2 Original Sheet 2 A. Application 1. General a. The regulations specified in this Tariff apply to the Intrastate services and facilities furnished by the Communications Corporation of Indian, hereinafter referred to as the Company, subject to the jurisdiction of the Regulatory Commission. 1) Failure on the part of the customers to observe these regulations of the Company, after due notice of such failure, automatically gives the Company the privilege to discontinue the furnishing of service. b. The regulations or conditions contained in the Local Exchange Tariff shall govern in the event of a conflict between these general regulations and any conditions applicable to the General Exchange Tariff and the Local Exchange Tariff. c. These tariffs cancel and supersede all other tariffs of the Company issued and effective prior to the effective dates shown on individual sheets of this Tariff. d. The regulations of the Company apply to the service(s) furnished by it when services and/or facilities are provided in part by this Company and in part by other companies. e. The customer is required to pay all charges for exchange service and for toll messages in accordance with provisions contained elsewhere in this Tariff. Bills for local exchange service are payable monthly in advance. Bills for toll service are payable in arrears. The customer is responsible for all charges for telephone service rendered at his telephone, both exchange and toll, including charges for toll messages on which the charges have been reversed.

3 First Revised Sheet 3 Cancels Original Sheet 3 B. Obligation and liability of the Company 1. Availability of Facilities a. The Company's obligation to furnish exchange and toll telephone service is dependent upon its ability to secure and retain, without unreasonable expense, suitable facilities and rights for the construction and maintenance of such facilities. 2. Liability of the Company a. Allowance for Interruption 1) The Telephone Company cannot guarantee the uninterrupted working of its (C) services and facilities. In the event of an interruption, which is not due to the negligence or willful act of the customer, a credit may be provided for the time service(s) were interrupted. (C) b. Adjustment of Billing 1) The Company will make an adjustment for charges overbilled or underbilled as follows: a) For local exchange service, an adjustment may be made to the known date of error or for a period of one year whichever is shorter, when such amounts can be determined. (1) Arrangements for the customer to pay any adjusted amounts fixed under this regulation will be a matter of mutual acceptance between the Company and the customer. Service will not be denied or discontinued for a failure of the customer to pay this amount. b) For disputed toll message charges, an adjustment may be made, subject to investigation, when the customer notifies the Company of such adjustment requirement within 45 days of the billing for such charges. The disputed toll charges may be rebilled after investigation. c) For other billing errors or tariff applications, adjustments may be made to the known date of error or for a period of one year, whichever period is shorter. Transmitting Messages 1) The Company does not transmit messages, but offers the use of its facilities, where available, for communications between parties, subject to the regulations and conditions specified in this Tariff. d. The customer indemnifies and saves the Company harmless against the following: 1) Acts or omissions of other companies when their facilities are used in connection with the Company's facilities to provide service. 2) Any defacement or damage to the customer's premises resulting from the existence of the Company's facilities on such premises, or from the installation or removal thereof, when such defacement or damage is not the result of negligence of the Company or its employees. 3) Any accident, injury or death occasioned by its facilities when such is not due to negligence of the Company. 4) Claims for libel, slander or infringement of copyright arising from the material transmitted or recorded over its facilities; claims for infringement of patents arising from combining with, or used in connection with, facilities of the Company, apparatus and systems of the customer; and against a" other claims arising out of any act or omission of the customer in connection with facilities provided by the Company. 5) Liability for failure to provide service. 6) Liability for telephone directories is covered elsewhere in this section under "Telephone Directories." 7) Any damage, injury, or death resulting from the existence of Company provided explosive atmosphere equipment on such premise, when such damage, injury or death is not the result of negligence of the Company or its employees. 5 Original Sheet 5 C. Use of Service and Facilities 1. Customer Service a. Customer service, as distinguished from public and semi-public or coin-box service, is furnished only for use by the customer, his family, employees or representatives, persons residing in the customer's household, or guests of the customer, except as the use of the service may be extended to: 1) Joint Users. 2) Guests and tenants of hotels, motels, hospitals, apartment houses and apartment hotels. 3) Member of a club. 4) Persons temporarily subleasing a customer's residential premises. 5) Patrons of the customer and the public in connection with automatic dialing equipment arranged for the origination of telephone calis only to prerecorded or preset telephone numbers. 6) The public in connection with municipal emergency fire and police reporting stations. 7) Customer-Owned, Coin-Operated Telephone Services. 2. Telephone Numbers a. The customer has no property right in the telephone number or any right to continuance of service from any specific central office; and the Company may assign or change the telephone number, the central office designation, or both, as is necessary in the conduct of its business. 3. Tampering with Facilities a. The Company may deny telephone service to any person(s) whose facilities owned by the Company shows evidence of tampering, or use of any device whatsoever, for the purpose of obtaining telephone service without payment of the charges for the service rendered.

