1 1.1. Name of Promotion Minnesota State Lottery Second Chance Promotion Ultimate Getaway January 2, 2018 Amended August 7, 2018 The name of this promotion is Ultimate Getaway Definitions A. Ultimate Getaway or Scratch Ticket or Ticket means the Minnesota State Lottery Scratch Game #694. B. The Lottery means the Minnesota State Lottery. C. Minnesota Second Chance Promotion or Second Chance Promotion means the drawings conducted by the Lottery in conjunction with the Ultimate Getaway Second Chance Promotion. D. Players Club means the registered database located on mnlottery.com Duration A. The Second Chance Promotion will begin at 5:00:00 a.m. Central Time ( CT ) on January 2, 2018, and conclude at 11:59:59 p.m. CT on September 4, Eligibility A. A person may enter the Second Chance Promotion by becoming a member of the Players Club and supplying all of the required registration information and entering the 10-digit validation number found under the scratch-off latex coating of a non-winning Ultimate Getaway Scratch Game Ticket. B. Each non-winning Ticket entered will receive one (1) in the Second Chance Promotion. C. Only one (1) winner per household per drawing is allowed. D. The submission of a winning Ticket invalidates that. E. 2 to receive a prize from the Lottery pursuant to law or a contract with the Lottery, and all other persons prohibited by law are ineligible to participate or receive a prize from this Second Chance Promotion. F. A person who submits an must be 18 years of age or older. G. The Lottery is not responsible for delayed, lost, or misdirected electronic submissions made through the Internet. H. A person may enter more than once; however, each player may use only one (1) unique Ticket number per. Multiple entries using the same non-winning Ticket number and entries made via automated computer scripts or other programming techniques will be disqualified. I. A player who uses a non-winning Ticket as an in this Second Chance Promotion that has previously been used in any other Minnesota Lottery Second Chance Promotion or contest will be considered ineligible. J. By participating, players agree to these official rules and to the decisions of the Lottery which will be final and binding in all respects. K. The Lottery will, by use of mechanical or other devices, randomly determine which of the entries submitted are selected. C. The Lottery will conduct two (2) drawings at Lottery Headquarters. A Lottery representative will draw fourteen (14) total prizewinners for the following prizes (each a Prize Package ): Six (6) Primary $10,000 Prize. Eight (8) Secondary $5,000 Prize The Lottery reserves the right to substitute prizes for any reason, including based on availability. The first three (3) entries drawn in each drawing will each receive a Primary $10,000 Prize Package. A Primary $10,000 Prize Package consists of up to $10,000 in pre-purchased airfare, hotel accommodations, restaurant gift cards, attractions/show/sporting event tickets, car rental, cruise, holiday package, and/or rail tickets for a total prize value of $14,760.15, which includes state and federal withholdings. The next four (4) entries drawn in each drawing will each receive a Secondary $5,000 Prize Package. A Secondary $5,000 Prize Package consists of up to $5,000 in pre-purchased airfare, hotel accommodations, restaurant gift cards, attractions/show/sporting event tickets, car rental, cruise, holiday package, and/or rail tickets for a total prize value of $5,000 (state and federal taxes are the responsibility of the prizewinner). D. A person may win only once in each Second Chance Promotion drawing, but may win a prize in more than one drawing. E. A list of all prizewinners will be posted on the Contest Winners page of mnlottery.com following the drawing and after Minnesota Lottery Rules for Second Chance Promotion Ultimate Getaway Effective 01/02/18 Page 3 of 7

4 prizewinners have been verified. The list will remain on the Contest Winners page for the duration of the Claim Period (as defined below). F. By entering this sweepstakes, prizewinners consent to full use of any and all information they provide on the form for publicity, public relations, and content creation purposes, including on the Lottery s social media channels and mnlottery.com, by the Lottery. Prizewinners may be required to execute an Affidavit of Eligibility form and appropriate releases and authorizations. Refusal to do so may result in forfeiture of any prize won. G. A prize won in a Second Chance Promotion may not be assigned or transferred to another person. All applicable federal, state, or local laws apply. H. Promotion questions may be referred to the Lottery at (651) , option 2, or promotions@mnlottery.com Prize Package Fulfillment Prize Package fulfillment will include the pre-purchasing, scheduling, and coordination of travel arrangements as detailed below: A. No cash option. B. Travel must be within the continental United States, Hawaii, or Alaska (U.S. Territories are not included). C. Each trip booking must include the prizewinner and may include up to three (3) guests. Prizewinner and guest(s) must travel together. Note: The prizewinner s guest(s) must be 18 years of age or older. A minor may accompany the prizewinner only if the prizewinner is the guest s parent or legal guardian. D. All components of the Prize Package