1 INSTRUCTIONS Annual Program Review Form and Instructions STUDENT AFFAIRS: CalWORKs PROGRAM & SERVICES San José City College The purpose of the Annual Program Review is to prepare a document to summarize the current status of a Student Service program. The Annual report should be a reflection of the previous year and a plan moving forward for the upcoming year. Use this reporting form for all of the following: To report the updates and major changes which have occurred in your program over the past year. Provide empirical data and discuss trends in data as a reflection of these changes. To report the PSLO/SAO assessment activities of the Program. Plan ahead for projected changes and request budgetary support for the next academic year. Program Description: This section should briefly address the function(s) of the program. Please include data or survey information where appropriate. Goals and Activities: This section should focus on program goals targeted this academic year. Information on program activities (types and frequency), measures of student participation, student success and evidence of service delivery should be included. PSLOs/SAOs Assessment Process: Provide a summary of the data collected that documents the progress that has been made in the development and assessment of (PSLOs) Program Student Learning Outcomes and (SAOs) Service Area Outcomes. Include data where applicable. Staffing Information (If you are requesting unstaffed positions, please include this information as an ongoing or a one-time expenditure in the Budget Information section) : This section should summarize current staffing in the program and anticipated staffing needs for the next academic year. Technology Information (If you are requesting technology, please include this information as an ongoing or a one-time expenditure in the Budget Information section): This section should summarize current technology use in the program and anticipated technology needs for the next academic year. Equipment information (If you are requesting equipment, please include this information as an ongoing or a one-time expenditure in the Budget Information section): This section should summarize current equipment use in the program and anticipated equipment needs for the next academic year. Please limit these projections to anticipated major expenditures.
2 Budget Information: This section should summarize the current budget for the program and anticipated budgetary needs for the next academic year. Please separate ongoing budget needs from one-time expenditures. Other: This section should address any changes in areas not noted above. This might include information on community needs, state regulations, and new initiatives with direct bearing on the program s activities.
3 Annual Program Review STUDENT AFFAIRS: CALWORKS PROGRAM & SERVICES - SAN JOSÉ CITY COLLEGE PROGRAM: CalWORKs PREPARED BY: Marilyn Brodie ADMINISTRATOR: Roland Montemayor EVALUATION YEAR (INDICATE ONE): ONE TWO THREE PROGRAM REVIEW SUBMISSION DATE: Program description CalWORKs is a program that helps eligible families become selfsupporting through temporary cash aid and assistance in finding and keeping a job. It is California s Temporary Assistance to Needy Families (TANF) Program and provides a number of services, including cash aid, child care, and job search guidance. When receiving CalWORKs cash aid, the family also receives Medi-Cal and may be eligible for CalFresh benefits formerly known as food stamps. The eligibility for the program can only be fully evaluated by an Eligibility Worker (EW) at one of our Social Services Agency Intake Offices and then referred San Jose City College. Once referred, they must maintain an active participation of hours per week in an approved activity such as vocational training, work study or volunteer positions on or off campus. Employment Services participants are assigned to activities meant to lead to a job and eventual self-sufficiency. Necessary supportive services, such as child care and transportation, will be provided. A CalWORKs student, parent or caretaker relatives are limited to a total of 48-months of cash assistance. This also applies to any months (from January 1998 on) in which the parent(s) received cash aid from any state. There are, however, certain exemptions to the 48-month lifetime limit. Once the CalWORKs 48-month lifetime limit is reached, unless an exemption is authorized, the adult will no longer be eligible for CalWORKs. However, the children may remain eligible.
4 PSLOs and/or SAOs Assessment process The CalWORKs Program developed PSLO s listed below and have assessed all of the PSLO s during the 2015/2016 academic year. The Program is finding the assessment of PSLO s are an ongoing process and constantly changes. New and continuing students will understand and comprehend the steps of matriculation at San Jose City College. Once a year CalWORKs staff conducts a file audit for all active students. The file must contain each of the documents reference below. The student must sign the orientation form indicating they understand the matriculation and orientation process. By signing a contract they must follow each step. 1. Ed Plan appointment/signature 2. TBA completion/signature 3. Childcare form completion/signature 4. Book Voucher completion/signature CalWORKs funding is dependent on passing the audit conducted by SJCC, SSA and State funders. Therefore, recommendations and our findings must match our PSLOs. Goals and Activities The CalWORKs Program goals for fiscal year are as follows: Continue strengthening the work study program and developing and securing sites for job placement, internships and volunteer positions for CalWORKs students. Continue creating new Distant Learning Programs and assignments to the meet the 20/35 required hours of participation per week, for times when school is not in session. Continue to work in cooperation with various campus departments and programs in student services, business services and academic affairs to enhance CalWORKs program needs to improve services. Staffing information Currently the CalWORKs Program consists of one full-time Academic Counselor, Hourly Program Assistant, a 12 hour a week Program Assistant and Director required by Santa Clara County Department of Social Services and State Chancellor s office grant compliance. CalWORKs interacts with other departments on
5 campus. A case management approach is set-up between all staff to maintain support services campus wide. It would be helpful if the program had funding for a job developer positions to assist in meeting program needs for job developing for work study positions. Technology information The CalWORKs Program is not in need of any additional technology at this time, but would like to be considered when new computers or technology are updated within the college Equipment Information No equipment is needed at this time, however the program will like to be consider when new equipment is provided by the college. Budget information (please indicate whether previous budget requests were met). Ongoing Budget Needs: CalWORKs funding includes county, state, federal, and district allocations. The budget for the CalWORKs Program during 2016/2017 Academic year was $529, 637 which was an increase from Currently, the budget is $534,146. In comparison to last fiscal year, the budget was an increase by $4,509. The budget covers salary, benefits, ancillary (travel and supplies), and work study positions for students during the entire year. The program anticipates restoration of funds for CalWORKs next year. However, grants are continuously being researched for additional dollars to assist and enhance the program services One-time Expenditures: Other: The CalWORKs Program is committed to improving its services and staying compliant with College goals and Santa Clara County Department of Social Services contract requirements.
