Box Stateline, NV Dear Ms. Jordan: This is being sent in response to vour letter of February 24, 2011 as well as the various s that have been sent by counsel working on your behalf. I wish to summarize below each account you have had with J.P. Morgan Private Bank, what we have provided to you, your counsel and your bookkeepers concerning the account and its current status. We believe that we have offered the relevant documents for these accounts that can be distributed externally under our existing policies and procedures. Should you or your counsel seek additional documentation or information, we will expect you to serve us with a court order to that effect. Extension of credit--in 2004 we provided an unsecured bridge loan (Account No ) to you and your former husband, Raymond A. Mirra. Upon receipt of property appraisal, that loan was converted to a home equity line of credit secured by the real estate located in Virginia (Account NO in March 2005 and converted to Account No in January 2008 due to system conversion). A draw down of funds pursuant to this credit would have been referred to as a "Prime Advance Loan" on account statements. In 2008, documentation was executed whereby Mr. Mirra and two related limited liability companies assumed the obligations for the credit outstanding. You in your individual capacity no longer have any debt obligation due to J.P. Morgan Private Bank pursuant to these loan documents. We have provided you with copies of the Amendment to Promissory Note dated November 30, 2004, the Home Equity Une of Credit Agreement and Disclosure Statement dated March, 2005, related Home Equity Line of Credit documentation and the Deed of Trust and Note Assumption Agreement dated April 24, Account statements for the loan A/C (A/C as of January 2008) for the period March 2005 through May 2008 also have been provided. Asset with brokerage account~in 2004 an asset with brokerage account (Account No. Q ) in the joint names of and Raymond A. Mirra was established. We have provided you with copies of the account opening documents for that account as well as account statements for the period June 2004-December Copies of statements are to be sent to c/o Jeanette Walsh, 12 Warren Drive, Syosset, NY going forward.

2 Personal checking account (Account # ) --in June 2006, a personal checking account in the name of was established. We have provided you with copies of all checks and statements for that account and statements continue to be sent to c/o Jeanette Walsh, 12 Warren Drive, Syosset, NY As regards the latter two accounts, due to their size and lack of activity, we believe it appropriate that they be terminated. The asset with brokerage account holds approximately ~7, in cash deposit sweep and the checking account currently holds ~;0.48. We require executed directions from both you and Mr. Mirra to close the asset with brokerage account and direct where the assets in the account are to be transferred. We have informed Mr. Mirra of this request and he has indicated that he intends to release any claim to those assets. We would appreciate receivinl~ vour directions as to where they should be transferred no later than May :15, 20:1:1. The personal checking account is solely in your name and thus we only need written directions from you as to where the funds should be transferred. If we do not receive those written directions within 45 days from the date of this letter, we will proceed with closing the account and send you a check in the amount of the outstanding balance. Dimon and Cutler: March 30, 2011 I am writing in regard to loans, mortgages, brokerage accounts, and bank accounts established in my name () and approved by JP Morgan Chase Bank spanning back as far as I have exhaustively pursued attempts by telephone, in writing, personally, and through my attorney to obtain account information from the Washington, D.C. office of the Private Banking Division, which appears to be the initiation point of the banking relationship for many if not most of the aforementioned accounts, in particular, from a Ms. Rose Cohen, whose name appears as private banker on many of the statements I have been able to obtain, and her manager, Mr. David Lawson. Ms. Cohen and Mr. Lawson have been extremely evasive, particularly unhelpful, and recalcitrant in providing complete and accurate records in regard to the accounts and multiple requests as mentioned. See the attached letter recently received from Ms. Cohen. Recently, my file was forwarded from Ms. Cohen s office to Ms. Yomboro s office (Executive Specialist, Chase Home Lending). I hoped that Ms. Yomboro from executive offices would be more helpful in providing the information. Unfortunately, based on the attached letter from Ms. Cohen, it seems that I am back to square one. Most of the accounts/loans were opened in my name jointly with another individual (Mr. Raymond A. Mirra, Jr.), which I was unaware of until recently. I have received, as a result of my queries to Mr. Lawson and Ms. Cohen, sporadic and incomplete records in response to my