must be used together, on one (1) trip, at one destination between specific dates of travel agreed on by the Lottery and prizewinner. E. Prizewinners have one (1) year from the drawing date to claim the vacation prize and one (1) year from claiming their prize to use their travel prize. Prizewinners must book travel within ninety (90) days of claiming their prize. F. 5 a. Airfare; b. Seat Selection; c. Accommodation; d. Transfers; e. Restaurants dependent on whether the restaurant(s) chosen offer electronic gift cards. Note: Maximum of two (2) restaurant reservations/gift cards per trip; f. Attractions/Show/Sporting Event Tickets Must be prebookable/pre-payable. Note: Maximum of two (2) attractions/events per trip; g. Car Rental; h. Cruise; i. Holiday Package; or j. Rail Tickets. G. All requests are based on availability. H. Winners of a Primary $10,000 Prize must pre-purchase a minimum of $7,500 in allowable travel expenses. Winners of a Secondary $5,000 Prize Package must pre-purchase a minimum of $3,750 in allowable travel expenses. I. Once the maximum prize allowance for a Prize Package has been met, the prizewinner must pay for any additional amounts. J. If a prizewinner fails to show at the designated time of departure, the trip packages will be forfeited. K. Hotels may require a valid credit card number or cash deposit from the prizewinner to cover incidentals. L. Airlines, hotels, and other travel vendors may have additional fees that are not pre-purchased as part of the Travel Package, including baggage and resort fees. If applicable, prizewinner and guest(s) are responsible for any fees. M. Travel insurance is the responsibility of the prizewinner and guest(s). N. 6 5.3. Claiming Prizes mail a check within thirty (30) business days upon receiving the final expenses for the fulfillment of the Prize Package. Each person whose was selected in the Second Chance Promotion drawing to win a prize valued at $600 or more must fill out and submit a claim form to the Lottery. In the event that the claim is not validated by the Lottery, the claim will be denied and the player will be notified immediately. If a prizewinner is delinquent in the payment of Minnesota state taxes or owes a debt as defined in Minn. Stat. 270A.03, subd. 5, the amount due will be deducted from the value of the prize including, but not limited to, the cash portion of the prize, and the cash value of the merchandise or non-cash prizes. If the prizewinner owes a debt as described above, they will be given thirty (30) calendar days from the date the Lottery informed them of the existence of a debt in which to fully satisfy the debt, as determined by the Minnesota Department of Revenue. In the event that the prizewinner fails to completely satisfy the debt within 30 days, the Lottery will liquidate the non-cash prize and forward any proceeds received to the Department of Revenue to satisfy or partially satisfy the debt on the prizewinner s behalf. If any cash remains after the Lottery has forwarded the proceeds of a non-cash prize to the Minnesota Department of Revenue, the prizewinner will be awarded the remaining cash Claim Period All prizes in this Second Chance Promotion must be claimed within one (1) year of the date of the drawing at which the prize was awarded. If, however, the one (1) year deadline is a Saturday, Sunday, or legal holiday, a claim must be submitted by the close of business the following business day. The Lottery reserves the right to substitute prizes based on when the prize is claimed during the claim period. The Lottery will post the end of the claim period on mnlottery.com. Questions may be referred to the Lottery at (651) , option 2, or promotions@mnlottery.com Allowance for Delay of Payment The Lottery may delay payment pending a final determination by the Lottery under any of the following circumstances. 7 A. if a dispute occurs or it appears likely that a dispute may occur regarding the prize; B. if a dispute occurs or it appears likely that a dispute may occur regarding the ownership of the ; C. if there is any question regarding the identity of the claimant; D. if there is any question regarding the validity of the or the claim presented for payment; or E. if the claim is subject to any reduction for delinquent debts owed by the claimant under Minn. Stat. 349A subd. 8. No liability for interest for any delay will accrue to the benefit of the claimant pending payment of the claim Emergency Termination If, for any reason, it is not possible to run all or any part of the Second Chance Promotion as planned due to, without limitation, errors in these official rules or advertising for this promotion, tampering, unauthorized intervention, fraud, technical failures, human error, or any other cause beyond the control of the Lottery that, in the sole judgment of the Lottery, could corrupt or affect the administration, security, fairness, integrity or proper conduct of this promotion, the Lottery reserves the right, at its sole discretion, to cancel, terminate, modify or suspend the promotion Governing Law By submitting an to the Second Chance Promotion, a person agrees to comply with and abide by these game procedures: Minn. Stat. 349A (State Lottery); Minn. R (Lottery Prize Payments); and all final decisions of the Director of the Lottery. All applicable federal, state, or local laws apply.