The customer agrees to notify the Company promptly whenever alterations or new construction on premises owned or leased by him necessitates changes in the Company's facilities; the customer agrees to pay the Company's current charges for such changes. 5. Misuse of Company-Provided Services a. The Company may deny telephone service to any person(s) who, over the facilities furnished by the Company, uses, or permits to be used, foul, abusive, obscene or profane language, or in a manner which could reasonably be considered frightening to others. 6. Use for Unlawful Purposes a. The service is furnished subject to the condition that it shall not be used for unlawful purposes. 7. Resale of Service a. The above described telephone service and facilities, rnay not be resold or otherwise used for perforrning any part of the work of transmitting, delivering or collecting charges for any message where any toll or other consideration has been or is to be paid to any party other than the Telephone Company, without the written consent of the Telephone Company. 8. Maintenance and Repairs a. All costs associated with the maintenance and repair of facilities furnished by the Company will be borne by the Company, except as specified elsewhere in this Tariff. b. The Company will be reimbursed for any loss or damage to its facilities on the customer's premises resulting from intentional destruction or any other cause, except from fire or unavoidable accidents.

7 Original Sheet 7 C. Use of Service and Facilities (Continued) 9. Right of Ingress/Egress a. All facilities furnished by the Company, on the premises of a customer, are the property of the Company. The Company's agents and employees shall have the right to enter said premises at any reasonable hour for the purpose of installing, inspecting, maintaining or repairing the facilities or for the purpose of making collection or upon termination of the service or for the purpose of removing such facilities. 10. Unusual Installation Costs a. Where special requirements of the customer involved unusual construction or installation, the customer may be required to pay additional costs as provided elsewhere in this Tariff. (See "Special or Experimental Assemblies of Equipment.")

8 Original Sheet 8 D. Application of Business and Residence Rates 1. Business Rates Apply at the Following Locations: a. In offices, stores, factories, mines and all other places of a strictly business nature. b. In boarding houses (except as listed under D. 2.), offices of hotels, halls and apartment buildings, quarters occupied by clubs or lodges, public, private or parochial schools or colleges, churches, hospitals, libraries and other similar institutions (does not include dormitory rooms at schools and colleges). c. At residence locations when the customer has no regular business telephone and the use of the service, either by himself, members of his household or his guests, or parties calling him, can be considered as more of a business than of a residence nature. This may be indicated by advertising either by business cards, newspapers, handbooks, billboards, circulars, motion picture screens, or other advertising media, such as on vehicles, etc. When such business use is not such as commonly arises and passes over to residence telephone during the intervals when, in compliance with the law or established custom, business places are ordinarily closed. d. At residence locations where an access line is located in a shop, office or other place of business. e. In a residence where business and residence are in the same premises and service is not installed in the business. f. In any location where the listing of service at that location indicates a business, trade or profession, except as specified under D. 2. I I i ~ U! I r I' i

In a private residence where business listings are not provided. b. In private apartments of hotels, rooming houses or boarding houses where service is confined to the customer's use, and elsewhere in rooming and boarding houses which are not advertised as a place of business or which have less than five rooms for roomers or which furnish meals to less than ten boarders, provided business listings are not furnished. c. In the place of residence of a clergyman or nurse, and in the place of residence of a physician, surgeon or other medical practitioner, dentist or veterinarian, provided the customer does not maintain an office in the residence. d. In college fraternity or sorority houses where individual-line service is provided.

