3 INSTRUCTION 2952F4 Page 2 of 9 BONNEVILLE ONLINE SCHOOL ENROLLMENT REQUIREMENTS Eligibility for enrollment in the shall be pursuant to applicable Idaho Code and Bonneville Joint School District No. 93 policy and procedures. All enrollments shall be contingent upon the availability of District resources. Therefore, the Board reserves the right to adjust the capacity of the school as required. Guidelines Eligibility for Enrollment In order to enroll in the, student eligibility must be established. Entrance Age No pupil may be enrolled in kindergarten whose fifth (5 th ) birthday does not occur on or before the first (1st) day of September of the school year in which the child registers to enter kindergarten. 1. No pupil may be enrolled in first grade whose sixth (6 th ) birthday does not occur on or before the first (1st) day of September of the school year in which the child registers to enter school. Birth Certificate A certified birth certificate issued from the Bureau of Vital Statistics, or other legal proof of a child s age, shall be required of students enrolling in the Bonneville Joint School District No. 93 for the first time. Immunization Records Immunization records or a valid exemption are required for student admission to the Bonneville Joint School District No. 93 schools. Proof of Residence Unless open enrollment status is granted by the Bonneville Joint School District No. 93, students who enroll in District schools must reside full-time within the School District boundaries with their natural parent or legal guardian. I realize this is a privilege and a responsibility. By signing this agreement, I commit: To use the computer for educational purposes, not to play games or conduct personal or commercial business on District time (refer to Board policy #3270 Computer/Network Acceptable Use Agreement); To save work on the network only as directed and approved by my instructors; if I do not have a network account, I may need to save my work on a floppy diskette or USB drive, not the hard drives of the District s computers; Not to load any software on District computers without permission, recognizing that it is illegal to load copyrighted software on a computer unless the software license allows it; To check all uploaded or downloaded software for viruses before using data, shareware, or freeware on school equipment and to immediately notify school personnel if a virus is found; Not to use the network to access or transfer information that is password protected, belongs to someone else, or is not for general access; To use the Internet only while under supervision and with permission of a staff member; To avoid material I may encounter on the Internet that does not relate to educational pursuits (such as materials that encourage illegal activities, or are obscene, violent, etc.); To abide by District s Computer/Network Acceptable Use Agreement policy #3270; To treat hardware, software, and the network with respect and to do all within my power to protect them from misuse, abuse, or theft; and To understand that any violation of the regulations above is unethical and may constitute a criminal offense. 7 Page 6 of 9 BONNEVILLE SCHOOL DISTRICT #93 Bonneville Online High School AGREEMENT FOR USE OF INSTRUCTIONAL PROPERTY Bonneville Joint School District No. 93 (D93) has made arrangement to permit the family of the student to use certain computer equipment, printer, headphones, mouse, (Instructional Property) to facilitate the student s education while enrolled in the Bonneville School District, (BOS), provided that the parent or guardian of the student who is enrolling (Responsible Party) is willing to accept responsibility for the Instructional Property as set forth below. (Please initial each statement after reading) BOS shall permit the student and Responsible Party to use the Instructional Property issued by D93. Responsible Party's and the student's rights to use and possess Instructional Property expire upon the student's termination of enrollment. Notwithstanding the foregoing, BOS reserves the right to terminate any use or possession immediately if either has reason to believe that the student or Responsible Party is violating any term or condition of this agreement. Responsible Party shall return all Instructional Property as instructed within ten (10) days of the termination date in the same condition as delivered, normal wear and tear excepted. At no time shall legal title to or ownership of any Instructional Property vest in the Responsible Party or student, who shall only have the rights to temporary use and possession. Responsible Party agrees to fill out, sign, and return the Instructional Property Receipt Acknowledgment Form (enclosed with the Instructional Property) to the BOS principal acknowledging delivery and affirming satisfactory operating condition upon receipt. Responsible Party shall maintain the Instructional Property at the residence set forth as the mailing address on the registration form unless Responsible Party provides thirty (30) days' written notice of a new address to the BOS principal. Responsible Party shall be solely liable for any loss or damage to the Instructional Property until it is returned and shall take all reasonable precautions to protect it. Responsible Party agrees to inform the BOS principal of any loss or damage to Instructional Property from any cause within three (3) days of loss or occurrence of damage. A Loss Report Form that includes provisions for the Responsible Party to pay for lost or damaged Instructional Property will be provided. Upon receipt of the completed form, BOS will determine replacement costs and procedures. Responsible Party is liable for maintenance and repairs of Instructional Property while in his/her possession and will follow all guidelines for Instructional Property requiring repairs as directed by BOS and D93 staff, technical support and the warranty agreement with the original equipment manufacturer. Responsible Party is solely responsible for upgrading to new software versions when publicly available, obtaining and installing antivirus file updates, and overall maintenance of each software application provided.
