergency Medical Evacuation and Repatriation We will pay for the costs of emergency medical evacuation and/or repatriation recommended by International SOS a) Child / Insured Person below Age 70 years old 250,000 per Family b) Insured Person Age 70 years old and above 85, Loss of Deposit or Cancellation We will reimburse you for the irrecoverable deposits or charges paid in advance for trip cancelled due to medical or compassionate reasons, strike/riot, bankruptcy or insolvency of travel agency in Singapore. 5,000 per Family 6. Trip Curtailment We will reimburse you for the proportional return of irrecoverable prepaid costs if it is necessary for you to curtail your trip due to strike/riot, serious injury/illness or hijacking of the scheduled public conveyance in which you are traveling on. 5,000 per Family 7. Travel Delay If the scheduled public conveyance that you had arranged to travel is delayed due to industrial action, adverse weather conditions or mechanical breakdown for at least 6 consecutive hours from the time specified in the itinerary, 500 per Family Limit for each full 6 consecutive hours S$100 11

Baggage & Personal Effects We will reimburse you for the loss of or damage to baggage and personal effects while overseas. We will reimburse you for loss of money and replacement cost of travel documents in your personal custody arising out of robbery, burglary or theft. 5,000 per Family Limit per set or pair of Article 800 Limit for personal money Baggage Delay We will pay you a cash benefit if your checked-in baggage is delayed for more than 6 consecutive hours due to misdirection. 500 per Family Limit for each full 6 consecutive hours Terrorism We will indemnify for loss arising from terrorism excluding any act of terrorism involving the use or release or the threat of any nuclear weapon or device or chemical or biological agent.