10 Original Sheet 10 E. Establishment and Furnishing of Service 1. Application for Service a. Applications for service may be made orally or in writing. (1) These applications become contracts upon the establishment of service. Applicants for service are responsible for payment of an amount estimated equal to one month's service billing and service connection charges (if applicable). The terms and conditions specified for such contracts are subject to these General Regulations, the General Service Tariff and Local Exchange Tariff for the exchange from which service is to be furnished. b. Any change in rates, regulations or conditions prescribed and approved by the Utility Regulatory Commission shall act as a modification of the contract to that extent without further notice. c. Requests from customers for additional service may be made orally. A move from one geographic location to another within the same exchange is not considered to terminate the contract; orders for such changes may be made orally. 2. Establishing Credit a. The Company, in order to assure the payment of its charges for service, will require applicants and customers to establish and maintain credit. b. The establishment or reestablishment of credit as provided in this section shall not relieve the applicant or customer from compliance with other provisions of this Tariff, as to advance payments and the payments of bills and in no way shall modify the provisions regarding disconnection and termination of service for failure to pay bills due for service furnished. (1) For application involving Customer-Provided Equipment, See Section 7. Effective: May 5,2010

(Continued) 1) Applicants The Company may refuse to furnish service to an applicant who has not established creditworthiness or has not paid charges for service (residence or business) previously furnished by the Company at the same or another location, until arrangements suitable to the Company have been made to pay such charges. a) Applicants for business service may establish their credit-worthiness in one of the following ways. (1) Demonstrating to the satisfaction of the Company that his credit is acceptable by providing information which is pertinent to the applicant's credit standing. (2) Payment of cash deposit. b) Applicants for residential service will be determined credit-worthy solely on the credit risk of the individual and in an equitable and nondiscriminatory method, without regard to the sex, marital status, and the collective credit reputation or economic character of the area wherein he resides. Each new applicant for residential service shall be deemed credit-worthy and shall not be required to make a cash deposit if he satisfies the following criteria. (1) If the applicant has been a customer of the telephone company within the past two years and: (a) (b) (c) Owes no outstanding bills for service rendered by any other telephone utility. Did no have more than two bills which were delinquent to any telephone utility during the last twelve consecutive months that service has been rendered for a period of less than twelve months, and has not had more than one delinquent bill in such period. Within the last two years did not have a service disconnected by a telephone utility for nonpayment of a bill rendered by that utility. Effective: May 5,2010

Establishing Credit (Continued) b. 1) b) (Continued) (2) If the applicant has not been a customer of a telephone utility during the previous two years and who meets the following criteria: Either the applicant: (a) (b) (c) Has been employed by his present employer for less than two years. Has been employed by his present employer for less than two but the applicant has been employed by only one other employer during the past two years. Has been employed by the present employer for less than two years and has no previous employment due to having recently graduated from a school, university, vocational program, or has recently been discharged from military services. And the applicant either: (d) Owns or is buying his or her home, or is renting a home or apartment and has occupied the premises for more than two years. (3) The applicant may provide a written guarantee of payment, Signed by a third party guarantor acceptable to the Company, for all telephone service rendered or to be rendered to the applicant. The guarantor may terminate the guarantee upon thirty (30) days written notice. The guarantee shall be in full force and effect, to and including the date the guarantee shall terminate, and the guarantor shall be obligated, as provided in the written guarantee, respecting the payment of the amount of the applicant's bill on the date of termination. Establishing Credit (Continued) b. 1) Applicants (Continued) c) Prior to the establishrnent of an applicant's credit, the Company, at its option, rnay permit service to be installed upon the advance payrnent by the applicant of an amount equal to applicable service charges and initial nonrecurring charges applicable for service installation plus the estimated amount of the applicant's rnonthly bill for service. Such advance payment will be credited to the applicant's service account but does not relieve the applicant of his responsibility to subsequently establish credit in accordance with 1) b), preceding, or 2. c., following. d) If credit is not so established, the Company may disconnect the service not sooner than eight days after mailing or delivery of written notice of its intention to disconnect. The customer may be required to make a reasonable cash deposit, or an additional one in cases where a deposit has been made, or provide the Company with a written guarantee in accordance with E.2.b. 1) b) (3), when the customer has been mailed disconnect notices two consecutive months or any three months within the preceding twelve month period. If an additional deposit or a guarantee is required, a written notice will be prepared and presented to the customer giving him then days to make the deposit or provide a guarantee. b) A present customer may also be required to reestablish his credit. For a present customer, who ahs not had service long enough to meet the criteria set out in paragraph a) above, a deposit, an additional deposit, or a guarantee may be required if toll usage exceeds the estimated toll usage that was considered by the Company when service was initially provided. For a present customer for whom no deposit has been required, a deposit or a guarantee may be requested if monthly toll usage exceeds three times the charge applicable to the customers basic exchange service. c) If a customer fails to reestablish his credit as required by the Company, his service may be disconnected not sooner than ten days after mailing or delivery of written notice of intention to disconnect. If the service has been disconnected, the deposit or guarantee must be provided before service will be reconnected.

