1 2018 Pond Owner Liability Insurance Many municipalities and pond owners require this Type of insurance in order to allow their park and lake facilities to be used for a clubs' sailing activities. In 2017 over 60 AMYA Sanctioned Clubs insured their Pond Owners with this policy. Every pond owner found the insurance provisions satisfactory. Sailing site property owners or their managing agencies can be provided as Additional Insured under AMYA's Pond Owner General Liability Insurance policy with The David Agency, Elmhurst, IL. This Liability policy provides $1,000,000 Bodily Injury and Property Damage coverage for the pond owner. Your AMYA Sanctioned Club can provide your pond owner with this insurance, which is in effect for a full calendar year (expiring December 31st), for the cost of $ A Club must have at least half (50%) of its member s current in their AMYA membership to apply for this policy. Insurance applies only to the Pond Owner, not the club or individual club members. If your club sails on different ponds with different owners you will have to apply for separate Additional Insured Certificates of Insurance. Example One Pond location/owner per policy! To provide your pond owner as an Additional Insured: #1 - Fill out the 2018 AMYA Pond Owner Insurance Application Form with Liability Description that is available as a download. PLEASE DO NOT re-use any form from a previous calendar year. It will be sent back! The form asks you to provide the following: - Name of the pond owner, municipality or agent for which the policy is being provided. - It also asks , fax and address of the person to which the policy certificate should be sent. - Provide your club's information with contacts person's address, the name and number of your club. DO NOT MAKE UP YOUR OWN FORM. PRINT OUT THIS TWO PAGE APPLICATION

2 CONTACT THE POND OWNER FOR CORRECT NAME, ADDRESS, AND ADDRESS (there will be a $25 fee charged back to the Club if the agency is not supplied current or correct Pond Owner Information) DO NOT FILL OUT THE FORM UNLESS YOU HAVE CONTACTED THE POND OWNER FOR CORREST INFORMATION, THEN MAIL Make out a check for $50.00 to... The David Agency Mail the your check and proper form to: Nick Mortgu AMYA Insurance Committee Secretary 501 Main Street Riverton, NJ The David Agency will or if no address is available fax a certificate of insurance directly to the pond owner naming them as Insured. The David Agency will also provide the AMYA Insurance Secretary with a copy of the insurance certificate. You will then be ed a copy for your records. It takes about three weeks to process the application form so please allow for the processing time. Please only contact the AMYA Insurance Committee Secretary, via or phone, if you have any questions or need another copy of the Certificate. PLEASE DO NOT CONTACT THE DAVID AGENCY for any changes or additions to the Certificate(s). This may subject your club to the $25 incorrect information charge assessed to us!

AMYA member s dues are what makes it possible for us to provide the two primary insurance policies: Regatta Liability and On the Beach, which make the Pond Owners Insurance available. All completed forms must first be approved to determine if a club is eligible to apply for Pond Owners Insurance. The David Agency will not process any

5 insurance application unless it is initialed and forwarded by the AMYA Insurance Committee Secretary. Do not send your application to the David Agency I have checked with the pond owner for current and up-to-date name, representative, address, phone number and address.
