2 . About ASMO: The Afghan Social Marketing Organization (ASMO) is a social marketing and behavioral change communication organization established in August 008. ASMO as a free-standing, not-for-profit Afghan organization aims to create positive health outcomes by giving Afghan households the information they need to make informed and healthy choices and expanding access to high quality health products through commercial markets. The Sustaining Health Outcomes through the Private Sector Plus (SHOPS Plus) project led by Abt Associates awarded Afghan Social Marketing Organization (ASMO) a project that improves the delivery of high quality family planning and maternal and child health products to target populations in Afghanistan through the private sector. SHOPS Plus will provide technical assistance to ASMO in designing and implementing high quality social marketing interventions which leads to increased access to affordable priority health products and increased demand for priority health products and services. In addition, SHOPS Plus will collaborate with ASMO in developing a vision and roadmap for ASMO sustainability.. Purpose/Intent: ASMO is soliciting quotations from qualified Printing Companies to supply packaging materials for health products as per scope given below. See section statement of work of this RFQ. Companies willing to respond to this RFQ are encouraged to prepare and send submission to the business address of ASMO. Submission should be in line with the requirements and instruction of this RFQ. Read the requirements and instructions below. 3. Prior Submission: To ensure good understandings of the work and specification of materials, design, quality, print, sizes and technical points, ASMO will organize clarification briefing in ASMO to provide bidders an opportunity for obtaining exact particulars of the work in evidence. Sample material will be supplied, question if any and related to particulars of the work, how the work to be delivered, what type of contract and payment method will be clarified. Interested bidding company willing to bid is required to submit a prior written request via to Khalid Ahmadzai at kahmadzai@asmo.org.af with cc Khyber Khishki at kkhishki@asmo.org.af Request should be sent no later than 03:00PM, 7 th /Mar/08. RFQ Contents: In order to simplify the evaluation process and obtain maximum comparability, ASMO requires that all responses to the RFQ be organized in the manner and the format described below:.. Executive Summary: Respondent company should include in the submission one page Executive Summary to state how and why the company is best and qualified to fulfill the requirement of the job. Description should include the intention of the company in submitting the quotation and the capability to perform the job. Also, Executive Summary should include company name, form (whether partnership or sole trader) formal business address, and contact details of prime persons. It should also include details for key personnel... Cost & Budget: To achieve easiest quote analysis, it is desirable to have price information quoted in the manner specified in this RFQ. It is acceptable therefore for the bidding company to quote price of materials directly using the annexure -6 of this RFQ. Price schedules should be filled & completed in the following manner. - Annexures comprises of several table, each for a single item with description, unit of measurement and quantity - Unit price per unit of measurement is requested. Currency is ideally in AFN (see note below) - Each line in the table to be totaled followed by total per table - Each table for this purpose is a single quote, therefor properly calculated, signed & stamped and dated - Any ink type pen except for pencil should be used in filling the price - Prices tables should not contain error or correction Note: If price quoted in USD and to be comparable, daily exchange rate will be used to covert the quotes for evaluation & analysis purpose. This RFQ including annexures along with Executive Summary made by bidding company form submission package. Therefore every single pages and documents supported the RFQ must bear signature, date and company stamp. 5. Submission Guideline: Quotation must be printed written in English or in Dari/Pashto and to be contained in envelop with proper sealing and marking. Envelop should include RFQ reference number, name and contact number of the submitting company. Quotations should be sent no later than 03:00PM, 3 th /Mar/08 and to the attention of Mr. Khalid Ahmadzai, Procurement Manager, at ASMO s main office and at the below given address. 3 P a g e : R F Q ( R e q u e s t f o r Q u o t a t i o n)

Evaluation/Selection Process: Responses (submission) received will be reviewed to determine responsiveness with respect to the requirements. Responses that have not met the requirements or determined to be nonresponsive to the requirements will be considered ineligible. In other words, any response failed to be prepared in accordance with instruction of this RFQ and failed to have required documents as follow will be considered as non-responsive and is regarded as disqualified submission. 6.. Eligibility Criteria for Reviewing the Submissions:. Executive summary and budget & cost. Other documents that includes: proof of past experience related to this job and supported with copy of achieved contracts 3. Color printed or copy of business licenses. Contact information of previous clients for whom your company worked Responsive submission will be reviewed and scored by the ASMO evaluation committee pursuant to the grading scale it creates. The criteria to evaluate and score responsive submission will include cost and past related experience. 6.. Markings: Proposals will be marked as per number of points achieved that comprises of cost and past experience. Cost effective proposal: 70 mark 70 mark allocated for cost. Cost will be deemed reasonable if it is competitive and is readily comparable. Unit price and total cost per single items will be included in evaluation. High level accuracy in calculation is to ensure. Error in calculation is subject to disqualification. Past experience: 30 mark Evidences of past experience should be included in the submission. Past experience must be related to the job and must be supported with copy of previously achieved contracts. ASMO will verify the concluded contracts directly and independently. Full mark to achieve depends on the number of job previously completed satisfactorily and significance of the contract. 7. Statement of Work (Scope): Statement of work for this purpose is completion of job that include supply and delivery of materials. Supply may include but not limited to materials, cost of labor, print and any other cost associated with completion and bringing the materials to present and workable condition. 5 Read Annexure # -3 of this RFQ for details of materials. 8. Required Documents: Certain documents as per following to support evaluation is mandatory to provide. Failure to do so, give rise to disqualification. - Executive Summary as mentioned above. - Proof of past experience evidenced by copy of contracts 3- Contac information of previous clients - Copy of business silence 9. Compliances Certain compliances and terms that include but not limited to the following will be incorporated in the contract or PO (purchase order). 9.. Executive Order on Terrorism Financing Contracting company expected to be engaged in contractual relationship with ASMO should be aware that U.S. Executive Orders and U.S. law prohibits transactions with and the provision of resources and support to, individuals and organizations associated with terrorism. It is the legal responsibility of the company representative to ensure strict and full compliance with these Executive Order and laws. 9.. Prohibited Source Countries For the purpose of this work, no goods/resources to be procured/supplied from countries that are classified by USAID as prohibited sources such as Cuba, Iran, Laos, North Korea, Sudan and Syria Payment/Tax Responsibilities: ASMO s term/condition for payment is in Afghani at predefined installments. Appropriate tax will be deducted for deposit to the Government Tax Authority. 9.. Anti-Human Trafficking ASMO is committed to a work environment that is free from human trafficking, which for purposes of this policy, includes forced labor and unlawful child labor. ASMO will not tolerate or condone human trafficking in any part of organization. This policy is consistent with ASMO s Code of Ethics and Business Conduct and our core values to protect and advance human dignity and human rights in our business practices. 6 constitutes trafficking in persons. Action involving, suspension and termination will be taken if vendor or contractor evidenced for non-compliant. Trafficking for this purpose includes but not limited to the following: Trafficking in persons shall mean the recruitment, transportation, transfer, harboring or receipt of persons, by means of the threat or use of force or other forms of coercion, of abduction, of fraud, of deception, of the abuse of power or of a position of vulnerability or of the giving or receiving of payments or benefits to achieve the consent of a person having control over another person, for the purpose of exploitation.