Cash Deposits 1) Amount of Deposits a) If an applicant fails to establish his creditworthiness as outlined in F. 2. b. 1), the applicant may be required to make a reasonable cash deposit. The amount of a deposit which a customer or applicant may be required to pay to the Company as a means of establishing credit shall not exceed an amount equal to the estimated one-sixth (1/6) annual billing for local service and toll charges. The determination of a customer's estimated monthly bill shall be based upon his prior or current use of the class of service involved (monthly service and message toll charges). b) The amount of a deposit may be adjusted on the basis of 1) a) above, at the option of the Company at any time when the character or degree of the customer's use of the service has rnaterially changed, or when it develops that the character or degree of such use will materially change and such change is not temporary. c) The fact that a deposit has been made shall in no way relieve the applicant or customer from complying with the Company's regulations as to advance payments and the prompt payment of bills on presentations, nor constitute a waiver or modification of the regular practices of the Company providing for the discontinuance of service for nonpayment of any sums due the Company for service rendered. The Company may discontinue service to any customer failing to pay current bills without regard to the fact that such customer has made a deposit without regard to the fact that such customer has made a deposit with the Company to secure payment of such bills or has furnished the Company with a guarantee in writing of such bills.

Cash DepOSits (Continued) 1) Amount of DepOSits (Continued) d) The Company will provide a written receipt at the time the customer pays his deposit in full or in partial payments. e) The Company will maintain a record of each customer deposit that has been collected and the record will indicate as follows: (1) The name of the customer (2) The customer's current address, if account is active (3) The amount of deposit (4) The date the deposit was made (5) A record of each transaction affecting the deposit 2) Interest to be Paid on Deposits Interest at the rate of six percent (6%) per annum shall be paid by the Company on cash deposits made by customer for the purposed of establishing credit if they are held for more than a twelve (12) month period. The deposit shall not earn interest after it is mailed or personally delivered to the customer, or otherwise lawfully disposed of in attendance with the laws of the state. Simple interest shall be computed from the date of payment of the deposit and shall be paid to the customer in one of the following ways: a) By credit to the customer's account annually. b) By payment, no more than once in any twelve (12) month period, when requested by the customer. c) By adding the accrued interest to the amount of the deposit at the time such deposit is refunded or applied to an unpaid bill of the customer in accordance with F. 2. C. 3), next page.

