The Club is open from 6.30 am until p.m. Monday to Friday, and 7.00 am until 10pm on the weekend. Our ladies and gentlemen s changing rooms have showers, hairdryers and lockers. Please provide your own towel. We have one squash court available. This can be booked 7 days in advance via the Leisure Club host. Our Gymnasium equipment includes rowing machines, X Trainers, bicycles, treadmills, free weights area, and stretch area which includes a 4 station multi gym. Our Leisure Club host will be happy to advise you on the correct use of the equipment. The heated Swimming Pool is 16ft wide by 35ft long with an adjoining children s pool. (Diving in not permitted). 3 CONDITIONS OF MEMBERSHIP H²O Leisure Club The Company refers to the Croydon Park Hotel. The facilities refer to the Hotel & Recreation area and include the swimming pool, sauna and changing rooms, gym equipment, whirlpool and squash. Membership shall be valid for six month/one year only from the date of issue of the membership card. No membership fees will be refunded under any circumstances. Cancellation can only take place after a minimum 6 months membership. No cancellations will be taken before this time. Membership is processed once we receive the fee along with the completed application form and photographs. The full annual or half annual payment is required, monthly payments are available. If payment is made by cheque, the cheque guarantee card number must be written clearly on the back of the cheque and made payable to: - Croydon Park Hotel. Membership fees are as follows: DIRECT DEBIT Full adult membership per month Off peak adult membership (Monday to Friday 9am to 5pm only) per month Corporate membership per month (Minimum of 5 persons from the same company paid and contracted by company) Child membership per month All contracts to be based on a minimum of 6 months. 4 ALL OF THE ABOVE MUST BE STRICTLY ADHERED TO AND NO VARIATIONS WILL BE ACCEPTED. Guest Fees: Each member may be accompanied by up to two guests, at one time, at a charge of 5.00 per child (4-15 years old) and per adult (over 16 years old) per visit. Payment must be made at the main Hotel Reception before entering the Leisure Club and your receipt must be submitted to the Leisure Club Host. The Company may vary the items at which the facilities are available in connection with repair, alterations or maintenance or when required for the Company s own use without prior notice. The Company and its employees and agents shall be under no liability in respect of the injury or death to members or their children or guests. We do not accept liability to any loss or damage to property brought into the Hotel (whether with an attendant or in a locker) howsoever caused. All children under the age of 15 years MUST be accompanied by an adult at all times. Members, children and guests shall observe any regulations of the use of the facilities, which may be posted in the Hotel and which may be added to and varied. Any members or guest breaking any rules or regulations in leisure club may be asked to leave. Members shall ensure that their children and guests observe the conditions set out above and shall indemnify the Company and its employees and agents in respect of any loss, claims and liabilities arising through the death or injury of such member of his/her children or guests. Such indemnity shall continue notwithstanding the termination, expiry or renewal of Membership. The Company shall be under no obligation to renew or extend membership but in the event of such extension or renewal the conditions set out above continue to apply. The Company reserve the right to alter membership and/or guest fees at any time without prior notice. 4

In addition to this, if you are in any doubt as to your medical condition you are strongly advised to consult your GP before taking part in any form of exercise. The Clarion Hotel Croydon Park reserves the right to refuse entry to its Leisure Club facilities to any individual on health and safety grounds. 1. 7 RULES AND REGULATIONS Gym (free weights and machine area) Under 16s are not permitted to use the gym All gym users must complete a Physical Activity Readiness Questionnaire All gym users must have an induction on the correct use of gym equipment prior to the use of any equipment. Appropriate clothing and trainers must be worn at all times Do not remove weights from the weights area Pool Children under the age of 15 must be supervised by an adult No running No diving No ball games No fighting Food or alcohol are not permitted Before entering the pool you must shower No jumping The correct swim wear must be worn Sauna Children under the age of 15 are not allowed inside the sauna Hanging wet clothes in the sauna is not permitted Food and alcohol are not allowed Oils are not permitted in the sauna If you are pregnant please do not use the sauna. The Croydon Park Hotel will not be liable for any injuries sustained. Please be aware that the temperature inside the sauna can go up to 100⁰Celsius. Jacuzzi Children under the age of 15 are not permitted to use the Jacuzzi Food and alcohol are not allowed in the Jacuzzi No more than 4 people can use the Jacuzzi at any given time Before entering the Jacuzzi you must shower Please be aware that the temperature of the Jacuzzi is set at 40⁰Celsius If you are pregnant please do not use the hot tub. Changing rooms Please be aware that the changing rooms close at 10pm Running or chasing in the changing room is prohibited Lock all items inside the locker and make sure its locked Anyone found defacing or damaging the changing room will have their membership revoked and will be held liable for any damages incurred. Croydon Park Hotel is not responsible for any items that are lost, stolen or damaged. General All members must show their membership card every time they come to the leisure club No alcohol is permitted inside the leisure club Children under the age of 16 must be with an adult at all times Appropriate foot wear must be worn in all areas 7