4 very detailed requests. I have also attempted on numerous occasions to contact JP Morgan Chase representatives through creditor contacts listed on my credit report. Some of the documents received, including account applications bearing my name for mortgages, brokerage accounts, etc., appear to bear inauthentic signatures representing my authorization for said account applications. I should mention here that Mr. Mirra is well known to me as an exbusiness associate as well as known to Ms. Cohen and Mr. Lawson. I am contacting your offices and requesting that you address the questions raised in my letters to Ms. Yomboro dated March 24, 2011 and March 21,2011, (see attached) in an efficacious manner. Upon further inquiry and analysis of documents received this far it appears that the loan number 98**** noted in my letter to Ms, Cohen and Mr, Lawson is a home equity Iine of credit from JP Morgan Chase Bank, N.A. bearing the loan number This line of credit is for 4 milson dollars. According to Rose Cohen, JP Morgan has released me from any obligation for this loan, however it still appears as open on my credit report. In regard to this loan (number ); line of e~edit/loan number ; line of credit/loan number ; Ms. Cohen has asserted that these all reflect the same home equity line of credit. I believe these relate to the attached internal loan appraisal documents. I reiterate my request for all application documents and all due diligence documents JP Morgan received in evaluating this loan. See attached. I have Chase statements reflecting a home equity line of credit under account number of 4 million dollars beginning in March 2006 through January Beginning in February of 2008 the.statements continue under account number through May All documents regarding a home equity line of credit begin in June of 2004 and I need the missing statements from the period of June 2004 through March 2008, and June 2008 through current. I have been told that my name is not on the statements post May of 2009 but I have received documentation that that is not the ease. Please provide the statements as requested.- Lastly, Ms. Cohen at first asserted that the JP Morgan Asset Account for account number Q JP Morgan Private Bank, was merely an asset account and "was not associated with a line of credit". I asked her repeatedly about the "Prime Advance Loan" number (see attached) reflecting approximately $2,500, in loan advances from the period June 2004 to December Cohen repeatedly insisted that

11 these transactions represented "a draw down," avoiding references to a loan in association with After much verbal tarrying, wherein I insisted that large amounts of money were clearly being extended as credit on a loan ( ) on the Q account statements and that there had to be account opening documents associated with this loan she then shifted her position saying t~iat there were no loan application documents that instead (in contradiction to her many assertions that this was not a "loan" previously) the loan was extended via the home equity line(s) (account numbers ; ; and ) all of which she maintains are the same home equity line of credit. The asset account openk~g documents for account number Q wherein the "draw downs" or amounts loaned and identified as prime advance loan transactions show that under "Section B Line of Credit" in the box "do you request a line of credit?", "no" was selected. (See attached.) I need the account opening documents for the "Prime Advance Loan" and to understand how_ and i~this loan account retates to any of the aforementioned home equity lines of credit as asserted by Ms. Cohen. Was the $2,500, an additional amount loaned over and above the $4, home equity line of credit as in account numbers , , and ? We have recently become aware of another loan or line of credit at JP Morgan account number t 2 that I am being held responsible for. Please provide all of the account opening documents, statements, and loan application. Your expeditious response is important. As advised in your letter of April 4, 2011, I am writing to your office within 30 days of receipt of an unsatisfactory response from JPMorgan Chase Bank, Chase Home Lending, Executive Offices (the Bank ), dated April 18, 2011 (see attached). The Bank, has persisted in their letter, in refusing to provide the documents I have requested. As detailed in my previous letter to Mr. Dimon, CEO, and Mr. Cutler, Chief Legal Counsel, JPMorgan Chase Bank, applications in my name for a $4,000, home equity line of credit, as well as an asset/brokerage account, were submitted to the Bank without my knowledge, consent, or authentic signature, by my former business partner and fiduciaries who were employed by same. As a result of these applications, the Bank requested, in order to complete the Bank s due diligence process in considering the intended obligation extensive my personal financial records/information, including, but not limited to, my tax returns, bank statements, trust instruments, articles of organization and formation for LLC s I managed, and financial proformas detailing my assets and debts generally, again, without my knowledge or consent. At no time did any individual from the Bank contact or speak with me directly, either at the private bank responsible for opening the asset account or at Chase Home Lending, responsible for evaluating the home equity line of credit. Ultimately, the Bank approved the home equity line of credit and documents which I have received, were executed, implementing the line of credit, collateralizing my assets. These documents bear forged signatures for me, and are, in several instances, notarized in states in which I can show, by credit card statements and other documents, that I was not present.

14 I have made these facts known to the Bank (Mr. Dimon, Mr. Cutler and Ms. Cohen, Executive, JPMorgan Private Bank). I am seeking my personal financial documents provided to the bank, by the aforementioned individuals without my knowledge or permission. Particularly, the Hawk Mountain Trust and Hawk Mountain LLC documents analyzed in the Bank s credit approval memorandum (see attached). As you can see in their letter of response, the Bank states that no additional documents will be provided unless I get a court order (see attached). They also assert that this letter will be their final communication on the matter, and suggest I seek legal counsel. I fully expected the Bank to forward my personal financial documents provided to them by the aforementioned individuals having been apprised that they may have, unwittingly, been a party to a fraud. I am shocked that the Bank would take an obstructive position in response to my request, given the circumstances. I fully expected not only their full cooperation in assisting me, but a thorough investigation via their fraud division to determine what improprieties may have occurred and to ensure the integrity of their system. The Bank does not seem remotely interested in actions that would comport with integrity or transparency in this matter. I intend to pursue this matter to the fullest and appreciate your assistance and advise in this regard.