Cash Deposits (Continued) 3) Refund or Application of Deposits a) The Company will promptly refund cash deposits along with a statement accounting for each transaction involving the deposit and interest without request of the customer when one of the following conditions are met. (1) upon satisfactory payment by the customer for a period of twelve consecutive months, or ten out of any twelve consecutive months, providing that the customer did not make late payments for any two consecutive months. Refund of deposits will be made to customers in the same manner as set forth in F. 2. c. 2). (2) The customer establishes his credit by another means as previously set forth. (3) Service is terminated at the request of the customer, the deposit and accrued interest will be applied to the final bill. Upon special request, the Company may refund the deposit and accrued interest within fifteen days after payment of the final bill. (4) An applicant cancels his application for service and any charges incurred are paid in full. b) When the service is terminated or the application is canceled and there are charges due to the Company, the deposit plus interest will be applied to the charges, and the balance, if any, will be returned to the customer, applicant or person making such deposit. The customer will be rendered a net bill on or about the same date each month and will show at least the following information. (1) Customer's name and billing address (2) Telephone number (3) Date of Bill (4) Total local service charges (if measured service, shall include the number of additional calls and the rate and total amounts of charges) (5) Itemization of toll calls and charges (6) State and Federal Taxes (7) Previous Balance (8) Explanation of other charges and credits (9) Explanation of codes and abbreviations (10) The past due date or the date on which the bill becomes delinquent (11) Total amount of the bill t

The customer is responsible for the payment of charges for all services furnished including, but not limited to, calls originated or accepted at a customer's service location. c. Bills are due when rendered and may be paid at any of the Company's business offices or other authorized payment locations. d. Failure to Pay Charges for Service. (1) Regular Monthly Bills (a) (b) A residence customer's monthly service bill which has remained unpaid for a period of more than seventeen (17) days following the day the bill is postmarked, if mailed, or delivered by other means, shall be considered a delinquent bill. A business customer's monthly service bill which has remained unpaid for a period of more than seventeen (17) days following the day the bill is postmarked, if mailed, or delivered by other means, shall be considered a delinquent bill. (2) Late Payment Charges (a) (b) (c) A Late Payment charge of 1.5% per month applies to all past due balances. Customers with past due balances that sign up for electronic payments will receive a one-time waiver of the late payment charge. Final collection procedures, temporary disconnection of service, and the requirements for deposit are unaffected by the application of a late charge. The late payment charge does not extend the time for payment or otherwise enlarge or change the rights of the customer. Notice of intention to pay late will not avoid this charge. Failure to pay Charges for Service (Continued) (3) Disconnection for Nonpayment (a) (b) When a customer is delinquent in the payment of a regular monthly bill, the Company may disconnect the service five (5) days after mailing or personal delivery of written notice to the customer or a responsible member of his household, of intention to disconnect. The disconnect notice will not be rendered prior to the date on which the account becomes delinquent. The disconnect notice will be in language which is clear, concise, and easily understandable to a layman and shall state in separately number large types of printed paragraphs: 1) The date of the proposed disconnection 2) The specific actual basis and reason for the proposed disconnection 3) The telephone number of the Company's business office where the customer can call during regular business hours in order to question the proposed disconnection or seek information concerning the customers rights (c) The Company will postpone the disconnection of service for then (1) days if prior to the disconnect date specified on the notice the customer provides the Company with a medical statement from a licensed physician or public health and immediate threat to the health and safety of a designated person in the household of the customer. The postponement of disconnection shall be continued for one (1) additional ten (10) day period upon the provision of an additional medical statement.

Failure to pay Charges for Service (Continued) (3) Disconnection for Nonpayment (Continued) (d) The Company will not disconnect a customers service: 1) If a bill applicable to another residence or location is not delinquent as defined in paragraph F. 3. D. 1) 2) For failure to pay for service to a previous occupant of the premises being served unless the Company has good reason the customer is attempting to defraud the Company by using another name. 3) On the basis of the delinquent character of an account of any other customer except if the other customer is the guarantor of his or her account for telephone service. 4) If the customer shows just cause for his or her inability to pay full amount due, and the customer: a) pays a reasonable portion of the amount due equal to $20.00 or 25% whichever amount is greater and; b) agrees to pay the balance of all amounts due in three equal installments and; c) agrees to pay all undisputed future bills for current service as they become due and; d) has not breached any similar or previous agreements with the Company regarding to the above paragraph provided the Company will add the late payment charge to the outstanding bill and the terms of the agreement will be put in writing by the Company and signed by the customer and a representative of the Company.

