2 H e a l t h c a r e S c i e n c e / P r e - M e d P r o g r a m HOSA JOURNAL This year each Healthcare Science/Pre-Med/HOSA Member will be responsible for keeping a journal. The journal will consist of an essay, a list of personal goals, and documentation of the various events and activities done throughout the year. The items must be placed in your portfolio. Your work must be typed neatly and legibly, with correct grammar and spelling. This is WORTH 3950 POINTS. PORTFOLIO CONTENTS I. Opening Essay How will I be a contributing member of HOSA? Guidelines: [200 points] 1. Essay will have 5 paragraphs [3-5 sentences in length] 2. Think of a statement that summarizes your answer to the question How will I be a contributing member of HOSA? 3. Think of 3 main points/examples that support your thesis. Each of these points you wish to make will be the main ideal of each paragraph in the body of the essay. 4. Write three main paragraphs [body] 5. Write an introduction paragraph, which will include your thesis [main idea/answer to question] 6. Write a conclusion paragraph. This should describe how the three points you made support your thesis. 7. Proof read grammar spelling and clarity!! 8. Rewrite and submit essay [At least one full page, no more than two.] II. III. IV. Person Goals: These are things that you hope to accomplish for yourself within the next year, as well as in the next five years. EXAMPLES- [100 points] 1. List short term goals [graduating high school, bring up GPA] 2. List long term goals [college, career] Documentation of events and activities You are to create a scrapbook documenting the activities you have done throughout the current year. You may be creative with documenting these activities. HINT: SCRAPBOOK at least five (6) pages not to include cover page. [600 points] 1. Must include title of activity example: American Red Cross Blood Drive, HOSA Class Debate 2. Date of Activity 3. You may use pictures, drawings, business card, flyer, or an essay; be creative to capture that activity. Interview of Nursing Home Resident [500 points] V. Complete 25 hours of community service [1250 points] 2

This is a guideline. You may use all of these questions and add some of your own. You may include a picture. Be sure to get permission from the individual and have them sign the picture release form. Hours may be accumulated at various acceptable places. You MUST send me an to kshelton2@mcpss.com at least one week prior to community service activity to get my approval AND include that along with your community service form in portfolio. FAILURE TO DO SO WILL RESULT IN DENIED COMMUNITY SERVICE HOURS for that activity. Any project performed for community service hours must meet the definition and guidelines as stated by the instructor. A student should complete the minimum of 25 hours of service for 9 th grader by the date assigned 25 hours of service for 10 th grader by the date assigned 50 hours of service for 11 th graders by the date assigned 50 hours of service for 12 th graders by the date assigned ALL community service MUST be pre-approved by the site supervisor before starting the service Community service can be performed after school, evenings, weekends and during spring/summer/holiday breaks The community service must be completed and documented before the portfolio is submitted by the student The student CANNOT profit monetarily or receive credit in another way for the service. Written verification of the completed hours is necessary for completion of the program. The service MUST benefit others The service CANNOT be something that is required by someone else Examples of Community Services Activities.