8 TERMS AND CONDITIONS 1. Lost/stolen membership cards must be reported to a member of staff at the Leisure Club. 2. First payment must be made at the hotel reception on completion of the application form. 3. All monthly membership have to be set up by standing order. 4. Membership can also be paid in full at the hotel reception 5. Corporate membership has to be a minimum of 5 people from the same company paid and contracted by the company. 6. All contracts must be based on a minimum of 6 months 7. Children 15 or under membership must be authorised and signed by an adult 8. Membership is valid from date of first payment. 9. Direct Debits will be collected on the 1st of each month. 10. It is a condition of acceptance of membership that the member agrees to pay the initial subscription fee and agrees to be bound by these terms and conditions 11. H2O The Leisure Club reserves the right to refuse entry to any members who are deemed to be under the influence of alcohol or any non-prescription drugs 12. Smoking anywhere on/inside the premises of the Leisure club is forbidden 13. H2O leisure club memberships that have been cancelled and have been charged will be refunded 14. H2O Leisure club membership card must be swiped at the card reader presented to staff on every visit. 15. If you require to cancel a pre-booked activity then 24 hours notice is required or will be charged 16. H2O Leisure club reserves the right to exclude Leisure club members if their behaviour is deemed inappropriate or constitutes a risk to staff and/or members without any refund of fees already paid. 17. Children under the age of 15 years or under must be accompanied by an adult at all times. 18. Only authorised users will be admitted to the gym. An Induction or Familiarisation session is required to become an authorised user. All users must be signed off by a member of the gym staff prior to accessing the facility. 19. Access will be restricted during maintenance work from time to time 20. To access the leisure club all members must be 16 years and over. 21. All facilities and activities are subject to availability and are governed by the leisure club rules 22. Data collected in accordance with the Data Protection Act It is the member's responsibility to ensure the data we hold for them is up to date and accurate. 23. H2O leisure club reserves the right to amend terms and conditions at any time. 24. H2O leisure club membership does not include the use of a club/hotel towels. Towels are for residents only 25. Please be aware that the prices are subject to increase within a reasonable notice period left at the discretion of the Croydon Park Hotel. 26. H2O leisure club have the right to open and dispose of any items found inside lockers 27. H2O leisure club reserves the right to alter the provision of any specific services provided. 28. H2O leisure club reserves the right to alter all car park provisions 29. H2O leisure club only freezes membership in case of emergency i.e. medical 30. All H2O Leisure Club members are entitle to 10% off their food bill in Whistlers and Oscars Brasserie. This discount cannot be applied to any drinks, only food. Discount is not available in conjunction with any other promotions. This discount is valid for a maximum party size of 4 people. The 10% discount is not valid on special promotions, including Christmas & New Year s, Valentine s, Mother s day, Easter period, Father s day. The discount cannot be applied to Afternoon Tea bookings. The Croydon Park Hotel maintains the right to withdraw this or any promotion at any time without notice. 9 CONFIRMATION I understand and accept that exercising in the leisure club carries with it a risk of personal injury or even death. I confirm that I am voluntarily participating in activities in the leisure club and that the information provided on this form is complete and correct. I also agree to inform the leisure club secretary or leisure club staff immediately of any change in my condition and to complete and sign a new health questionnaire form. I confirm that I am not aware of and have not been advised of any medical condition or reason why I cannot participate in the activities offered by the clarion hotel Croydon park leisure club and agree that I do so at my own risk in relation to any pre-existing medical condition I understand that your staff are not medically qualified and that regardless of the information given above admission does not constitute acceptance by the clarion hotel Croydon park of any liability in respect of pre-existing medical conditions.