Failure to pay Charges for Service (Continued) (3) Disconnection for Nonpayment (Continued) (d) Continued 5) If a customer is unable to pay a bill, which is usually large due to prior incorrect billing, application of rates, human, or mechanical error, and the customer a) pays an amount equal to the customers average bill for the twelve (12) monthly bills immediately proceeding the bill in question; and b) agrees to pay the balance of all amounts due in three (3) equal monthly installments, and c) agrees to pay all undisputed future bills for current service as they become due, provided, however, that the Company may not add any late payment charge to the outstanding balance, and the terms of the agreement will be put in writing by the Company and signed by the Customer and a Representative of the Company. (e) The Company will normally disconnect service for nonpayment only between the house of 8:00 A.M. to 3:00 P.M., but will not disconnect service at all on any day which the Company business office is closed to the public, or after twelve (12:00) Noon of the day immediately preceding any day on which the Company's business office is not open to the public.

Restoral of Service (1) If a customer's service is restored after having been disconnected in accordance with this Tariff, but a Company service order to terminated such service has not been completed, the customer will be required to pay the reconnection-of-service charge specified in Section 9 of this Tariff. Monthly service charges will not apply for the period during which the customer's service is disconnected. (2) When a customer's service has bee disconnected in accordance with this Tariff and the service ahs been terminated through the completion of a company service order, service will be re-established only upon the basis of an application for new service. (3) If service is temporarily suspended for nonpayment of charges, service will be restored upon payment of all charges due. (4) The Reconnect charge as specified in Section 9, may be collected or billed on the customer's next statement, at the option of the Company. f. Non-Sufficient Funds (NSF) (1) A charge will be assessed when a customer's payment for service or deposit becomes dishonored, is not valid, or is rejected. Distribution and Content (1) The Company will fumish the customer one (1) copy of its directory, containing white page listings for all published telephone numbers within that customer's calling area, for each access line or trunk. Additional directories will be provided at a charge set forth in Section 5 of this Tariff. The directory will normally list the name, address and telephone number of all customers located in the serving exchange, except the non published and non listed telephone number omitted at the customers request, and will be revised at least once each fifteen (15) month, except when known. Service changes may require rescheduling of the directory issue date. The Company will provide one (1) copy of each new directory issue to the Public Service Commission. (2) The front pages of the directory will also include the following: (a) (b) (c) The Company name. The exchange areas that is included in the directory. Emergency number, i.e., police & fire. (3) Directory information will also be made available to the directory assistance or intercept operators (except numbers non listed and nonpublished at the customers request). New or changed listings will normally be provided to the directory assistance and intercept operators within two (2) normal working days after the connection or change of service excluding Sundays and Holidays. b. Ownership and Use (1) Directories furnished to customers remain the property of the Company and are provided to customers as an aid in the use of the telephone service. The company reserves the right to charge for directories issued in replacement of directories. No auxiliary cover, except such as may be provided by or authorized by the Company, shall be used on any directory furnished by this Company.

26 Original Sheet 26 F. Termination of Service 1. By the Company a. The Company may deny or discontinue service without request by the customer, provided suitable notice has been given to the customer as required: (1) For failure to pay all charges due the Company, as outlined in paragraph F. 3. d. 3., of this tariff. b. The Company may deny or disconnect service without request by the customer and without prior notice only: (1) If a condition dangerous or hazardous to life, physical safety or property exists; or (2) Upon order by any court, the Commission, or other duly authorized public authority; or (3) If fraudulent or unauthorized use of service is detected and the Company has reasonable grounds to believe the affected customer is responsible for the tampering; or (4) If the Company's service is used in a manner disruptive to the service of other customer's; or (5) In extraordinary circumstances where unlimited access to the toll network may result in substantial loss of revenue of the Company. In such cases, reasonable efforts will be undertaken to discuss such circumstances with the customer. Effective: May 5,2010

27 Original Sheet 27 F. Termination of Service 2. At Customer's Request a. The Company will discontinue service and remove it's facilities upon request of the customer; and (1) The customer shall notify the Company at least three (3) days in advance of the day disconnection is desired. The customer shall remain responsible for all service used and the billing therefore until service is disconnected pursuant to such notice. (2) After requested by the customer, the Company will required to disconnect the service within three (3) working days of the requested disconnection date or the customer will not be liable for any service rendered to that address expiration of the three (3) day period. (3) The customer shall not charge service or authorize the charging of service to any account which has been disconnected at the customer's request or otherwise. A customer shall be responsible for any service he or she charges or authorizes charged to such an account in violation of the prohibition in the foregoing sentence.

