1 Franklin Redevelopment and Housing Authority I. GENERAL SPECIFICATIONS Request for Proposals PEST CONTROL RFP # PEST Franklin Redevelopment and Housing Authority ( FRHA ) is accepting proposals from licensed and insured pest control firms for services to its residential and administrative properties. Services will be performed primarily at FRHA s 162 units at Berkley Court, Pretlow Gardens, Old Town Terrace, The Terrace at Bruce Street and the FRHA Central Office, Administration Office and Maintenance buildings. The Request for Proposals ( RFP ) documents will be available beginning Monday, May 22, 2017 at the Franklin Redevelopment and Housing Authority Central Office, located at 100 East Fourth Avenue, Franklin, VA or online at Respondents proposals should be prepared in accordance with the instructions in Section IV below. An optional site visit may be coordinated by contacting FRHA. Requests or questions regarding this RFP should be directed in writing to 100 East Fourth Avenue, Franklin, VA or via to franklinrha@live.com. All requests and questions must be received no later than Tuesday, May 30, Respondents proposals must be received by FRHA in sealed envelopes at 100 East Fourth Avenue, Franklin, VA by 11:00 AM ET, Friday, June 9, Envelopes should be marked with RFP# 2017 ETERM. Proposals will be held in confidence by FRHA and not released in any manner until after the completion of negotiations with the successful respondent. By submitting a response, each respondent is affirming his/her/its commitment to comply with the Laws of the City of Franklin, Commonwealth of Virginia and the United States of America governing Fair Employment Practices, and all rules and regulations of the U.S. Department of Housing and Urban Development (HUD) governing Equal Employment Opportunities and Non-discriminatory Practices. FRHA reserves the right to reject any or all responses and to waive any informality in the selection process. II. BACKGROUND Franklin Redevelopment and Housing Authority is a political subdivision of the Commonwealth of Virginia, created under Title 36 of the Code of Virginia (1950), the Virginia Housing Authorities Law. The FRHA is located in Franklin, VA, approximately 50 miles west of Norfolk. It operates rental housing and provides other housing assistance to low-income residents of Franklin and Southampton County. A seven (7) member Board of Commissioners appointed by the City Council of the City of Franklin governs FRHA. An Executive Director is appointed by the FRHA Board and is responsible for managing the daily operations of the agency. FRHA owns and manages 150 multi-family housing units on three sites. FRHA also administers a Housing Choice Voucher (Section 8) rental assistance program providing rent subsidy for up to 315 participants and has created a successful homeownership program. III. SCOPE OF SERVICES The services under this RFP shall include, but is not limited to, control and elimination of Page 1 of 6

2 troublesome, annoying or destructive animals/insects and their hybrids, including rats, bats, mice, snakes, spiders, moths, beetles, bees, wasps, flies, fleas, roaches, ants, crickets and silverfish within the property boundaries of the specified building(s) and any other household pest on the Authority's properties. The contractor shall furnish all professional and skilled services, labor, materials, tools, equipment, insurance and permits, as necessary, to provide as needed service for the Authority s three Multi- Family Housing developments and Administration building, as well as any other Authority-owned property. Work, as described above, shall be primarily performed in Franklin, VA at the following locations: 1. Pretlow Gardens (925 Pretlow St) 40 units 2. Berkley Court (702 Cameron St) 75 units 3. Oldtown Terrace (331 Bruce St) 35 units 4. FRHA Central Office (100 East Fourth Ave) 5. Berkley Community Center (601 Campbell Ave.) 6. Pretlow Community Center (925 Pretlow St.) 7. Old Town Terrace Community Center (331 Bruce St.) 8. The Terrace at Bruce Street (340 Bruce St.)-12 units IV. REQUIRED SUBMISSION FORMAT AND EVALUATION FACTORS The proposal must be submitted in accordance with the specifications of this Request for Proposals and include all of the items listed below. The proposal shall include a Table of Contents and be simply bound and presented in tabbed sections as described below. No requirement and/or specification should be construed as an attempt on the part of FRHA to limit competition. Sections to be included are listed below along with their maximum point values: 1. Company Profile (35 points) A letter introducing the company, its qualifications and experience in providing extermination services. Also list any current contracts and the names, addresses, and telephone numbers of at least three (3) client references in which you performed work within the past five (5) years. Firms should highlight experience with clients similar to FRHA in size, needs and work scope. 2. Rate & Response Times (50 points) List the proposed hourly rate, and response times, for service calls under each of the three (3) time frames listed below. In all cases, the first hour is understood to be guaranteed and the rate is to include all labor, services and equipment needed to accomplish the work. A separate hourly rate and response time should be listed for service calls during each of the following times: Page 2 of 6

