1 GSLI : 1) Enrollment of new recruits who have joined the services of the Bank between and : The application to enroll as a member of the GSLI Scheme, irrevocable letter of authority to deduct monthly premium from salary and the nomination form are hosted in the HRM website. The staff members/officers who have joined the services of the Bank, between and should enroll themselves for the scheme by making use of this online registration facility. All the new entrants should complete this process on or before 27 th May After their submission of the application and nomination, it should be authorised by the Branch Manager. After authorization, pre-printed application and nomination forms will get generated. Necessary print-outs may be taken out and one set of the same should be forwarded to CO HRM duly signed by the staff member and authorised by the Branch Manager. The newly recruited employees who joined between and can exercise the option for any category related to their cadre. If they fail to exercise their option on or before 27 th May 2012, it will be deemed that she/he has opted for the lowest category applicable to their cadre. As such enrolment as member of the GSLI is made compulsory and the monthly premium will be deducted from their salary. The option once exercised is final and binding and it can not be modified later. According to the category chosen by them with respect to the Sum Assured, the monthly premium will be deducted from the salary with effect from the month of May As the Scheme shall be operative with effect from April 2012, the newly recruited employees who joined between and are required to pay the monthly premium with effect from the month of April Two months premium shall be deducted from the salary of May 2012 and thereafter, there will be regular deduction of monthly premium for the insurance cover opted by the employees. 2 The Savings/Risk ratio for the GSLI premium amount is subject to change for every year depending upon the mortality rate arrived at by LIC. LIC of India has informed that the said ratio for the year has been arrived as 50:50 for the Award Staff and 67:33 for the Officers. The Accident Benefit Premium amount may also vary from year to year, as fixed by United India Insurance Co. (UIIC), 2)Change of category for Promotees (Officers and Award Staff) who have got promoted between to The promotee Officers / Clerks will have to switch the corresponding category in the higher grade compulsorily. The details of their corresponding Sum Assured category and the irrevocable-letter-of-authority are hosted in the HRM website under GSLI tab. The staff members concerned have to take out a print out and send a duly filled-up form to CO HRM/GSLI Section. The form intended for Nomination is available in the HRM Website and the procedure for submitting the nomination is also furnished in the HRM- Website.

3 The nomination submitted by the staff members should be authorised by the BM/ABM. On authorisation a reference number will be generated. A hard copy of the nomination, bearing the system generated reference number, duly witnessed by two persons and autheticated by the BM/ABM should be forwarded to CO/HRM GSLI Section. Nomination forms not duly filled in/ not witnessed/ not authenticated will not be taken as valid nomination. The nomination now submitted will supersede the earlier one and will be binding. Importance of continuous remittance: The scheme is introduced in our Bank to have low cost premium using the advantage of discounts available to groups under LIC Scheme. The Scheme remains as fully contributory by the staff members. Hence the members of the GSLI Scheme should realize the importance of keeping the insurance cover in tact by remitting the premium in time. It has been stated by LIC of India that if premium is not received in respect of any member for a continuous period of six months then the said member will cease to be covered under the Scheme and will not be allowed entry into the Scheme again. Hence, Staff members who proceed on EOL / placed under Suspension / under LOP will have to make their own arrangements for the continuous remittance of monthly premium, by giving authorization/standing instructions to the branch to remit regularly by debiting their SB account so as to keep the cover under the Policy in tact. In case of member proceeding on EOL, advance payment in full can be made by contacting CO/HRM Dept-GSLI Section. Other Important Points: The membership of GSLI scheme shall automatically cease immediately upon the retirement / death / resignation /VRS / cessation / termination / separation/dismissal/crs/removal/discharge etc. In the unfortunate event of employee dying in harness, the family members of the deceased staff will receive not only the Insurance cover from LIC through our Bank, but also the accumulated savings portion subject to the payment of the monthly premium regularly. In the case of death on account of accident the family members of the deceased staff will receive twice the amount of the Sum Assured as LIC and UIIC will settle the claim amount sum-assured respectively. In the case of death due to accident the same should be informed immediately to the CO:HRM-GSLI section and also the same should be reported to the following address of United India Insurance Co giving the full details regarding the incident/accident.
