1 Pre-Qualification Questionnaire : Provision of Architectural Services for the redevelopment of the Windermere Steamboat Museum To be completed by the architect leading the design team. A condition of teams being invited to Stage 2 (Design Concept Phase) of the Competition will be that the standard requirements of the Lakeland Arts Trust (in the areas of finance, insurance and health & safety) are capable of being met by the winning design team. The information provided by bidders will be assessed in a manner proportional to the objectives and requirements of this project. Each organisation that expresses an interest in the Contract and completes a PQQ (whether it is a single organisation or a consortium) is referred to as an Applicant. Where organisations intend to act jointly in seeking to be awarded the contracts (such as in a consortium bid), the participating organisations must identify one Lead Applicant. The Lead Applicant will be responsible for the overall preparation and submission of the PQQ on behalf of all members of the consortium and for addressing questions about the organisation or the joint organisation (an administrative lead for the purpose of the PQQ and the process beyond). A Consortium s Expression of Intrest response will be evaluated as a single entiity. The Trust does however require information about each of the members of any proposed consortium, and each Relevant Organisation is required to complete PQQ Sections 3,4,5, 7 and 9 and only the Lead Applicant should complete the other sections on behalf of all consortia members, ensuring all services and works are addressed 1. Compliance with EU / UK Procurement Legislation Please confirm that none of the circumstances specified in Regulation 23 of the Public Contracts Regulations 2006 apply to your organisation or to any member of your Consortium (if applicable). 3.1 Please confirm (by placing a cross [X] in the box) that the Company is NOT bankrupt, insolvent, subject to an administration order, or in the process of being wound up. 3.2 Please confirm (by placing a cross [X] in the box) that NONE of the Company s Directors or representatives has been convicted of conspiracy, corruption, bribery, fraud or money laundering. Each member of a consortia must complete the above. If one member of the consortia fails, the overall application will fail. 4. Financial 4.1 The following financial information is required to be provided for the lead consultant only. (a) A statement of the organisation s turnover, profit and loss and cash flow position for the 3 most recent full year of trading (or part year if full year not applicable) and an end period balance sheet, where this information is not available in an audited form at (a). Please note that full sets of audited accounts are NOT required. (b) Where (a) cannot be provided, a statement of the organisation s cash flow forecast for the current year and a bank letter outlining the current cash and credit facility position. (c) If the organisation is a subsidiary of a group, (a) is required for both the subsidiary and the ultimate parent. (d) The name and address of your Bank : Financial assessment will be based on assessment of the Applicants net worth and a Dunn & Bradstreet Ratings. Each member of any consortia will be assessed If the Applicant or a member of a consortia has a negative net worth, the application or the consortia as a who will be failed Note: The information provided will be subject to an assessment by Lakeland Arts Trust s Finance Director, CEO and Honorary Treasurer. Organisations that are new to the market or are less than two years old are required to submit other adequate information to demonstrate their economic and financial standing, but ensure that a credit check can be undertaken on their organisation. Where applicants do not currently have the required level of cover, they should append a letter from their insurance provider confirming that they would be able to obtain GBP 5m of cover should they be successful in winning the commission. References 6.1 As per the Preliminary Briefing Paper, your Expression of Interest document should give full client contact details (name, address, , telephone number) for the 3 projects used as examples of projects delivered by the lead architect within the previous 10 years. - the inclusion of a reference (if references taken up) which is required for the lead architect regarding the execution of their duties and the design services provided. Note: The Lakeland Arts Trust may elect to contact any of the given organisations for a reference. Your permission to do so will be assumed unless you explicitly state any objections. 7.