3 a. Normal Work Hours Monday through Friday from 8:00AM to 5:00PM EST. b. Evening Hours Monday through Thursday from 5:01PM to 7:59AM EST. c. Weekends Friday from 5:01PM to Monday 7:59AM EST. Same rate shall be used for holidays recognized by the federal government. NOTE: The proposed cost will be considered in addition to the above technical and experience factors in determining the most advantageous proposal. 3. Section 3 Status (15 points) Statement of Section 3 business status as defined in the Housing and Urban Development Act of State whether firm is 1) a Section 3 business, 2) is not a Section 3 business, or 3) is not a Section 3 business but has made significant effort, or is willing to donate a fixed portion of the contract value, to further HUD s Section 3 goals. If the last option is selected, provide an explanation of the firm s ongoing efforts. (i.e. donations to PHA programs, outsourcing, etc.) 4. Contractor s License (Mandatory) Copy of the contractor s license. License information will be used to determine whether contractor has been debarred or is otherwise unable to perform the duties under this RFP. 5. HUD-5369-C-Modified (Mandatory) Completion of Form HUD-5369-C, Certifications and Representations of Offerors, Non Construction Contract. The FRHA modified version is attached to this document and available on the FRHA website. The entire proposal should not exceed 20 pages, excluding the table of contents, tabs and dividers. (Note: Ten (10) sheets of paper with information printed on both sides are considered to be 20 pages. Other information thought to be relevant, but not applicable to the categories below, may be provided but only as an appendix or addendum to the requested items. Please do not submit unrelated promotional material or information that is of a repetitive nature as it may not be evaluated. The proposal should be concise and to the point. V. SCHEDULE Routine services are to be performed monthly at all locations. In addition, emergency service may be requested at any time and coordinated with firm. VI. SELECTION PROCESS FRHA intends to negotiate with the top ranked firm(s) after an evaluation of the factors listed above. A final payment plan and contract will be negotiated with the number one ranked firm. If an agreement cannot be reached, FRHA will then negotiate with the number two-ranked firm, etc. FRHA reserves the right to conduct negotiations with one or more respondents and to enter into a contract with more than one firm if, in the sole opinion of the FRHA, that method will provide the greatest benefit to the FRHA. 4 VII. CONTRACT TERM AND SPECIFICATIONS The term of the contract is intended to be two years from the date of the contract with an option of up to three (3) annual renewals. Prices and terms shall remain the same as the original contract except if both parties mutually agree to changes. FRHA reserves the right to negotiate price adjustments in future years based on economic conditions. The work to be performed under the contract is subject to the requirements of Section 3 of the Housing and Urban Development Act of 1968, as amended, 12 U.S.C. 1701u (Section 3). The purpose of Section 3 is to ensure that employment and other economic opportunities generated by HUD assistance or HUD-assisted projects covered by Section 3 shall, to the greatest extent feasible, be directed to low- and very low-income persons, particularly persons who are recipients of HUD assistance for housing. The parties to the contract agree to comply with HUD's regulations in 24 C.F.R Part 135, which implement Section 3. As evidenced by their execution of the contract, the parties to the contract certify that they are under no contractual obligation or other impediment that would prevent them from complying with the Part 135 regulations. The respondent may not transfer or assign any portion of the contract without prior written approval from the FRHA. Firm must have insurance with an A-rated carrier showing a $1,000,000 professional liability. At execution of contract, firm shall provide a Certificate of Insurance naming FRHA as an additional insured as our interests appears with 60 days notice of cancellation. VIII. RFP TERMS AND CONDITIONS FRHA reserves the right to reject any and all proposals submitted, to award portions of this contract in whole or in part, to not award contract, and to waive any informalities in the submission of proposals. FRHA does not discriminate against faith-based organizations or any person on the basis or race, color, genetic information, sex, religion, national origin, age, familial status, or handicap.