8 Page 7 of 9 Responsible Party agrees that: a. Instructional Property shall be used solely for the education of the student while enrolled at BOS and not for the benefit of any other person or for any other purpose; b. All Instructional Property shall be used pursuant BOHS and manufacturer instructions, policy and procedures; c. Each software application provided shall be subject to, and used pursuant to, the license and/or use agreement that accompanies that software application; d. All use of Instructional Property shall be subject to BOS and D93 policies and procedures regarding Network/Internet use and protocol; e. Responsible Party is solely responsible for ensuring that software settings, default configurations, and administrative privileges are maintained at the original specified settings and will be liable for any damage to Instructional Property, files, and/or other software applications resulting from unauthorized changes or modification to default configurations and/or original settings; and f. Responsible Party is solely responsible for keeping user IDs and passwords confidential to prevent unauthorized use and understands that passwords should be changed on a monthly basis. Responsible Party agrees to indemnify, defend, and hold harmless Bonneville Joint School District No. 93, BOS, their respective employees, officers, directors, agents, assignees, and all affiliated companies and/or entities (Indemnified Parties) from and against any and all claims, actions, suits, proceedings, costs, expenses (including, without limitation, court costs and attorney fees), damages, obligations, judgments, orders, penalties, fines, injuries, liabilities, and losses arising directly or indirectly out of or in connection with any matter covered by this Agreement, other than those caused by BOS. No Indemnified Party makes any warranty or representation, either express or implied, as to the value, design, condition, merchantability, or fitness for a particular purpose, or fitness for use of any Instructional Property. In no event shall any Indemnified Party be liable for any actual, incidental, direct, indirect, special, or consequential damages in connection with or arising out of this Agreement, or the existence, furnishing, functioning, usage, or maintenance or any Instructional Property provided under this Agreement. Responsible Party agrees to maintain at his/her expense adequate insurance to cover damage to Instructional Property by fire, theft, flood, explosion, accident, act of God, or other cause to the full replacement value of the Instructional Property and agrees that he/she will be financially liable for it regardless of the availability of insurance proceeds. Responsible Party represents that he/she has the power to bind all the student s parents or legal guardians, all of whom shall be bound by these terms. Responsible Party cannot amend this Agreement unless accepted in writing by a representative of the BOS and BOS principal. This Agreement shall constitute the entire agreement between the parties with regard to Instructional Property and any prior understanding or representation of any kind shall not be binding on either party, except to the extent incorporated herein. The waiver of any right under this Agreement by either party shall not be construed as a waiver of the same right at a future time or a waiver of any other right under this Agreement. This Agreement shall be construed and enforced pursuant to the laws of the state of Idaho.

Notify your BOS principal of any malfunction. They will direct you to call Dell tech support. 4. Return the equipment to BOS within ten (10) days of the student withdrawing from BOS, or legal action will be taken. 5. Be safe. Be smart. Enjoy this equipment to further your student s education.
