DIRECTOR OF PUBLIC WORKS SUBJECT: RESOLUTION DECLARING FINAL ACCEPTANCE OF THE 2012 PAVEMENT MANAGEMENT PROJECT, NO. 670 Summary: Palp Inc. dba Excel Paving, has completed the 2012 Pavement Management Project, No. 670, in conformance with the plans and specifications to the satisfaction of the Public Works Department. Staff is recommending final acceptance of the work performed by Palp Inc. dba Excel Paving. Recommendation: Waive further reading and adopt the following resolution, entitled: A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SIGNAL HILL, CALIFORNIA, DECLARING WORK TO BE COMPLETED IN CONFORMANCE WITH THE PLANS AND SPECIFICATIONS FOR THE 2012 PAVEMENT MANAGEMENT PROJECT, NO. 670, IN THE CITY OF SIGNAL HILL, IN ACCORDANCE WITH THE CONTRACT ENTERED INTO BETWEEN PALP INC. DBA EXCEL PAVING, AND THE CITY OF SIGNAL HILL

2 Final Acceptance 2012 Pavement Management Project, No. 670 Page 2 Fiscal Impact: The final contract construction cost for the 2012 Pavement Management Project, No. 670, is $199,935. A total of $200,000 was allocated in the FY , Capital Improvement Budget for this project. Background and Analysis: On May 1, 2012, the City Council awarded a contract for the 2012 Pavement Management Project, No. 670, to Palp Inc. dba Excel Paving in the amount of $193, This project included sidewalk repairs, removal and reconstruction of failed pavement sections, replacement of concrete curb and gutter, cross gutter, and driveway, and installation of street traffic striping, pavement legends, and pavement markings. A total of $6, in additional work was authorized in accordance with contract unit bid prices for a final construction cost of $199,935. Palp Inc. dba Excel Paving, has completed the 2012 Pavement Management Project, No. 670, in conformance with the plans and specifications to the satisfaction of the Public Works Department. 3 RESOLUTION NO xxxx A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SIGNAL HILL, CALIFORNIA, DECLARING WORK TO BE COMPLETED IN CONFORMANCE WITH THE PLANS AND SPECIFICATIONS FOR THE 2012 PAVEMENT MANAGEMENT PROJECT, NO. 670, IN THE CITY OF SIGNAL HILL, IN ACCORDANCE WITH THE CONTRACT ENTERED INTO BETWEEN PALP INC. DBA EXCEL PAVING, AND THE CITY OF SIGNAL HILL WHEREAS, on the First day of May 2012, the City Council of the City of Signal Hill, California, entered into a contract with Palp Inc. dba Excel Paving, for the 2012 Pavement Management Project, No. 670; and WHEREAS, the Public Works Department has submitted its approval of the final quantities and cost figures. NOW, THEREFORE, the City Council of the City of Signal Hill, California, does resolve as follows: Section 1. That work required to be completed by said Contractor has been completed and that the total cost of said work is in the amount of $199,935. Section 2. That the work is hereby accepted and approved. Section 3. It is further ordered that a Notice of Completion be filed, on behalf of the Agency, in the Office of the County Recorder of the County of Los Angeles within ten (10) days of the date of this resolution. 5 RETURN TO: CITY ENGINEER CITY OF SIGNAL HILL 2175 Cherry Avenue Signal Hill, CA NOTICE OF COMPLETION NOTICE IS HEREBY GIVEN that the contract heretofore awarded by the City Council of the City of Signal Hill, to Palp Inc. dba Excel Paving, 2230 Lemon Avenue, Long Beach, CA 90806, that was the company thereon for doing the following work, to wit: 2012 Pavement Management Project, No. 670, in the City of Signal Hill, California, in accordance with the contract entered into between Palp Inc. dba Excel Paving, and the City of Signal Hill, California. That said work, located on real property owned by the City of Signal Hill, was completed by said company according to plans and specifications, and to the satisfaction of the City Council of the City of Signal Hill, and that said work was accepted by the City Council of the City of Signal Hill at a regular meeting held on the 6 th day of November 2012, by Resolution No. (see Exhibit A ) attached hereto and made a part hereof). That upon said contract, Rapp Surety, 120 Vantis Drive, Ste. 300, Aliso Viejo, CA 92656, was surety for Bond No given by Palp Inc. dba Excel Paving, as required by law. 1