28 Original Sheet 28 F. Termination of Service 2. At Customer's Request (Continued) b. Contracts for service may be terminated prior to the expiration of the contract period provided advance notice is given to the Company and upon agreement to pay all charges due for the service furnished plus any termination charges which might be applicable. c. Where a contract for service with a one month minimum period is canceled before establishment of the service is completed, a charge not to exceed the service charge specified is applied if all or a portion of the facilities has been installed. d. No minimum or termination charge will apply (unless otherwise stated specifically in this Tariff) where a new customer takes over the service of the former customer, provided the service is to be furnished at the same location without interruption and that the new customer assumes all unpaid charges on the original contract. Minimum and termination charges will apply for any service furnished under the original contract which is not retained by the new customer. e. No minimum or termination charge will apply in the event the service is terminated because of condemnation, destruction or damage to property by fire or other cause beyond the control of the customer. G. Minimum Contract Periods 1. Except as specified elsewhere in this Tariff, the minimum contract period is one (1) month from the date service or additions to service are established and the minimum charge is the authorized rate for one (1) month. For purposed of rate administration, each rnonth is considered to have thirty (3) days. 2. The Company may require a contract period longer than one (1) month at the same location in connection with special types or arrangements of service or for unusual construction necessary to meet specific demands for service. If a contract period is longer than one (1) month, the terms of the contract will be put in writing and signed by the customer and representative of the Company.

33 Original Sheet 33 I. Taxes or Fees to be Billed to Customers 1. General a. When a municipality or political subdivision imposes upon the Company and license, occupation, franchise, permit, inspection or other similar tax, such tax, fee or charge shall be billed to the telephone customers receiving service within the municipality or political subdivision, allocated uniformly on the basis of each such customer's monthly charges for the types of service made subject to such tax or fee or charge. J. Recording of Two-Way Telephone Conversations 1. General a. Telecommunications services are not represented as adapted to the recording of two-way telephone conversations. However, customer-provided recording equipment may be connected, directly, acoustically or inductively with telecommunications services, subject to the following conditions: b. Either a distinctive recorder tone that is repeated at intervals of approximately fifteen seconds is required when recording equipment is in use and is electrically connected with services of the Telephone Company or; a consent to record is required prior to the recording or; by verbal notification which is recorded at the beginning, and as part of the call, by the recording party. The consent must be in writing or be part of the recording. These will not be required: c. Neither tone or consent are required: (1) When used by a broadcast licensee provided at least one of the following requirements is met: (1) the licensee informs each party to the call if its intent to broadcast the conversation (2) to broadcast the call, or (3) such awareness of the licensee's intent to broadcast the call may be reasonably imputed to the party. (2) When used by the United States Secret Service of the Department of Treasury for recording of two-way conversations which concern the safety and security of the person of the President of the United States, member of his immediate family or the White House and its grounds.

34 Original Sheet 34 J. Recording of Two-Way Telephone Conversations 1. General c. Neither tone or consent are required: (Continued) (3) When used for recording at United States Department of Defense Command Centers of emergency communications transmitted over the Department of Defense's private line system when connected to telecommunications services. (4) When used for recording patently unlawful purposes such as bomb threats, kidnap, ransom requests, obscene telephone calls and outgoing calls made in immediate response to such calls. (5) When used for incoming calls to the telephone number publicized for emergencies involving health or safety of life and property and outgoing calls made in immediate response. (6) When used with calls made by Federal, State or Local law enforcement authorities or federal intelligence authorities and under color of law. (7) When used by the United States Nuclear Regulatory Commission of the Department of Energy with respect to the telephone systems located at its Operations Center for recording of two-way telephone conversations. d. If the Company is required to investigate alleged non-consensual recording which disclosed a conflict between the parties to the conversation on the matter of consent, the recording party will have to provide taped or written evidence that consent was obtained.
